Law enforcement officials from New York to Florida are examining clues to find the person or persons who sent 10 packages containing what appear to be explosive materials to prominent Democrats, critics of President Donald Trump, and to CNN's New York offices as authorities classify the incidents as domestic terrorism.Three new packages were detected Thursday -- two intended for former Vice President Joe Biden and one for actor-director Robert DeNiro -- each bearing the same markings as the seven packages detected earlier this week. The apparent bombs, while rudimentary in design, according to law enforcement officials, targeted two former US presidents, a former vice president and potential presidential contender, and numerous outspoken critics of the Trump administration.Investigators are reviewing security footage from the CNN building and examining the packages themselves for potential clues, according to current and former law enforcement officials."Everything is a clue," a law enforcement official said.As authorities examine clues, they are also finding more questions to answer.One key question authorities are puzzling over is why none of the apparent bombs detonated, a second law enforcement source said, raising questions about the skill and motive of the bombmaker.This person said either the bombmaker was good enough to ensure none would go off and never intended them to explode or it was poorly constructed. Authorities consider the bombs to be potentially destructive because of the presence of what is believed to be pyrotechnic powder. Though it appears they were handled through the postal system -- and thus jostled and moved -- without triggering any explosion.William Sweeney, FBI special agent in charge of the New York field office, said Thursday afternoon that the powder discovered with the package sent to CNN did not pose a biological threat but another official said it was undergoing testing.The FBI's counterterrorism team is leading the investigation and has classified it as a domestic terror matter, the law enforcement official said. That classification is based on the absence of other information and could change.The Secret Service, US Postal Inspector Service, and ATF are also working on the investigation with various state and federal offices. (KGTV) -- The Citizen Public Safety Oversight Task Force of La Mesa met Wednesday virtually, following two high-profile incidents involving the city's police department.The recently formed group is tasked with looking at whether La Mesa needs a police oversight committee and then decide what it will look like.The group's chair, Jamal McRae, realizes they are a temporary task force, but hopes it rolls over to become the permanent commission."We're still trying to educate ourselves as to what our committee will do, as far as, will be able to have subpoena power? Do we want subpoena power? We're still in the research phase." McRae said. "We're trying to determine whether we want to put this on a ballot to have individuals on the city vote on it, or if we wish the city council will just have the final say.McRae points out the idea for the task force came before the controversial incidents involving La Mesa police officers, and before the national calls for police reform following the death of George Floyd."We're a group of individual citizens that got together and wanted to make a change in the community after the Helix High School incident," McRae said. "So we have been running through some ideas for a few years, just among community members."McRae says the group understands the importance of their work and the urgency to complete their tasks. That's why they have scheduled three meetings next month. Mike Lee, R-Utah, appeared at Judge Amy Coney Barrett's confirmation hearing in-person on Monday and delivered his opening remarks without a mask.On Oct. 2, Lee announced that he had tested positive for COVID-19. He was one of about a dozen high-ranking government officials — including President Donald Trump — who attended Barrett's Sept. 26 nomination ceremony at the White House and later contracted the virus.Lee said he had received a letter from Congress's attending physician, Dr. Brian Monahan, clearing him to return to his in-person duties — though the letter did not note that Lee had tested negative for the virus."Based upon current CDC guidelines, you have met criteria to end COVID-19 isolation for those with mild to moderate disease," Monahan's letter read. "Specifically, it has been greater than 10 days since symptom onset, you have had no fever in absence of fever reducing medication for at least 24 hours, and your other symptoms have improved. The CDC does not recommend repeat SARS-CoV-2 PCR testing if these criteria are met.”It's unclear if Lee was tested prior to appearing in the Capitol Building on Monday. Graham's refusal to take a test forced a change in format for the weekend debate.During his opening comments, Leahy claimed that Republican senator's aversion to COVID-19 tests was a political tactic to remove any risk that would delay Barrett's nomination until after election day.In his opening remarks, Graham said that his committee had consulted with Monahan and the Capitol architect officials to set up the hearing room "in a fashion we can safely do our business."Following senators' opening statements, Graham explained that