A train carrying dozens of Republican members of Congress, including House Speaker Paul Ryan, to their legislative retreat in West Virginia hit a truck on January 31, 2017 leaving one person dead.Wednesday the National Transportation Safety Board released a preliminary investigation into the crash.They said around 11:15 a.m., a 2018 Freightliner truck equipped with a McNeilus Truck & Manufacturing refuse body was traveling southbound on Lanetown Road in Albemarle County near Crozet, Virginia.The refuse truck, operated by Time Disposal LLC, had a 30-year-old driver and two passengers as it drove towards a highway–railroad grade crossing, later identified. The crossing is active and includes advance warning signs and pavement markings on its approach, the National Transportation Safety Board said. The crossing is also equipped with crossbuck signs, warning lights, bells and gates.As the truck neared the highway a westbound Congressional Special Amtrak train was approaching the highway–railroad grade crossing.The NTSB said the train’s lead locomotive was equipped with a forward-facing track image camera. Data that was taken from the camera showed that as the crossing came into view, the gates were down and the truck was on the grade crossing. Witnesses to the crash reported that the refuse truck entered the crossing after the gates were down.The train’s recorder showed the Amtrak train was traveling about 61 mph when the engineer applied emergency braking. The train struck the left rear of the refuse truck, causing the truck to rotate counterclockwise and then collide with a railroad signal next to the tracks. The refuse body separated from the truck, and the truck’s two passengers were ejected, according to the NTSB.As a result of the crash, one passenger in the truck died. The other truck passenger had serious injuries and the driver of the truck had minor injuries. Three Amtrak crewmembers and three train passengers sustained minor injuries, the NTSB said.NTSB investigators and Albemarle County Police Department officials and the Federal Bureau of Investigation documented the crash scene and the characteristics of the crossing, the train and the refuse truck. The track and operational characteristics of the crossing signals were also examined and documented.The crash remains under investigation. 2354
A scary moment happened for parents and young kids when a fight broke out at the Lied Memorial Boys and Girls Club near Lindell Road and Edna Avenue in Las Vegas.It's not clear yet how it all started but from the video, it appears that a number of adults were involved. But what concerns parents most is there were a lot of young children, even babies in the audience when the fight started. What seemed like a heated argument turns into a fistfight within seconds. People were throwing wild punches, shoving and tackling each other as others tried to break the fight.Scripps station KTNV in Las Vegas spoke with a parent who was at the Lied Memorial Boys and Girls Club. Concerned with his kid's safety, he did not want to be identified."It was scary you know just to know that all those kids around, and how fast the situation escalated... just one second everybody was having fun and the next second it was chaos," the parent said. "It was a 'think fast' moment because the kids were close. More than anything it was just getting the kids out of the way and then figuring out what was going on."KTNV reached out to Boys and Girls Club and the National Youth Sports League which oversees the game.The League responded saying it is still "gathering facts" saying, "If found that any of the individuals were involved in our league they will be immediately removed and we will press charges. This is in no way the kind of behavior that should ever happen at a child's game."Parents also told KTNV they're disappointed. They said as adults, people should be able to control our anger. 1610
A second report in as many days is predicting an “extremely active” hurricane season.The National Oceanic and Atmospheric Administration released their annual August update for the Atlantic hurricane season Thursday. They are predicting 19 to 25 named storms, where 7 to 11 of them become hurricanes and 3 to 6 of those become major hurricanes with winds of 111mph or greater.Hurricane season ends November 30.There have already been nine named storms in 2020, the earliest that has happened since historical records began. Hurricane Isaias was also the fifth storm of the season to make landfall. It’s the fastest we’ve gotten to five land-falling storms since the old record set back in 1916.Historically, according to NOAA’s Climate Prediction Center, only two named storms form on average by early August and the ninth storm typically does not form until October.