A Minnesota school security guard admitted to authorities on Wednesday that he lied about an active shooting after he accidently shot himself, St. Paul, Minnesota police said in a statement The department reported that Brent Patrick Ahlers, 25, was arrested for filing a false police report. St. Paul Police said that Ahlers told investigators that he was handling his gun when it accidently discharged, sending a bullet through his shoulder. He originally claimed that while working a guard shift at St. Catherine University that he was shot in the woods by a suspect. St. Paul Police said that the incident tied down vast resources, including 55 officers, four canines and a Minnesota State Patrol aircraft. “Last night I was talking to you about an incident that shocked the community,” Sgt. Mike Ernster told the Star-Tribune in a news conference. “It had basically 1,800 students held captive in their dorm rooms at St. Catherine’s, it had residents of the Mac-Groveland and Highland Park communities fearing they would be hurt in their homes.” 1107

A University of Utah track athlete was shot dead on the campus Monday night by a man she reportedly had dated and complained to police about, authorities said.University police found Lauren McCluskey's body in the back seat of a car on campus after her worried mother called police, according to university Police Chief Dale Brophy. McCluskey, 21, apparently had a dispute with the man, identified as Melvin Rowland, 37, university Police Lt. Brian Wahlin said.The shooting led police to lock down the campus for hours while they searched for the suspect. Brophy said someone picked the suspect up from campus after the shooting.Early Tuesday, Salt Lake City Police saw Rowland in downtown Salt Lake City and briefly chased him, Brophy said. Brown said Rowland slipped in to the back door of a church, where he was later found dead from a self-inflicted gunshot."This isn't right," Mark Harlan, the school's athletic director, said Tuesday of the death of McCluskey, a Pullman, Washington, native. 1005

  

A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. "On behalf of the senior leaders at DC VAMC, Veterans Integrated Service Network (VISN) 5 and the Veterans Health Administration (VHA), we concur with OIG's findings and recommendations and provide the attached action plans," the Office of the Undersecretary for Health said in response to the report.The investigation into the Washington DC VA Medical Center, which provides care to almost 100,000 veterans and employs more than 2,000 people, began in March 2017 after a confidential complaint, according to the inspector general's report.The-CNN-Wire? & ? 2018 Cable News Network, Inc., a Time Warner Company.
