A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. A suspect has been arrested and the gun and mask have been recovered after an incident at the Boulevard Mall in Las Vegas, according to police. The gun appears to be a fake gun, according to police. The identity of the suspect will be released Friday morning.  Police said they received calls about a person with a gun at the Boulevard Mall on Maryland Parkway. No injuries were reported and police say no shots were fired. A new poll finds broad support for special counsel Robert Mueller's investigation into President Donald Trump and his campaign.Almost 7 in 10 (69%) of respondents in the ABC News/Washington Post poll said they support the special counsel's probe into potential collusion between the Trump campaign and Russia. Sixty-four percent said they support looking into Trump's business dealings. Nearly 6 in 10 (58%) said they back the special counsel investigating allegations that Trump paid hush money to women who alleged affairs with him.The polling took place from April 8-11 -- meaning it took place largely in the aftermath of Monday's FBI raids of Trump personal lawyer Michael Cohen's home, office and hotel room. FBI agents removed Cohen's computer, cell phone, business files and financial documents, and also sought communications related to the "Access Hollywood" tape that captured Trump making lewd remarks about women, sources familiar with the matter have told CNN.The poll results come amid mounting concerns about the future of the Mueller investigation. The President and his aides have discussed firing Mueller for months and believe they have the power to do so, according to a source. A Nashville high school football coach has been arrested on a charge of soliciting an underage student.According to an affidavit from Metro Nashville Police, 25-year-old Weldon Garlington messaged a 17-year-old girl on Instagram asking her for sex. His Instagram profile states he is a football coach and associate dean of students at RePublic High School, a charter school near Brick Church Pike in north Nashville.Garlington was arrested late Thursday night after he allegedly confessed to police about having sent the messages.The conversation reportedly started with private Instagram messages to her over the weekend, asking the girl when she would turn 18-years-old. He also asked if she wanted to do anything with him "like sex lol." He later offered to reimburse her gas money to drive to his apartment. The affidavit claimed, the next day, he texted her saying, "My bad about all this... Let's just act like none of this was ever talked about."Garlington was booked into jail on 0,000 bond and was charged with solicitation of a minor.
