As hospitals are becoming overwhelmed with COVID-19 patients, North Dakota is permitting coronavirus-positive health care workers to continue assisting patients.The guidance stipulates that the health care workers must remain asymptomatic and take enhanced precautions in order to stay on the job. This comes as travel nurses are stretched thin as dozens of states are reporting record hospitalizations.Given the situation in North Dakota and elsewhere, it is possible more states will have to follow suit and continue using infected staff members in order to provide care. The CDC has spelled out guidance in these situations.The CDC says hospitals must exhaust a number of other guidelines, including adding travel nurses, postponing elective medical procedures, and postpone elective time off, before going into a crisis staffing situation.“If shortages continue despite other mitigation strategies, consider implementing criteria to allow HCP (health care personnel) with suspected or confirmed COVID-19 who are well enough and willing to work but have not met all Return to Work Criteria to work,” the CDC’s guidance states. “If HCP are allowed to work before meeting all criteria, they should be restricted from contact with severely immunocompromised patients (e.g., transplant, hematology-oncology) and facilities should consider prioritizing their duties in the following order:1. If not already done, allow HCP with suspected or confirmed COVID-19 to perform job duties where they do not interact with others (e.g., patients or other HCP), such as in telemedicine services.2. Allow HCP with confirmed COVID-19 to provide direct care only for patients with confirmed COVID-19, preferably in a cohort setting.3. Allow HCP with confirmed COVID-19 to provide direct care for patients with suspected COVID-19.4. As a last resort, allow HCP with confirmed COVID-19 to provide direct care for patients without suspected or confirmed COVID-19.”North Dakota Gov. Doug Burgum spelled out the challenges facing his state.“Our hospitals are under enormous pressure now,” Burgum said. “We can see the future two, three weeks out, and we know that we have severe constraints.”Despite Burgum’s warning, his state is among the ones not to issue an order requiring masks in public places.In update CDC guidance, the agency says that masks can reduce the viral load for the wearer, in addition to prevent the spread of the virus.Steven Weiser, MD, president of Altru Health System in Grand Forks, North Dakota,, wrote in an op-ed about the toll the virus is having on workers. Amid the pandemic, food banks are on average serving 60% more families than a year ago, according to a recent analysis by Feeding America. The analysis found that 80% of food banks in the Feeding America network have seen an increase in demand amid the pandemic.Despite this jump in demand, the organization says that food banks have “become accustomed” to increased demand for services. Feeding America’s analysis says that 40% of those going to food banks this year are doing so for the first time.Compounding issues for many families, food prices have jumped in the last year, according to USDA analysis. According to the USDA, the average cost of food prepared at home has increased 4% from October 2019 through October 2020. In the last 20 years, the average increase of food prices per year is 2%.The increase in prices has been most pronounced in meat, poultry and dairy. From October 2019 through October 2020, meat prices jumped 6.6%. Previously, meat prices generally increased 3% a year.The issue of increased prices was something that Feeding America leaders addressed last month as lawmakers have failed to come to an agreement on a new stimulus plan.“With food prices increasing at the fastest rate in 50 years and predictions that 1 in 4 children could face hunger this year, a 15 percent increase to the maximum SNAP benefit is the most effective way to meet the moment,” Kate Leone, Chief government relations officer at Feeding America, wrote. “Boosting SNAP benefits will provide families more resources to purchase the food they need through purchases at local grocers and businesses, which will stimulate economies across the country.“With the nation’s public health and economic crises continuing without an end in sight, our economy and families are being pushed to the very brink. We need our government to invest in the hunger-relief measures today because families struggling with hunger need it now.”Feeding America said that Americans can help by volunteering at their local food bank or donating to their local food bank or Feeding America’s COVID-19 Response Fund. An Arizona man was awarded a 5,000 verdict after a jury decided a dentist at a popular dental chain wrongly pulled all his teeth without consent.DeWayne Smith won his case against Western Dental Services and Dr. Steven Nguyen in January, ending a three-year legal battle.