With health care costs on the rise, a growing number of Americans are throwing out the old way of seeing a doctor and turning to a membership model. A monthly or annual fee gets you direct access to a doctor, no insurance needed.Twenty years into her career, bogged down by red tape, too many patients and long days, Dr. Shaila Pai-Verma was looking for a better way to practice medicine.“I was just miserable,” she said. “The joy of medicine is gone and then you're just doing paperwork.”So, a year ago, she started a new primary care practice with a new business model.“The patient basically has a direct contract with the physician and they take insurance companies out of it,” she explained.Patients pay a flat monthly or yearly fee. In exchange, they receive a broad range of primary care services and quick, unlimited access to their doctor via in-person office visits, phone or by text.“Everyone wants everything immediate. And so, I think this is it. It's good, especially in this time for people to have access,” said Pai-Verma.Membership fees range from about 5 to 0 per month on average – about 0 less than having typical health insurance. Most patients still carry catastrophic coverage for emergency treatments and hospitalizations, but that insurance is usually only -100 a month, so patients still save money.For Bonnie Micheli and her family, it was all about access.“With this, it's just so much easier to just know that I can contact directly here within a few hours for any issues that I'm having,” said Micheli.In late September, a bipartisan proposal was introduced in Congress that would expand access to the model and allow people to use their health savings account for direct primary care (DPC).Because they see fewer patients than traditional practices, some critics say the model could worsen the shortage of primary care physicians, a trend that’s already driven by burnout.But according to a recent study, DPC members had 25% lower hospital admissions and the cost of emergency room claims was reduced by 54%.“There's less ER visits and you know, better health care for the patient,” said Pai-Verma.While there is still debate, for a growing number of Americans, like Micheli, it’s becoming a simplified health insurance alternative.“Honestly, it’s just so nice to know what I'm paying every month or if you do the annual, what you're getting for that money, and you know exactly who to go to when you have a problem.” 2467

Wisconsin Gov. Tony Evers and the Department of Health Services issued a new order Tuesday limiting the capacity of bars, restaurants and stores in the state to 25% on Tuesday, as Wisconsin becomes a hot spot for COVID-19 in the U.S.Evers directed DHS Secretary-Designee Andrea Palm to issue the order Tuesday afternoon.The order is capping capacity to 25% in public spaces beginning on Thursday. Gatherings in indoor spaces without an occupancy limit are limited to 10 people."We're seeing increased COVID-19 hospitalizations across the state, and every region is reporting current and imminent hospital staffing shortages in one or more of their facilities," Palm said.Wisconsin has become a hot spot for coronavirus cases in the country after schools reopened this fall. The state has tallied record numbers of cases, hospitalizations and deaths in recent days. Areas that have been hit particularly hard include the state's two largest cities — Milwaukee and Madison — as well as smaller cities like Fox Valley, Green Bay and Wausau.The governor's top attorney, Ryan Nilsestuen, says he's confident the restrictions will withstand legal challenges from the GOP-controlled legislature. Evers' "Safer at Home: order issued last March was eventually blocked by the Wisconsin Supreme Court, after the legislature raised objections with a potential overreach of authority by the health department and the governor's office.“At the end of the day, doing orders and having them challenged in court maybe makes for great theater,” Evers said during a press conference Tuesday. “But it frankly does nothing in making sure people are safer in the state of Wisconsin.”The order goes into effect at 8 a.m. local time on Thursday and will remain in effect until Nov. 6. Violators could face forfeitures of up to 0, according to the governor.The order does not apply to outdoor spaces.The DHS tallied 2,020 new cases on Tuesday and 18 new deaths caused by complications from the coronavirus. Wisconsin has recorded 136,379 confirmed cases and 1,399 deaths since the pandemic began.“We’re in a crisis right now and need to immediately change our behavior to save lives,” Evers said in a statement. “We are continuing to experience a surge in cases and many of our hospitals are overwhelmed, and I believe limiting indoor public gatherings will help slow the spread of this virus. Folks, we need your help and we need all Wisconsinites to work together during this difficult time. The sooner we get control of this virus, the sooner our economy, communities, and state can bounce back.”Criticism to the news was swift. State Rep. Shae Sortwell, R-Two Rivers, went after the new orders in a Facebook video."Apparently he’s bound to determine to just completely destroy the hospitality industry in the state of Wisconsin because that’s what he’s looking to do here," Sortwell said.The Wisconsin Manufacturers & Commerce issued a statement in response to the new order."Unfortunately, Gov. With California hospitals dealing with an extreme number of patients, nurses are terrified of what’s next.It comes as California hits 2 million infections just 44 days after reaching 1 million. Hospitals in California have been pushed to the brink, and it’s expected to get worse as people travel and gather for Christmas and New Year’s.The state is urgently searching for 3,000 temporary medical workers to meet the demand, focusing on nurses trained in critical care. Hospitals are also confronting the shortage by trying to free up staff any way they can, including postponing certain medical procedures.The state has also temporarily loosened some restrictions. Typically, California requires one nurse for every two ICU patients. Regulators have temporarily relaxed that requirement to one nurse for every three ICU patients.The state has sent more than 600 temporary healthcare workers to hard-hit counties from the National Guard, the California Health Corps, and other partnerships, but officials are still looking for more.State officials have even started reaching out to other countries like Australia and Taiwan to get much-needed medical workers.Elected leaders and health officials across the U.S. are asking people to stay home for the holidays while also trying to show the public that the COVID-19 vaccines trickling out to health care workers and nursing home residents are safe.The Associated Press contributed to this report. 1453

