For the third time in three weeks, a major commercial airline flight was diverted mid-air because of a damaged window.The latest incident happened Sunday, when a JetBlue flight from San Juan, Puerto Rico, to Tampa, Florida, was diverted because of damage to the plane's windscreen.The airline said the flight was diverted to Fort Lauderdale out of "an abundance of caution following a report of damage to one of the outer layers of the cockpit windscreen." The plane landed safely and the passengers were accommodated on another aircraft, JetBlue said.That incident comes about three weeks after a?fatal Southwest Airlines flight in which a jet engine failed midair and debris knocked out a cabin window.Jennifer Riordan, a philanthropist and Wells Fargo executive in New Mexico, was partially sucked out of the plane as other passengers struggled to pull her back into her seat. She was later pronounced dead from blunt impact trauma, a spokesman for the Philadelphia Department of Public Health said.Investigators with the National Transportation Safety Board who looked into the failure said that one of the engine's 24 fan blades was missing.Despite the mid-air engine failure, Captain Tammie Jo Shults was able to safely pilot the flight in an emergency landing in Philadelphia.On May 2, a Southwest Airlines flight from Chicago to Newark, New Jersey, made an unplanned landing after a window cracked.Flight 957 landed safely at Cleveland Hopkins International Airport, FAA spokeswoman Elizabeth Cory said. Southwest said the diversion was in response to a reported crack in the outer pane of a multi-pane window.Passenger Linda Holley texted her son Ryan with a picture of a damaged window."Window on plane cracked during flight. Landing in Cleveland to be safe. Everything ok but scary," one of her texts read.In another, she said: "Yes Southwest. Just heard loud noise. Very large crack with piece of window missing at bottom. (AP) -- The U.S. Navy is investigating a video in which military work dogs attacked a "Colin Kaepernick stand-in" during a demonstration at a Navy SEAL Museum fundraiser.The event happened last year at the museum in Fort Pierce, Florida, but the video resurfaced over the weekend.On Sunday, the U.S. Navy said in a statement that it appears no active duty personnel were involved.The videos show dogs attack a man in heavy padding wearing a Kaepernick jersey. FLORIDA — Police say former Buffalo Bills and Miami Dolphins player Richie Incognito was involved in a disturbance at a Lifetime Gym in Boca Raton, Florida on Wednesday morning.Police said Incognito was involuntarily committed under Florida's Baker Act. The act allows officials to apprehend and order a mental evaluation of a person.Incognito reportedly threw a dumbbell at a man at the gym and that's why police were called, according to TMZ.com.Incognito played for the Buffalo last season. In the buildup to the draft, he took a restructured contract with the Bills, publicly fired his agent on Twitter after that deal was signed, and retired shortly after that. The Bills officially added him to the reserve/retired list on April 12.Once Organized Team Activities started around the league, Incognito -- according to ESPN -- wanted to make a return to the NFL, despite retiring from the league and team just one month prior. The Bills granted his reported wish, releasing him off the reserve/retired list on Monday.The move officially made Incognito an unrestricted free agent, able to be signed by any other team. For 20 minutes, the Southwest Airlines jet was a normal flight from New York to Dallas with 149 people aboard.The plane was flying at 32,500 feet Tuesday morning as passengers settled in for the three-hour flight.Suddenly, the alarms blared in the cockpit as what sounded like explosions boomed from the left side of the plane. Oxygen masks swiftly dangled from the ceiling.What followed was a terrifying sequence of events that ended with one woman dead, seven people injured and an emergency landing at Philadelphia International Airport.'Everybody was going crazy'Southwest Airlines Flight 1380 took off from LaGuardia at 10:43 a.m., and landed in Philadelphia about 11:20 a.m., federal officials said.The Boeing 737 was headed to Dallas with 144 passengers and five crew members . For about 20 minutes, everything seemed calm. Then what sounded like an explosion suddenly jolted the plane, passenger Marty Martinez said."I heard a loud boom and about five seconds later, all the oxygen masks deployed," he said. "I immediately knew something was wrong. It just didn't register what could have been."Something in the engine broke apart midair and burst through the window, passengers said. The shattered window partially sucked a woman out of the plane as passengers struggled to pull her back in."Everybody was going crazy, and yelling and screaming," Martinez said. "As the plane is going down, I am literally purchasing internet just so I can get some kind of communication to the outside world."Objects flying outAs the plane quickly descended, passengers close to the woman scrambled to hold her tight. Others stuffed clothes and jackets into the gaping hole on the window, said Martinez, who was sitting two rows away from the woman. Those items got sucked out of the plane, too, he said."We could feel the air from the outside coming in, and then we had smoke kind of coming in the window," Martinez said.In the chaos, it was hard to hear anyone. Flight tracking website FlightRadar24 estimated the plane descended from 31,684 feet to 10,000 feet in a little over five minutes."It was very loud, so announcements from the pilot or any other crew would not have been heard," passenger Amy Serafini said.Passenger Matt Tranchin watched the commotion as people tried to help."Everyone kind of descended on where this hole was," he said. He thought about his family, and whether he'd see them again."That I'll never live to see my son born. That I'll never be able to say goodbye to my wife, say goodbye to my parents. But I am. I feel really very fortunate for that," he told CNN affiliate WPVI.Nurse performs CPRAfter trying to pull the woman back for several minutes, a man in a cowboy hat and a second man finally got her back in her seat, Serafini said.A nurse aboard the flight volunteered to perform CPR."I