(KGTV) -- Police are investigating after a man was shot and killed before crashing into the side of a building in Chula Vista Wednesday afternoon. According to police, the initial call came in around noon as a drunk driver. Witnesses reported seeing a man in a tan SUV slumped over and followed him.Police say the SUV then slammed into a gate and crashed into the side of a self-storage building traveling roughly 10 to 15 miles per hour on Industrial Boulevard near L Street.When police arrived, they discovered that the man was dead inside the vehicle. Police confirmed later in the day Wednesday that the man was shot. Shortly before the crash, witnesses in the area reported hearing gunshots. Investigators are currently working to get surveillance video from the self-storage facility. Police are also asking that anyone with information on a possible suspect contact them.  932

(KGTV) -- A driver was arrested on suspicion of DUI after two cars collided on Interstate 805 in the South Bay, sending one car sliding down the freeway several hundred feet.According to Chula Vista police, the crash happened around 2:30 a.m. Tuesday on northbound I-805 near H Street in Chula Vista.A tow truck driver stopped to help and said the four people inside the overturned car were able to crawl out the wreckage and broken glass.The second car was able to pull over down the freeway, with minor damage.One of the flipped cars' passengers was transported to the hospital. The other three, as well as the other driver, was treated on scene for minor injuries.California Highway Patrol is investigating and believes alcohol may have been a factor. 782

CINCINNATI -- A preliminary investigation reports both technical problems and human error may have played roles in first responders' failure to locate a teen who was found dead of asphyxiation in a van parked in a high school parking lot Tuesday.According to documents from an internal review obtained by Scripps station WCPO in Cincinnati, the 911 operator who answered Kyle Plush's second emergency call said she couldn't hear him. Plush, 16, was found dead in a minivan near Seven Hills School hours later Tuesday night. In one 911 call, he said he was stuck in a van outside the school. In the second call, he described the make, model and color of the minivan where he was trapped and dying.Investigators don't believe there was a failure in the phone system at that time, so it's not clear why the second operator couldn't hear Plush. But the 911 operators' computers experienced trouble "around that same time frame," one of the internal documents states. The operator said her screen froze, preventing her from properly documenting the call. TIMELINE: What happened the day Kyle Plush died?Still, the operator tried sending a text message to Plush, asking him for the address of the emergency. She tried calling him twice, records show. He never responded.That operator's supervisors found her work in that incident was "unacceptable," according to one of the internal documents. Officials haven't said exactly where the officers searched. They tried calling Plush's phone, but he didn't answer. It was then that Plush called 911 the second time. "This is not a joke," he said. "I am trapped inside a gold Honda Odyssey van in the parking lot of Seven Hills. ... Send officers immediately. CHULA VISTA, Calif. (KGTV) -- Police are searching for a suspect after two women discovered the body of a homeless man in Chula Vista Tuesday morning. According to police, two sisters were walking on the 1400 block of Broadway around 1:20 a.m. when they discovered a man who wasn’t breathing.Officers tried to save the man, later identified as 56-year-old Ivan Velez, but he was pronounced dead at the scene. Investigators later found signs of trauma and are currently searching for a suspect. No description of the suspect was given. CHULA VISTA, Calif. (KGTV) - A pedestrian died and others, including children, were injured Monday in a crash involving a construction truck in Chula Vista. The truck driver lost control about 1:15 p.m. in the 500 block of 4th Ave near Shasta Street, police said. The location is a residential street near Scripps Mercy Hospital Chula Vista. The truck hit parked cars including a black pickup, which then struck a pedestrian before flipping over, said officers.Paramedics took the pedestrian to the hospital with critical injuries. The victim, an 81-year-old, later died, police confirmed. A mother and her two children, a baby and a girl, were in a parked car and suffered minor injuries. Chula Vista police are investigating the cause of the crash. At this time, police say it's unclear whether or not drugs or alcohol were involved. 844
