A driver was killed on Interstate 24 when a piece of concrete went through his windshield. Police said the object was likely thrown. The incident happened around 5 a.m. Tuesday near Shelby Avenue. Officials with Metro Nashville police said 54-year-old Joe C. Shelton was killed.According to police, the piece of concrete was likely thrown from the Shelby Avenue Bridge. The incident closed the interstate for most the morning, causing major delays for commuters.     509

A hospital in Orange County, California was locked down Tuesday morning after police said someone at the facility called and claimed to have a gun.The incident was reported just after 8:15 a.m. local time at the Orange County Global Medical Center in Santa Ana, police said.Santa Ana police said they received a call from someone inside the hospital claiming to be in possession of a gun. The caller did not say anything further, police said.The hospital was locked down as a precaution while about 40 officers conducted a floor-to-floor search, including the facility’s basement and roof.  608

A hole was gouged in the deck of Russia's only aircraft carrier, the Admiral Kuznetsov, Tuesday when a crane fell on it while the vessel was undergoing repairs in the northern city of Murmansk, according to Russian state media.The accident took place after a power supply disruption caused the pumps on the floating dry dock, which was holding the aircraft carrier, to break down, causing the structure to sink.The dock, known as PD-50, is one of the world's largest floating dry docks and the biggest in Russia, measuring 330 meters long (1,082 feet) and weighing more than 80,000 tons.Following the disruption, PD-50 quickly became submerged, causing two cranes to fall onto the deck of the Russian vessel, leaving a hole measuring four meters by five meters (13 feet by 16.4 feet), state media Sputnik said.The aircraft carrier had been exiting the dock when the incident occurred, state media TASS said, and the vessel is sheltering at the 35th Ship Repair Plant."For now, the circumstances look like this: when the Admiral Kuznetsov was lowered, a power surge occurred from the shore, pumps shut down, and the dock sank," Zvezdochka Shipyard spokesman Yevgeny Gladyshev told TASS.Russian officials told state media the damage would not delay repairs on the Admiral Kuznetsov, which are due to be completed by 2021.The 58,000-ton carrier went in for repairs and refit in May, more than a year after it returned from a mission off Syria that saw it spewing thick black smoke while at sea and also losing two of its 15 aircraft to crashes.Carl Schuster, a former director of operations at the US Pacific Command's Joint Intelligence Center, told CNN the Russian navy was already in a poor state of repair and the loss of the dock wouldn't help."The loss of one (floating dock) may not be that serious except we don't know the condition of the others ... I don't think it's beyond the realms of possibility that they're in serious trouble in terms of their ability to do work on that carrier," he said. 2046

  

A former model has come forward with an allegation that President Donald Trump sexually assaulted her in his VIP suite at the U.S. Open tennis tournament in 1997.Former model Amy Dorris told The Guardian that the then-real estate tycoon "shoved his tongue down my throat" and then grabbed her buttocks and breasts despite her denial of his advances.According to The Guardian, Trump's personal lawyers denied the accusation "in the strongest possible terms." Several people close to Dorris, including her mother, her friend and a therapist, told the newspaper that Dorris had told them of the alleged assault either immediately after the incident happened or in the years since.Dorris also provided The Guardian with several photos of her, her then-boyfriend Jason Binn and Trump at the tennis tournament.The White House has not yet responded to media requests for comment about The Guardian's report.According to the newspaper, Dorris was in New York in 1997 with Binn, a friend of Trump's. At the time, Trump was married to his second wife, Marla Maples.During the tournament, Dorris got up from her seat to use the restroom because she was having trouble with her contact lenses. When she was finished in the bathroom, she says she found Trump waiting outside the door.At that point, Dorris says Trump forced himself on her "after a brief exchange." She allegedly told Trump, "no, please stop," but said he "didn't care.""I just kind of was in shock," Dorris told The Guardian. "I felt violated, obviously. But I still wasn't processing it and just was trying to go back to talking to everyone and having a good time because, I don't know, I felt pressured to be that way."Dorris also claims while she wasn't sure if she got specific about the incident with her boyfriend, she later told Binn that he had to "do something" about Trump because he was "all over her."Dorris spent the next few days with Binn in New York, and saw Trump several more times during her trip. She said Trump did not make any more physical moves but continued to pursue her with leading questions.Dorris told The Guardian that she continued to stay in New York because she hadn't fully processed what had happened."I was there from Florida and I was with Jason. I had no money, nowhere to go. We were going from event to event and it was overwhelming," Dorris told The Guardian. "People spend years around people who have abused them, that's what happens when something traumatic happens, you freeze."The allegation is just the latest assault allegation made against Trump in recent years. Trump is currently facing two lawsuits regarding assault allegations, and several women came forward prior to the 2016 election to formally accuse Trump of assault. During that election cycle, a video leaked in which Trump boasted about sexually assaulting women.Dorris considered coming forward in 2016 with her allegation but decided to remain silent for her family's safety. But according to The Guardian, she wants to be a role model for her 13-year-old twin daughters."I want them to see that I didn't stay quiet, that I stood up to somebody who did something that was unacceptable," Dorris told The Guardian. 3189