the Capitol architect had deemed the hearing room to be in compliance with CDC social distancing guidelines. He said that he wanted to make sure the room was safe but added that like the rest of America, the Senate needed to get "back to work."He also added that he had tested negative "last Friday" following brief contact with Lee, and said that he would not be tested for the virus at the request of "political opponents."Earlier this year, Monahan asked senators to wear masks while on the Senate floor. LA MESA, Calif (KGTV) - According to the California Department of Public Health, seven deficiencies were issued after an investigation of Sharp Grossmont Hospital. In April state investigators conducted a survey consisting of 14 closed medical records. According to the investigator's findings documented in a statement of deficiencies and plan of correction report, "Hospital leadership did not provide notification of a potential investigation and the use of video recording during medical treatment without patient's consent." Investigators also noted deficiencies with regulations related to medical staff accountability, patient's rights, and use of restraint or seclusion. According to the state document, "The patient has the right to personal privacy. This Statute is not met as evidenced by: Based on interview and record review, the hospital failed to ensure the privacy of all patients undergoing medical treatment in the WHC (women's health center) ORs (operating rooms) when the hospital's Admission Agreement for Inpatient and Outpatient Services was utilized as consent to allow video recording during medical treatment. Also, the Admission Agreement did not allow the patients' the option to acknowledge and/or decline video recording. As a result, the hospital leadership allowed video recording of patients undergoing medical treatment without knowledge or consent." The deficiencies listed fall under violations to conditions of participation for a hospital as listed in title 42 of the federal code of regulations.   A deficiency is a violation of a federal or state law or regulation that the California Department of Public Health cites during a survey or investigation.Officials with the California Department of Public Health tell 10News issuing deficiencies is a form of enforcement action. It requires the facility to submit a Plans of Correction to fix any violations. According to Sharp officials, in 2012 they were alerted that beginning in 2011, potentially powerful drugs and equipment were missing from the anesthesia cart in each of Sharp Grossmont Hospital's three Women's Center operating rooms. The hospital installed a computer monitor with a motion-activated camera in each of the three Women's Center operating rooms. Between July 2012 and June 2013, the videos were recorded. The hospital says the cameras were intended to record only individuals in front of the anesthesia carts. In a statement to 10News, a spokesperson for Sharp wrote:"In their survey, the California Department of Public Health (CDPH) identified areas where opportunities for improvement existed. The hospital addressed each area of concern and created a plan of correction for those findings. Both CDPH and the Centers for Medicare & Medicaid Services (CMS) accepted Sharp Grossmont’s plan of correction, as confirmed in a follow-up survey. The hospital’s plan of correction provides that:The specific techniques used in the 2012-13 investigation have not been used since and will not be used for any future investigations.Any potential future investigations are required to strictly adhere to Sharp’s policies and procedures;Medical staff will be kept informed of any similar future investigations that may occur;Patients will receive full disclosure of any future investigations that may affect their privacy;The patient grievance practices have been strengthened to ensure patient complaints and grievances are appropriately and efficiently managed; andAdditional measures are in place to safeguard against the inadvertent release of patient information to third parties. Staff and clinicians have been educated on the hospital’s plan of correction and policies. While we are pleased that CDPH and CMS accepted our plan of correction, we regret that the investigation inadvertently recorded patients and may have caused distress to those we serve."More than 400 women who claim they were secretly recorded are suing the hospital.Court records show a hearing is scheduled in September. 4026

LAKESIDE, Calif. (KGTV) -- Nearly 200 people turned out for a vigil in Lakeside honoring two teenagers killed in a car accident last weekend. Isaac Culkin and Justin Kyte were killed on Saturday, Jan. 11 when the car they were riding in crashed into a tree. Two others in the car suffered moderate injuries, but survived. Those closest to the boys gathered at the scene of the crash for a candlelight vigil, a moment of silence and prayer. The teens' closest friends remembered them as hardworking, funny boys who loved to have fun offroading the desert. CHP says Kyte was behind the wheel when he lost control of the car and hit a tree on Willow Road. Culkin was in the passenger seat. CHP says speed was a factor, but it is unclear if drugs or alcohol were involved. Services for Culkin are scheduled Thursday, January 23rd. Information on Kyte's services have not been made public.