“This is one of the most active seasonal forecasts that NOAA has produced in its 22-year history of hurricane outlooks,” said U.S. Secretary of Commerce Wilbur Ross. “We encourage all Americans to do their part by getting prepared, remaining vigilant, and being ready to take action when necessary.”On Wednesday, Colorado State University’s Tropical Weather and Climate Research Center released their final predictions for the 2020 hurricane season. They believe there will be roughly 24 named storms, and 12 hurricanes.“The primary reason why we’re forecasting such an active season isn’t as much the storm activity that we’ve already had, but the large-scale conditions we’ve observed during July and what we expect to see during the peak of the season,” said Dr. Phil Klotzbach, from Colorado State University.NOAA says current oceanic and atmospheric conditions are making 2020 an active storm year, including warmer-than-average sea surface temperatures, reduced vertical wind shear, weaker tropical Atlantic trade winds and an enhanced African monsoon.Both groups say their predictions are for overall seasonal activity and do not predict landfall. A report from the Tucson Police Department is revealing new details about the death of Carlos Adrian Ingram-Lopez while in TPD custody in April.The police department began its investigation into the incident hours after it happened on April 21. Nearly two months later, on June 19, the department finished its report and handed it off to TPD Chief Chris Magnus. After a news conference about the incident Wednesday, TPD released the full report to members of the news media.TIMELINE: What happened after Carlos Ingram-Lopez died while in TPD custodyThe report recommends termination for officers Samuel Routledge, Ryan Starbuck, and Jonathan Jackson, who had resigned the day before the investigation was completed.The discipline report focuses on how officers are supposed to treat someone in a state of “excited delirium," how it greatly increased the risk of dangerous physical distress and how the three officers failed to meet their standards and training.The report draws a number of conclusions about the officers involved in the incident. It says:The initial report of Ingram-Lopez's behavior should have prepared the officers to deal with excited delirium before they even saw him.Ingram-Lopez's behavior at the scene made excited delirium very clear.The report documents dates of when the officers had training on excited delirium at the academy and in other training sessions after that.Excited delirium and the likelihood of drug intake make overheating and rapid heartbeat something officers should anticipate.The fact that he was calling for water confirms they should have been more aware of his physical distress.The officers were trained on, and should have been alert to, signs of breathing trouble, like wheezing, and simply saying “I can’t breathe." Ingram-Lopez did both.The officers had been trained on the “recovery position” designed to reduce physical distress on a restrained suspect.One of the officers who arrived later said within 15 seconds, “Shouldn’t he be in the recovery position?” That officer is not being disciplined.The officers put a “spit sock” over Ingram-Lopez's face because of his choking and clearing his throat made them fear he would spit and spread COVID-19. The spit sock was available to officers even before the COVID outbreak.While officers did not use prohibited methods like neck holds, they noted Ingram-Lopez was a large man and one of the officers kneeled on his back for a sustained period.Officer Jonathan Jackson was Lead Police Officer -- slightly more senior than the other officers who first arrived at the scene. He was expected to take command and organize the other officers. The report says he failed to command adequately and organize the police response.Other officers either reacted appropriately or were with the grandmother, where they were not well aware of what was happening with Ingram-Lopez.Overall, the report concluded the officers ignored their training and were unaware or indifferent to Ingram-Lopez's situation and physical distress.Click here to read TPD's full report.KGUN's Craig Smith first reported this story. 3106
A Southwest Airlines plane flying from Fort Lauderdale to BWI was hit by a pickup truck after landing at the airport. It happened early Monday morning. Passengers said the white truck crashed into the side of the plane on the runway. An ambulance was at the scene, but according to passengers, no one was injured. Passengers were not allowed to leave while crews continued their investigation. Southwest Airlines has not released any comment but the company did respond to a tweet from one of the passengers on the plane. A passenger named Michael Simon posted this tweet: "So our southwest flight just got into an accident with a pickup truck. Guess that makes me an airplane crash survivor." 874