  

A federal court in Texas sentenced ice cream manufacturer Blue Bell Creameries to pay .25 million in criminal penalties for shipments of contaminated products linked to a 2015 listeriosis outbreak, the Justice Department announced Thursday.Blue Bell pleaded guilty in May 2020 to two misdemeanor counts of distributing adulterated ice cream products.The sentence, imposed by U.S. District Judge Robert Pitman in Austin, Texas, was consistent with the terms of a plea agreement previously filed in the case.“The results of this investigation reflect the determination of the Defense Criminal Investigative Service to hold companies that sell food products to the military accountable and ensure they comply with food safety laws,” said Michael Mentavlos, Special Agent-in-Charge of the DCIS Southwest Field Office. “The health and safety of our service members and their dependents are of paramount importance.”The .25 million fine and forfeiture amount is the largest-ever criminal penalty following a conviction in a food safety case.“American consumers must be able to trust that the foods they purchase are safe to eat,” said Acting Assistant Attorney General Jeffrey Bossert Clark of the Justice Department’s Civil Division. “The sentence imposed today sends a clear message to food manufacturers that the Department of Justice will take appropriate actions when contaminated food products endanger consumers.”According to the plea agreement, Texas state officials notified Blue Bell in February 2015 that samples of two ice cream products from the company’s Brenham, Texas factory tested positive for Listeria monocytogenes, a dangerous pathogen that can lead to serious illness or death in vulnerable populations such as pregnant women, newborns, the elderly, and those with compromised immune systems.Blue Bell directed its delivery route drivers to remove the remaining stock of the two products from store shelves, but the company did not recall the products or issue any formal communication to inform customers about the potential Listeria contamination.Two weeks after receiving notification of the first positive Listeria tests, Texas state officials informed Blue Bell that additional state-led testing confirmed Listeria in a third product.Blue Bell again chose not to issue any formal notification to customers regarding the positive tests. Blue Bell’s customers included military installations.In March 2015, tests conducted by the Food and Drug Administration (FDA) and Centers for Disease Control and Prevention (CDC) linked the strain of Listeria in one of the Blue Bell ice cream products to a strain that sickened five patients at a Kansas hospital with listeriosis, the severe illness caused by ingestion of Listeria-contaminated food.At least three people in Kansas died as a result of the contamination, according to the CDC.The FDA, CDC, and Blue Bell all issued public recall notifications on March 13, 2015. Subsequent tests confirmed Listeria contamination in a product made at another Blue Bell facility in Broken Arrow, Oklahoma, which led to a second recall announcement on March 23, 2015.According to the plea agreement with the company, FDA inspections in March and April 2015 revealed sanitation issues at the Brenham and Broken Arrow facilities, including problems with the hot water supply needed to properly clean equipment and deteriorating factory conditions that could lead to insanitary water dripping into product mix during the manufacturing process.Blue Bell temporarily closed all of its plants in late April 2015 to clean and update the facilities.
