Allergan has recalled almost 170,000 sample packs of birth control pills because of a packaging error that could put consumers at risk for unintended pregnancy.According to the recall, four placebo capsules were placed out of order in a sample pack of Taytulla and the first four days of therapy had four non-hormonal placebo capsules instead of active capsules."As a result of this packaging error, oral contraceptive capsules, that are taken out of sequence, may place the user at risk for contraceptive failure and unintended pregnancy," The recall states. "The reversing of the order may not be apparent to either new users or previous users of the product, increasing the likelihood of taking the capsules out of order."Allergan says they are notifying customers by recall letter and is arranging for return of all recalled sample pack product with the lot #5620706 Exp. May 2019. ALPINE, Calif. (KGTV) -- A man has died following a deputy-involved shooting in Alpine Monday afternoon.  The man's family later identified him as Daniel Ayala. According to the San Diego County Sheriff's Department, deputies shot Ayala at an apartment complex on the 2600 block of Alpine Boulevard around 3 p.m. during a confrontation.No deputies were injured, sheriff's officials said.According to Lt. Michael Blevins, dispatchers had received several 911 calls reporting a man yelling in the apartment complex. Blevins said at one point he made statements threatening to kill people. A witness told 10News, Ayala had pounded on his door, yelling incoherently.When deputies arrived they say they went up to Ayala's apartment."He exited the apartment with a knife in his hand and advanced towards the deputies," said Lt. Blevins.That's when the deputies opened fire. Blevins said more than one deputy fired a gun, though he said they were still investigating exactly who discharged their weapon and how many shots were fired. 1064

(CNS) - San Diego County officials opened an erosion control center in Alpine Thursday to help businesses and homeowners prevent debris flows in the areas burned by the Valley Fire.The center at 2914 Tavern Road will offer free sandbags, fiber rolls and other items to help residents stabilize properties before rains arrive and potentially trigger mudslides and debris flows, county officials said.Starting Thursday, the center will be open to business and property owners from 8 a.m. to 4 p.m. daily Monday through Saturday, and from 9 a.m. to 2 p.m. on Sundays. Appointments are not required this weekend, but will be required starting on Monday.Residents and business owners can schedule an appointment to pick up erosion control materials by calling the county's stormwater hotline at 888-846-0800.Staff will be available to advise on how to protect properties, homes, garages, sheds and other structures. Staff can also visually inspect properties and offer information on best practices to protect structures.As of Wednesday morning, the Valley Fire remained at 17,665 acres and was 90% contained, according to a Cleveland National Forest incident page.The conflagration, which broke out early on the afternoon of Sept. 5 off Spirit Trail and Carveacre Road in Japatul Valley, ultimately destroyed 30 residences and 31 outbuildings, damaged 11 other structures and left three firefighters injured, according to Cal Fire.Jamul resident Jim Grigsby told ABC 10News, “The fire burned down three of my neighbors' houses. And I’m looking out through my window now and it’s kind of sad ... We’ve been through three fires out here, and this is the first time it got this close so we had to get all of this stuff out.”On Thursday, Grigsby went to the erosion center to pick up “fiber coils or swaddle to divert water and use them to fil up the sandbags and build a retaining wall to the gullies on the property." 1934

Actor Verne Troyer died Saturday at age 49, TMZ reported. Troyer played Mini-Me in the "Austin Powers" movies. Troyer died on Saturday afternoon, his family said in a statement to TMZ. The cause of death was not reported. Troyer had been treated for alcohol poisoning earlier this month, according to TMZ.  359
