SAN DIEGO (CNS) - County supervisors voted 4-1 today to have the chief administrative officer direct the Health and Human Services Agency to develop a plan for in-house medical care of jail inmates and possible outsourcing of those duties as well.Sheriff Bill Gore will review the plan, and officials will also put out requests for proposals from private contractors to handle those duties.Supervisor Dianne Jacob made the motion to examine both an in-house model and outsourcing.``I don't think we should go with just one approach,'' Jacob said, adding she is a proponent of outsourcing county services, if it results in good service for less money.Gore has cited his department's million annual health care bill for inmates as a reason to explore cost-saving strategies. He asked supervisors to consider possible vendors for the county's health care needs in its jails and other facilities.Gore told supervisors that his goal was simply to get the best care at the best value, with an aging population of inmates and other serious issues.``I am proud of our response to these challenges, but I'm not one to assume that I have all the answer to these challenges,'' Gore said.Gore also objected to waiting 180 days for a staff appraisal of outsourcing. ``If it's a good idea in sixth months, it's a good idea today,'' he added.He also praised the 300 men and women who provide medical care to inmates as outstanding employees.Supervisor Nathan Fletcher, who is opposed to any privatization of services and proposed having HHSA handle inmate care, cast the lone no vote.Before the board voted, Fletcher said outsourcing to a for-profit entity would not lead to a better outcome.``As a board, we are the ones who are on the hook,'' in terms of lawsuits if an inmate doesn't get the appropriate care, Fletcher said. ``We should have input.''Fletcher asked the board to vote on his proposal to have the HHSA administer medical and behavioral health services in jails, but the motion died for lack of a second.Board Chairman Greg Cox said he was encouraged that HHSA would develop its own proposal.Cox praised both county medical employees and contractors for working in such a demanding field, but said the system right now is too bifurcated, adding that several legal items on Tuesday's closed-session agenda involved inmate care.Supervisors voted after a lengthy public hearing, in which nearly every person who called was opposed to private outsourcing. (Expletive) you for putting my life in danger," the prosecutor said.When officers responded to Davies' sixth-floor condo near the roof of the complex, a shot rang out from behind her door and an officer returned fire, according to Runyon.The prosecutor said officers retreated and more shots were fired at them and members of a SWAT team.Hours later, police observed a rifle, revolver and a magazine for a rifle being thrown out of the window where Colbert was holed up, Runyon said.After Colbert's arrest, officers found 17 expended casings and another rifle under a pile of clothes in the room, the prosecutor said.Even though traces of cocaine and methamphetamine were found in Colbert's system, and even though he had a history of mental health issues, the defendant knew what he was doing that day, Runyon said.Defense attorney Melissa Tralla told the jury that Colbert had a psychotic break and was paranoid and delusional. She said that days before, Colbert had called Davies at least 50 times and was not acting right.Colbert was rambling, saying he had special powers and was part of the New World Order, Tralla said.The attorney said Colbert was showing signs of mental illness by age 6, and was diagnosed with schizophrenia by age 15. SAN DIEGO (CNS) - A parolee who allegedly stabbed a pair of transients -- one fatally -- in two separate Midway District attacks on Thanksgiving Day was charged Friday with murder, attempted murder and assault with a deadly weapon. SAN DIEGO (CNS) - An outbreak of leptospirosis, a contagious bacterial disease, has been reported in dogs in the region, the San Diego County Health and Human Services Agency announced Thursday.Since October, 34 confirmed and probable cases of leptospirosis have been documented among local dogs, primarily in the Hillcrest and Mission Hills areas. Several dogs required hospitalization and at least one was euthanized.Boarding at kennels that allow dog-to-dog interaction is a major risk factor, though some ill dogs only had contact with other dogs at dog parks. The kennels that were linked to cases notified the owners of all potential contacts of diagnosed cases and closed for at least two weeks for cleaning and disinfection.Signs of leptospirosis in dogs are varied and can include fever, muscle weakness, vomiting, diarrhea, loss of appetite, low energy and frequent urination, which may be bloody.No human cases have been linked to this outbreak in dogs, but anyone who develops fever, headache, muscle aches -- especially in the back and calves -- or other illness after contact with a sick dog should see their health care provider.This week, the county sent a health advisory to local physicians and veterinarians about the outbreak.Leptospirosis is a disease caused by various species of spirochete bacteria called leptospira. These bacteria can infect multiple species of mammals, including humans, dogs, rats, mice, raccoons, skunks and opossums. The bacteria are shed in the urine of infected animals and may contaminate food, water, soil or bedding. Dogs and people can get infected through direct contact with skin abrasions, mucous membranes or by drinking contaminated water."When dogs visit kennels and parks and play, lick and sniff each other, they are at risk of getting these bacteria," said Dr. Wilma Wooten, the county public health officer. "If your dog shows any signs, you should take it to your vet immediately."A vaccine is available to prevent leptospirosis in dogs, especially for those boarding in kennels and frequenting dog parks. 2076
