It has been nearly a month since 20 Democrats met for the first debate to vie for the party’s nomination in next year’s presidential election. On Tuesday and Wednesday, 20 Democrats will once again take the stage, this time in Detroit. What is differentWhile there will be 20 Democrats on the stage -- 10 each night -- one Democrat has since dropped out of the race. Rep. Eric Swalwell of California ended his bid for the presidency earlier this month. His spot on the debate stage has since been filled by Montana Gov. Steve Bullock. With this debate being aired on CNN, there will be a different set of moderators. Don Lemon, Dana Bash and Jake Tapper will conduct the debate. Unlike last month’s Miami debate, CNN has told the candidates that there will be no “show of hands or one-word, down-the-line questions.” Also different, CNN has threatened to reduce the time of candidates who are constantly interrupting. The requirements to qualify for the debate also remained the same, and was based off of polling and fundraising criteria.Biden back as frontrunnerVice President Joe Biden’s lead took a bit of a hit in the days following the last debate thanks to a contentious confrontation from Sen. Kamala Harris of California. Harris’ emotional rebuke of Biden’s stance on public school busing in the 70s was easily the most memorable moment of the night. It also seemed to have, at least temporarily, cut into Biden’s lead. Polls from CNN and Quinnipiac University had Biden’s polling as low as 22 percent. Harris, for her part, saw her numbers increase to above 10 percent. Biden has seen his polling number return to near 30 percent. Harris, however, has continued to poll above 10 percent, indicating that she perhaps draw some support from some of the other candidates. Biden and Harris are the two center-stage candidates for the July 31 debate. But Harris might not be Biden’s biggest concern on Wednesday. Sen. Cory Booker and Biden have been involved in a spat in recent days over criminal justice reform. The Internal Revenue Service requires anyone who owes the government money in their taxes to pay by midnight, even if they file for an extension. With that rule in mind, a lead tax research analyst at Kansas City-based H&R Block said it’s easier to complete taxes today instead of pushing it back another six months. "If you're going to have to go to the trouble to actually figure out what you owe, you might as well file your return, get it over with," said Nathan Rigney, a research analyst. "And you may discover in fact you get a refund." He added most Americans are seeing a smaller tax return, or owe the government for the first time, but they received more take-home pay during the year. To avoid having to pay next year, Rigney suggested looking at your withholding. It's the amount of money your employer keeps from your paycheck to pay the government on your behalf. You can fill out a W4 to change your withholding. Depending on your changes, come this time next year, you may be getting a check instead of writing one. "[If you don't, the amount you owe] might even be a little bit worse just because of the way the withholding tables were implemented last year for only half of the year. This year, it will be for the full year," Rigney said. If you recently waited in a crowded doctor’s office or you’ve called to make an appointment and were told the next slot available is in several weeks or months, you’ve already experienced the effects of America’s doctor shortage.It's become more common for doctors, like New Jersey urologist Dr. Thomas Mueller, to practice with a packed patient schedule."The amount of patients we see is borderline insane," Mueller says.“I'll be the first one to say I don’t think it’s the best thing in the world," he says. "The things that I do to combat it is I just invest a lot of time beforehand.”Mueller and the team of physicians at Delaware Valley Urology each see upward of 50-60 patients a day.And that’s still not enough. "With the baby boomers becoming, you know, in their 70s, there are a lot of people to be seen," the doctor says. "The overall structure of medicine, at least as far as training is concerned, they’ve never really increased the enrollment in medical schools.”Unless significant steps are taken, the Association of American Medical Colleges predicts the shortage is only going to get worse.“I think I am at my max (amount of patients)," Mueller says. "I don’t think I can do a whole lot more."To help with the issue, legislators are proposing several bills that would raise grant money for more medical residency slots, and to make it easier for foreign doctors to practice in the U.S.In addition, medical schools have increased scholarships. Some have even created specific residency slots for those willing to practice in rural areas.“There are folks who think that there is a shortage," says Dr. Bob Motley. "I think we have as much of a problem with the maldistribution.”Motley runs Thomas Jefferson University's Physician Shortage Area Program. “We have about 50% of all physicians in Pennsylvania that are actually clustered in three counties," he says. "But 75% of Pennsylvanians actually live outside those areas.”Motley’s program has graduated roughly 400 doctors, and almost 80 percent are now practicing in rural communities hit the hardest by this doctor shortage."There's a lot to be learned in health care and we definitely have not figured it out," Mueller says. "It’s not a broken system by any stretch of the imagination but it’s things that are ever changing. And I think everyone is striving to make it better.”In addition to seeing 50 to 60 patients a day, Mueller also trains residents to handle the patient load as it is now."It's not for the faint of heart," he says.
