PHOENIX (KNXV/KGTV) - A small plane that crashed on a Phoenix street Monday morning -- killing both passengers  -- was just sold from a pilot in Solana Beach.The victims have been identified as 54 year-old Theodore Rich and 49 year-old Elaine Carpenter. FAA officials said the aircraft involved is a single-engine, home-built Acroduster. The plane went down in the intersection of 7th St. and Deer Valley Dr., near an FBI field office, at around 6:50 a.m. local time.One eyewitness said he saw the plane go down. At first he thought it was aerobatics, but after seeing the plane heading straight for the ground, he knew something was wrong. Video from the scene showed emergency crews working around debris in the roadway near several damaged cars. Police told media on scene the vehicles had substantial damage but no one on the ground was hurt.10News reporter Lauren Davis talked to Avi Maltzman, the Solana Beach man who just sold the plane to Rich this weekend. Maltzman said he flew with Rich for several touch-and-goes and described Rich as an experienced pilot. Maltzman said Rich even called him from Arizona just to let him know he made it home safely. “He impressed me very well with his flight skills. He did good eight or nine landings. He was comfortable and I felt comfortable with him," said Maltzman.Maltzman said the plane was built by professionally in Santa Barbara in 1993. Maltzman bought the plane in 2011 and said it was in great condition. Those who knew Rich said flying was his passion. They said Rich died doing what he loved.    1614

PHOENIX, Arizona — Aaron Wallace was sleeping when he was attacked in his bed.The patient at the Arizona State Hospital was stabbed by Reuben Murray, a fellow patient with a murderous past, who was wandering the halls unsupervised in the middle of the night with a sharpened pencil, according to a recent lawsuit filed against the state.The lawsuit raises new questions about safety and security inside Arizona’s only public psychiatric hospital and whether conditions have improved after sweeping changes and promises were made following an extensive KNXV television station three years ago.  The lawsuit also alleges warnings about Murray’s aggressive behavior were ignored in the days before the stabbing.“The place is not better,” said Josh Mozell, an attorney representing Wallace. “Things have not changed. PARADISE HILLS (KGTV) -- A Paradise Hills nursing home sees a surge in COVID-19 cases, worrying the families of those who live in the facility.According to the administrator of Reo Vista Healthcare Center, 71 residents and 28 staff members have tested positive for COVID-19. One resident passed away. State data from Monday shows Reo Vista Healthcare Center with the highest one-day positive patient total in the county.“Eight residents are currently hospitalized in stable condition. The remaining residents who tested positive for COVID-19 are isolated at the facility with mild symptoms,” said Curtis White, Reo Vista Healthcare’s administrator.White added that four employees have recovered and the remaining who have tested positive are also self-isolating with mild symptoms.Cesar Noriega’s 90-year-old mother is currently staying at Reo Vista. She has also tested positive for the virus. “I was terrified,” he told Team 10.“It’s a staggering number by any measure,” said Mike Dark, an attorney with California Advocates for Nursing Home Reform. He is not affiliated with Reo Vista Healthcare.Dark said California rules require nursing homes to come up with testing plans to cover all residents and health care workers. “The problem has been an execution of those plans. It’s been spotty at best,” Dark said.Another concern at nursing homes is the staff, which typically consists of certified nursing assistants. He said many often work at multiple places due to low pay.“When the virus comes into a facility and they’re exposed to it, they can carry it unwillingly into another facility that they’re working at,” Dark said.Noriega said he likes Reo Vista, but he needs to think of his mother’s health. “What happened with the outbreak there, it’s tragic. It’s unfortunate, so I need to get my mom out. I can’t leave my mom there,” Noriega said.The facility currently holds about 135 residents and 150 staff members, according to White. He said a baseline COVID-19 test was conducted on May 28th and a second round of testing occurred between June 16 and June 18. Police in Germantown, Wisconsin are looking for a bathroom creeper who took pictures of a woman while she was using the restroom. It happened Monday afternoon at the McDonald’s on Mequon Road. “While she was in the stall she saw two arms holding some sort of recording device,” Police Chief Peter Hoell said. That’s when the startled woman ran out of the bathroom after the suspect. The restaurant’s manager saw the commotion and ran out of the building behind the woman, they both saw the suspect get into an old RV and take off. “The vehicle should stick out,” said Hoell. Surveillance cameras captured images of the man and his getaway vehicle. Because the man was driving an RV, the chief said he believes he may be from out of town. Hoell hopes that social media will help in the investigation. “Depending on the interest, that post might go to multiple states, we may have an opportunity to identify this person even if he is not from Wisconsin,” said Hoell. 1002