  

When COVID-19 forced businesses to reduce capacity, restaurants had to get creative. Many opened up outdoor space to dine.Now, with winter coming, restaurants are scrambling to figure out how to keep outdoor spaces comfortable.“We won a James Beard award here. I think what we’re known for is amazing food and service and great wine,” said Chef Jennifer Jasinski, owner of Crafted Concepts Restaurant Group. Rioja is one of the restaurants.“We’re just trying to plan. Hope for the best, plan for the worst,” Jasinski said.Like many restaurants across the U.S., Rioja is struggling with bringing business back and following physical distancing guidelines.“The order that let us increase our patio at each of our restaurants was a really big help over the summer. It got us to about 65 to 70% of our prior year occupancy,” Jasinski said.Outdoor space has been the saving grace for many.The National Restaurant Association Research Group conducted a survey of more than 3,500 restaurant operators from August 26 to September 1 nationwide.“Places that have table service, approximately three fourths are currently utilizing outdoor space which is a very large percentage,” said Mike Whatley, vice president of state and local affairs for the National Restaurant Association. “And it's making up an average of 44% of daily sales, which is very high by historical standards. Almost half of their money currently is coming from outdoor dining.” But outdoor space may present some challenges in the coming months.“Equipment for outdoor dining, especially when it gets cold, it gets expensive,” Whatley said. “What we found is that the average restaurateur is hoping to utilize their outdoor space two months more from this point moving forward.”“It’s absolutely terrifying. I feel like it’s 'Game of Thrones' and winter is coming and I’m scared. It’s not white walkers, I’m just scared of the occupancy,” Jasinski said. And she’s not alone.Many restaurants across the country are facing the same battle, and searching for solutions. The City of Chicago is reaching out to help by hosting a Winter Design Challenge, inviting the community to reimagine the winter outdoor dining experience for a prize.It’s similar to how Larimer Square in Denver, Colorado, where Rioja is located, was re-imagined, by letting the community have a say.“It wasn't just about the seating capacity, it was about the experience itself of being on the street and I mean really, it’s worked,” Jon Buerge said. Buerge is the Chief Development Officer at Urban Villages, the property management company in charge of Larimer Square, a prime example of the “Street-ery” concept.“This 'street-ery' concept is a new one where you block off a street in an urban area, and allow restaurants to take over some of that space. We’re seeing that in a number of jurisdictions,” Whatley said.“Some of the things that we've been exploring are tent systems that could be heated and, on a nice day, you can roll up the walls,” Buerge said. He said solutions really depend on the place.“Seattle has been slower to reopen. The people in Seattle have been less willing to go out and restaurants have been closed for longer. So it depends on the market, it depends on the weather, it depends on whether the people in these cities are ready to come out,” he said.As temperatures drop, restaurants are weighing the benefits and costs of seating diners outdoors.“There's all these hoops we have to jump through for less dollars,” Jasinski said. “I haven't heard of anybody else having any brilliant ideas that don't cost a ton of money.” 3588
