Psychological first aid. That's the mission of a new program within the Medical Society of the State of New York. It's called "Peer to Peer" and it's a way for doctors to counsel each other, especially as they try to manage the exhaustion and trauma brought on by COVID-19.Dr. Frank Dowling said the medical profession has been stressful from its onset. “You can find journal articles from 1850 talking about docs and depressions alcohol and suicide," Dowling said. Initiating the "Peer to Peer" program has been a career goal of his, a bucket list item.“Because we’re professionals, we expect ourselves to take all this in and, incorrectly in my view, not feel it and we have a professional demeanor,” Dowling said.Dr. Dowling says doctors are under immense pressure to not show the stress they take on. “Could you imagine a cardiac surgeon working on someone you love, bringing them to the emergency room saying 'OMG, I have a life in my hands?' So it gets pushed aside in its own natural way where people know how to do it and we do what we’ve got to do,” Dowling said.Add in insurance headaches, trauma, and then: 2020. And COVID-19. New York's medical system is still dealing with the stress of the pandemic.“Those stressors don’t stop because we have a pandemic, the pandemic adds to the burden of stressors that already exists,” Dowling said.Dr. Charles Rothberg chairs the Medical Society, State of New York's Committee on Physician Wellness and Resiliency. They've been working on the "Peer to Peer" program because physician burnout is not new. COVID-19 hit, and they knew it needed to get off the ground.“The program is essentially for people that are engaged in a stressful profession that from time to time find that their coping mechanisms are exceeded by the stressors they experience,” said Rothberg.There's been doctor suicides, addiction and financial problems, real life struggles, exasperated by the coronavirus. For doctors, there's often a stigma associated with the stress. “There was concern that physicians would not want to consult a peer for fear that they would be reported or trigger an obligation of a colleague to do the reporting,” Rothberg said.Peers, he says, should provide safety, comedy, a connection - and basic support. “A physician should know that they are doing a good job even if sometimes unexpected outcomes occur for example,” Rothberg said.Before they launched "Peer to Peer", they were working on ways to prevent and reduce doctor burnout. Ironically, that program was halted because of COVID-19. Which, then of course, made the burnout even worse."Peer to Peer" offers a number to call- 1-844-P2P-PEER and an email, p2p@mssny.org, to contact, 24 hours a day, 7 days a week, not for treatment, but casual conversation, to talk it out with someone who's going through the exact same thing. “Maybe to just share with a peer that this happened to me also and here’s how I responded to it a lot of the problems that people have are common to each other,” Rothberg said.Or, as Dr. Dowling puts it, head over to the zoom diner, meet a friend and take a load off. “I want the docs that we help in the peer to peer to get well, to get better, to love life again and love why they went into medicine," said Dowling. 3263

Researchers from Florida Atlantic University released a video on Tuesday that shows how droplets disperse into the air from a person’s mouth.The researchers used different kinds of masks to analyze the disbursement of the droplets. The researchers also showed that without a mask, six feet of distance might not be enough to avoid infection.The FAU team tested a single-layer bandana-style covering, a homemade mask that was stitched using two-layers of cotton quilting fabric consisting of 70 threads per inch, and a non-sterile cone-style mask that is available in most pharmacies. The team used a mannequin, Laser lights and synthetic fog to help visualize the spread of the droplet from a cough..The researchers said that stitched quilted cotton masks worked the best in the simulation, with droplets from emulated coughs traveling just 2.5 inches from the mask. By comparison, without a mask, the droplets moved more than 8 feet, with some droplets traveling up to 12 feet during a heavy cough. The researchers also noted that the droplets remained suspended in midair for up to three minutes in the simulated environment.“We found that although the unobstructed turbulent jets were observed to travel up to 12 feet, a large majority of the ejected droplets fell to the ground by this point,” said Dhanak. “Importantly, both the number and concentration of the droplets will decrease with increasing distance, which is the fundamental rationale behind social-distancing.”FAU said that droplets from a cough traveled 3 feet, 7 inches from a bandana; 1 foot, 3 inches from a folded handkerchief; and 8 inches from a cone-style mask.“Preliminary results from our experiment reveal that significant concentrations of small particles from a turbulent jet such as from a heavy cough/sneeze can linger in still air for more than one minute. It only took the particles a couple of seconds to travel 3 feet; in about 12 seconds it reached 6 feet and in about 41 seconds it reached around 9 feet,” said Siddhartha Verma, an assistant professor in FAU’s Department of Ocean and Mechanical Engineering.Last week, the University of Washington’s Institute for Health Metrics and Evaluation projected that 33,000 American lives would be saved between now and October 1 by near universal wearing of masks.Last month, a study in the Lancet found that the use of masks and respirators by those infected with the virus reduced the risk of spreading the infection by 85%. The authors analyzed data that showed that N95 respirators in healthcare settings were up to 96% effective. Other masks were found to be 77% effective.The CDC issued guidance in April to recommend mask wearing while in public, which has become a requirement to enter retail establishments in more than a dozen states.The CDC's guidance stressed several points: That wearing a face covering is not a substitute for social distancing, and that the public should not use medical or surgical masks. President Donald Trump's former attorney and "fixer" Michael Cohen said he is "done being loyal" to his former boss and admitted that he knew what he was "doing was wrong" when he arranged payments during the 2016 election to silence women who claimed to have had affairs with Trump."I'm done with the lying," Cohen said in an interview with ABC News' George Stephanopoulos that aired Friday. I myself have held the hand of dying patients who were crying out for their families that they can’t see.