“They treated him like garbage,” said Craigg Voigtmann, one of Smith’s attorneys. “They took out all of his teeth and then abandoned him.”Western Dental disputes the allegations. In a statement, the California-based chain called the jury’s decision surprising and disappointing.On a Saturday in January 2015, Smith scheduled an emergency visit at the Western Dental. He was having severe tooth pain in his right upper wisdom tooth. It was his first visit ever at the dental office.“I really don’t like going to dentists but I was in pain,” Smith said.At the appointment, Western employees and Nguyen told him Smith had serious dental issues and that he would eventually need to replace all his teeth with implants or dentures.Smith understood, and under pressure from Western Dental sales staff agreed to a long-term treatment plan. However, Smith said he “made it very clear” that he only wanted to have one tooth pulled that day to relieve the pain.During the procedure, Western Dental staff numbed the area, sedated Smith with nitrous oxide, and then packed his mouth full of gauze, according to court testimony.Smith left not knowing they had extracted all his teeth.“I go to pulling this gauze out and then I realize there’s nothing there on the bottom, and there’s nothing there on the top,” he said. “I just fell to the floor. I didn’t believe what I was seeing.”Employees testified that Nguyen left immediately after the procedure and that no one appeared to tell Smith what they did.After the procedure, Western Dental also sent Smith a ,000 bill, he said.“(It’s) like running over a dog in the middle of the road and driving off,” said Smith’s fiancé, Sharon.Nguyen testified that he had a direct conversation with Smith, who told him he wanted to have all of his teeth extracted that day. Western Dental also produced consent forms with Smith’s signature.But Smith’s attorneys claimed those forms were manufactured after the procedure. And one Western employee testified against the company in hours of damning testimony.Bianca Esquer was a patient care coordinator at Western Dental from 2013 to 2015. She worked directly with Smith during his appointment.In a deposition and at trial, Esquer said that Smith made it clear he did not want a full extraction that day. She also discussed how employees and dentists are paid bonuses and commissions based on how much treatment patients get.Esquer also testified it’s hard to believe anyone would agree to have all their teeth pulled on their first visit and without detailed planning.“You don’t send someone home with a full extraction without teeth,” she said.Smith was not provided dentures from Western Dental until months after his procedure, he said.Scripps station KNXV in Phoenix reached out to Nguyen for comment through his attorney, Jeff Tonner. As governors around the country consider new or stricter restrictions to control the latest surge in coronavirus cases, a recently published study identifies restaurants, cafes and gyms as some of the places having the highest risk of coronavirus spread outside the home.The study, published this week in the journal Nature, looked at data from millions of Americans, tracked by their phones as they went about daily life during the first wave of the pandemic this spring.They used the data and an epidemiological model to run simulations on viral spread at points of interest outside the home. Their findings in the simulations closely matched actual coronavirus caseloads, according to the Washington Post.“We found large variation in predicted reopening risks: on average across metro areas, full-service restaurants, gyms, hotels, cafes, religious organizations, and limited-service restaurants produced the largest predicted increases in infections when reopened,” the study states.Researchers say these locations pose more risk because the mobility data, data showing how mobile people are at these places, shows Americans tend to spend longer amounts of time and at a higher density of people.Their models add support to pandemic measures around the country that limit capacity at some of these points of interest, including capping indoor gatherings to a certain percentage or number of people.“Reducing maximum occupancy substantially reduced risk without sharply reducing overall mobility: capping at 20% maximum occupancy in the Chicago metro area cut down predicted new infections by more than 80% but only lost 42% of overall visits, and we observed similar trends across other metro areas,” researchers stated.The study also looked at disparities in lower income neighborhoods, where more of the population has to leave their home for essential jobs, grocery delivery may not be available or is financially not possible, and businesses tend to be smaller and potentially more crowded.“Because disadvantaged groups suffer a larger burden of infection, it is critical to not just consider the overall impact of reopening plans but also their disparate impact on disadvantaged groups specifically,” the study states.The researchers hope the information is helpful to policymakers and city leaders as they consider reopening restrictions. 2356
