A wallet-sized card with a picture of a young girl in West Michigan on the front may not mean much, but once flipped over the card’s intent is revealed.It asks people to pay it forward.“When we see the cards, we’re sad, but we’re happy because people look at the card and see our daughter,” said Cindi Rasmussen.Cindi and her husband, Mark, are the parents of the girl on the card.Her name is Molly.The parents describe the 16-year-old Forest Hills East student as fun-loving, social, and always on the go.“My favorite line of mine was, ‘When you finish what you’re doing, can you take me to Starbucks? Or can you take me to the gas station to get candy?’” Mark remembered.But this past June, everything stopped for the family.Molly died while swimming with a friend at Lake Bella Vista in Cannon Township. According to her parents, Molly was an epileptic and had a seizure in the water.“There’s not a day or a second that I don’t think about her,” said Mark.“That whole night was awful,” said Cindi.However, in the midst of their grief came Molly’s Cards, which encourage people to perform a small task, like opening a door or paying for coffee, in honor of the Grand Rapids native.Cindi says one of her former co-workers created Molly's Cards after Cindi asked people to pay it forward in lieu of sending more flowers to their family.The family estimates more than 1,000 Molly’s Cards have been distributed already. A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. A sergeant involved in the search for a missing hiker in Zion National Park says there are several discrepancies he sees in the case of her 12-day survival in the park.Holly Courtier went missing while hiking in the park on October 6. A rescue effort involving the national park’s team and others, including K-9 teams and trained investigators, found Courtier on October 18 after a credible tip of a possible sighting.Her family said Courtier hit her head, became disoriented, and was able to stay near a “water source” in the park.In an interview with ABC4 in Salt Lake City, Sgt. Darrell Cashin says he finds some “discrepancies and questions that do not add up.”Cashin is a liaison with Washington County Sheriff’s Search and Rescue, a job he’s had for about nine years, in addition he says he has 25 years experience on search and rescue teams.One thing he says didn’t add up was the water source. Park officials say Courtier was found along the Virgin River, but Cashin says that river was likely not drinkable or at least would have caused her to get sick.In August, Zion National Park officials sent out a warning to all visitors to avoid contact with the Virgin River “until further notice” because of levels of cyanobacteria from a toxic algal bloom.“If she had been drinking that water, unless she had some really high immune system, she would’ve been very, very ill and probably unable to come out on her own,” Cashin told ABC4. “She either took a lot of water with her or had another clean water source that was near here, but the Virgin River is not that source.”CNN reports the family told them Courtier was only able to take a few steps before collapsing. However, Zion National Park officials report she was able to walk out of the park on her own with minimal assistance. She was taken to a nearby hospital to be checked out and stayed the night.In the interview with CNN, Courtier’s family does not clarify if the water source was potable, but they did say Courtier was without food while she was lost in the park.Cashin alleges there are also questions about her decision to go on the hike. A man who tried to enter a teenage girl's bathroom stall was confronted and killed by the girl's father, officials say. According to Phoenix police, around 11:30 p.m. on August 2, 40-year-old Melvin Harris went to the QuikTrip near Dunlap and 19th avenues to pick up his teenage daughter and her friends. A man, who would later become the victim, approached Harris' car in the parking lot and asked for money. Harris gave him some money, and he went into the QT. Harris was later informed that a man tried to enter the bathroom stall his daughter was using. The teen told an employee, who in turn alerted an on-duty security guard. The man exited the store and was pointed out to Harris, who realized it was the same man he had given money to earlier. Harris went into the store and told the security guard that, "he needed to take care of the situation, or [Harris] would do it himself." The guard reportedly told Harris he would handle it. According to witnesses, Harris later approached the man and punched him in the face. Witnesses also reportedly told police that Harris stood over him, hitting him in the face several more times. He also allegedly kicked and stomped on him. Harris then left the area. Police identified and located Harris at his home nearby. He reportedly told police that the man punched him first, hitting him in the neck. He denied striking the man when he was down. The victim was taken to the hospital with a broken nose and brain injuries. He later died.Harris has been charged with second-degree murder. 1612

A Maryland police officer has been arrested and charged with rape after allegedly assaulting a woman during a traffic stop.Ryan Macklin, a six-year veteran of the Prince George's County Police Department, was taken into custody Monday evening, according to a news release from the department.He has been suspended without pay, the department said, and has been charged with first-degree rape, second-degree rape, perverted practice, second-degree assault and fourth-degree sex offense.Police said Macklin pulled the woman over at about 1 a.m. on Thursday. During the stop, police said, Macklin forced the woman "to perform a sexual act" while they were both in the woman's car in a parking lot. Police said Macklin was on duty, in uniform and driving a marked police cruiser at the time.Prince George's County Police Chief Hank Stawinski said at a news conference that the woman came forward "several hours" after the incident with the encouragement of friends. Police said they do not know why Macklin allegedly targeted the driver."The charges against this officer are highly troubling," Stawinski said. "Officers take an oath to protect others, not to abuse their authority in order to victimize someone. Those who live and work in Prince George's County deserve the very best from the men and women of this department."Stawinski said police have information that suggests "there may be additional people impacted by this officer's conduct."
