A patient in Minnesota who had COVID-19, and attended the Sturgis Motorcycle Rally in South Dakota, has died, the Minnesota Health Department confirmed to E.W. Scripps.The Washington Post reported that this is the first death from the virus that was linked back to the rally.In an email to E.W. Scripps, a spokesman for the Minnesota Department of Health said the patient was in their 60s and had underlying health conditions "as do many people in that age group."The spokesman said the patient had been hospitalized in the intensive care unit.The 10-day event ended on Aug. 16.According to the Associated Press, health departments in at least eight states are also trying to track virus outbreaks.At least 260 cases in 11 states can be tied directly to the event, the Washington Post reported. 802

A New Jersey man who won a 8 million Powerball jackpot in 2013 has been charged with sexually assaulting a child.According to NJ.com, Pedro Quezada of Wayne, was charged with two counts of first-degree sexual assault and one count of second-degree endangering the welfare of a child.Prosecutors say Quezada assaulted the victim for three years when the child was between the ages of 11 and 14 when he lived in nearby Passaic.Quezada won a 8 million jackpot in March 2013 when he purchased the winning ticket at a Passaic liquor store. He took his winnings in a lump sum, meaning he took home 1 million after taxes.At the time of his win, Quezada told CNN the money would not “change his heart.”At the time of his Powerball victory, CNN reported that Quezada was married with five children. However, it was later revealed that Quezada was not actually married — though the woman who he claimed was his wife sued him, saying the lottery ticket was purchased with the couple’s shared assets. A train carrying dozens of Republican members of Congress, including House Speaker Paul Ryan, to their legislative retreat in West Virginia hit a truck on January 31, 2017 leaving one person dead.Wednesday the National Transportation Safety Board released a preliminary investigation into the crash.They said around 11:15 a.m., a 2018 Freightliner truck equipped with a McNeilus Truck & Manufacturing refuse body was traveling southbound on Lanetown Road in Albemarle County near Crozet, Virginia.The refuse truck, operated by Time Disposal LLC, had a 30-year-old driver and two passengers as it drove towards a highway–railroad grade crossing, later identified. The crossing is active and includes advance warning signs and pavement markings on its approach, the National Transportation Safety Board said. The crossing is also equipped with crossbuck signs, warning lights, bells and gates.As the truck neared the highway a westbound Congressional Special Amtrak train was approaching the highway–railroad grade crossing.The NTSB said the train’s lead locomotive was equipped with a forward-facing track image camera. Data that was taken from the camera showed that as the crossing came into view, the gates were down and the truck was on the grade crossing. Witnesses to the crash reported that the refuse truck entered the crossing after the gates were down.The train’s recorder showed the Amtrak train was traveling about 61 mph when the engineer applied emergency braking. The train struck the left rear of the refuse truck, causing the truck to rotate counterclockwise and then collide with a railroad signal next to the tracks. The refuse body separated from the truck, and the truck’s two passengers were ejected, according to the NTSB.As a result of the crash, one passenger in the truck died. The other truck passenger had serious injuries and the driver of the truck had minor injuries. Three Amtrak crewmembers and three train passengers sustained minor injuries, the NTSB said.NTSB investigators and Albemarle County Police Department officials and the Federal Bureau of Investigation documented the crash scene and the characteristics of the crossing, the train and the refuse truck. The track and operational characteristics of the crossing signals were also examined and documented.The crash remains under investigation.  2354

  

A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. "On behalf of the senior leaders at DC VAMC, Veterans Integrated Service Network (VISN) 5 and the Veterans Health Administration (VHA), we concur with OIG's findings and recommendations and provide the attached action plans," the Office of the Undersecretary for Health said in response to the report.The investigation into the Washington DC VA Medical Center, which provides care to almost 100,000 veterans and employs more than 2,000 people, began in March 2017 after a confidential complaint, according to the inspector general's report.The-CNN-Wire? & ? 2018 Cable News Network, Inc., a Time Warner Company.
