(CNN) -- Uber released its highly-anticipated safety report on Thursday revealing, among other details, that it received 5,981 reports of sexual assault in 2017 and 2018.Among those, there were 464 reports of rape.Uber first pledged to release the report nearly a year ago in response to a CNN investigation that found at least 103 Uber drivers in the United States had been accused of sexually assaulting or abusing their passengers in the previous four years. The drivers were arrested, wanted by police, or had been named in civil suits related to the incidents. Uber ultimately announced it would do away with a policy that previously forced individuals with sexual-assault complaints into arbitration and made them sign non-disclosure agreements.Uber went public in May but warned investors in its IPO paperwork about the report, which it said could negatively impact its brand."The public responses to this transparency report or similar public reporting of safety incidents claimed to have occurred on our platform ... may result in negative media coverage and increased regulatory scrutiny and could adversely affect our reputation with platform users," the company says in the filing.Uber and competitor Lyft have faced legal action related to safety issues around the country. A lack of transparency about the number of incidents involving drivers has been a sticking point in lawsuits against the company. John McCain during a private morning memorial service in Phoenix, Ariz.A black hearse pulled up to Arizona's Capitol Wednesday morning, where uniformed service members flanked the route McCain's casket traveled into the Arizona State Capitol Museum rotunda. There, McCain will lie in state after the private service.Inside the museum, McCain's family mourned the loss of the Arizona senator. According to Circulate San Diego, 1,992 people were injured, and 101 people were killed in 2018 while walking and biking on roads throughout San Diego County. (CNN) - The maker of Taytulla birth-control treatment issued a nationwide recall in the United States on Tuesday over concerns that misplaced capsules in pill packs could cause unintended pregnancies.A physician reported that four placebo capsules were packaged in the wrong order where active capsules should have been, drugmaker Allergan said in a statement.The physician sample pack should have 24 pink capsules with hormones followed by four maroon capsules without hormones, Allergan said. Instead, the faulty pack had four maroon placebos at the start of the treatment."As a result of this packaging error, oral contraceptive capsules that are taken out of sequence may place the user at risk for contraceptive failure and unintended pregnancy," the statement said."The reversing of the order may not be apparent to either new users or previous users of the product, increasing the likelihood of taking the capsules out of order."Allergan said it is arranging for return of all sample pack products with the lot #5620706 Exp. (KGTV) — An inmate who escaped from San Quentin State Prison in Northern California the day after Christmas was apprehended Saturday about 200 miles away.Shalom Mendoza, 21, was reported missing just after 9:30 p.m. on Dec. 26 after he had been working outside of the prison perimeter with other inmates, according to the state Corrections and Rehabilitation Department (CDCR).Investigators began searching the surrounding area and say he appeared to have carjacked a Toyota Rav4 from a Home Depot parking lot in the San Rafael area.RELATED: San Quentin State Prison inmate escapes, suspected in carjackingThe Toyota was later found abandoned off the 101 Freeway north of Paso Robles on Friday. That led CDCR officials closer to Mendoza.A civilian saw Mendoza at a Taco Bell location in Paso Robles and called authorities, who placed him in custody without incident at about 12:30 p.m. Saturday.Mendoza now faces escape and carjacking charges. He was serving a five-year sentence for use of a deadly weapon during a carjacking/attempted carjacking and evading or attempting to evade a peace officer while driving recklessly. 1132