went back and we started CPR on the lady, which we continued for about 20 minutes. We were still doing CPR when the plane landed," said nurse Peggy Williams. "We made every effort that we could possibly make to save this woman's life."Martinez said it was a rough landing, and he wasn't sure if the plane was going to crash. The jet could have been landing on a freeway or a skyscraper for all he knew, he said."I didn't know if we were going to be running into a building. I didn't know what state the plane or even the pilot was in, if we were in condition to land," he said. "Finally when we ... came to a halt, of course, the entire crowd was (in) tears and people crying and we were just thankful to be alive."Air traffic callBefore the plane landed, the pilot asked the air traffic controller to send medics to meet it."Injured passengers OK, and is your airplane physically on fire?" the air traffic controller asked."No, it's not on fire, but part of it is missing. They said there's a hole and that someone went out," the pilot responded."Um, I'm sorry, you said there was a hole and somebody went out? Southwest 1380, it doesn't matter we will work it out there," the air traffic controller said.The air traffic controller asked other planes to prepare for the airport to be shut down.The investigationThe National Transportation Safety Board said a preliminary look at the engine shows one of its 24 fan blades was missing.A first look showed evidence of metal fatigue where the blade attached to a hub, according to Chairman Robert Sumwalt of NTSB.The crew reported damage to one of the aircraft's engines as well as the fuselage and a window, the Federal Aviation Administration said.Southwest CEO Gary Kelly said the plane was inspected Sunday, but he had no details on what parts were examined. "I'm not aware of any issues with the airplane or any issues with the engine involved," he said."This is a sad day and our hearts go out to the family and the loved ones of the deceased customer," he said. "We will do all that we can to support them during this very difficult time."The woman killed was identified as Jennifer Riordan, 43, according to CNN affiliate KOAT. It said she worked for Wells Fargo in Albuquerque, New Mexico. Southwest said it's the first in-flight death in company history.Flight recorders found The flight data recorder and the cockpit voice recorder have been sent to Washington, Sumwalt said. For many restaurants, like Sam's No. 3 in downtown Denver, the experience is part of what they serve.“We were built to serve people inside,” said Sam Armatas, owner of the restaurant. But with ever-changing COVID-19-related dining restrictions and winter looming, delivery is becoming a more enticing option for customers. And for Sam’s No. 3, delivery apps make that easy.“We’re able to continue to serve our product, try and stay relevant as far as people eating our food,” Armatas said. The diner has three locations. At two of them. 90% of orders are now made through delivery apps. This can be convenient for customers, but costly for some of the restaurants. Exposure to consumers has it's price.“There are negatives. I mean they take a commission but those commissions are now capped,” Armatas said. “You're pretty much at the mercy right now of the delivery services hoping to get your food out hot, tasty and attractive still.”He chooses to stick with the apps to get his food out there to people, while for other restaurants, the cons of delivery apps outweigh the pros.“At the moment, we will not use any third-party services at all for delivery,” said Giles Flanagin, Co-founder of Blue Pan Pizza.Blue Pan relies on their team of 17 part-time in-house delivery drivers, instead.“In-house delivery can work cost-wise, if the restaurateur is willing to put in the time and the effort to build that specific revenue stream,” he said. “If I use Doordash, Grubhub, or Postmates and I pay a 25% commission, not only am I losing all of my profit, but I’m in the red.”Flanagin said Blue Pan has been using their own delivery since they opened in 2016. They tried a delivery app to serve areas farther away, but too many bad experiences led them to cancel.“When a customer gets a pizza from a third-party delivery and it’s a poorly delivered experience, they don't look at Grubhub or those businesses. They call us and they're upset,” he said. For him, the reputation of his business and their food is important.“I think the best way I can summarize making a decision to use a third-party delivery service is buyer beware. This is our experience and I’m not saying it's everyone's experience,” Flanagin said.It’s a balancing act for these apps like Uber Eats and Grubhub. They have a business to run, but they also have to consider the restaurant and the driver.“Restaurants are just trying to find any possible ways to break even or minimize their costs,” said Alexandre Padilla, an economist and professor at the Metropolitan State University of Denver. “It’s a very complicated issue where the apps are providing a service where they are trying to attract drivers to meet the increase in demand due to the pandemic.”As potential customers opted to stay home in March when lockdowns began, the demand for drivers went up.Gig economy workers like Julian Rai almost completely switched from rideshare apps to delivery apps backs in March.“Remember that we are basically waiters on wheels, we’re servers on wheels,” he said. “If it weren't for tips, we’re making less than minimum wage just from the delivery fee. Like a waiter, it’s very similar to what a server would make before tips. So at the end of the day, well over two thirds to three fifths of my income comes from tips.”Rai explained they may spend 20 to 40 minutes on one single order so, reasonably, they ask for some compensation for that.It’s a tough balancing act between restaurant, app, and driver.“I don’t know that that balance has been struck yet,” Rai said.For now, delivery is a means to an end for these restaurants that thrive on providing quality food and a great dine-in customer experience.“Our business model isn't built to survive this way,” Armatas said. “We’re just trying to stay relevant, trying to survive. If we can get through winter great. That’s the hope, the dream, is that by March we’re still here.” 3901
