There's an acute nursing shortage in the United States, but schools are turning away thousands of qualified applicants as they struggle to expand class size and hire more teachers for nursing programs.In America, experienced nurses are retiring at a rapid clip, and there aren't enough new nursing graduates to replenish the workforce. At the same time, the nation's population is aging and requires more care."It's really a catch 22 situation," said Robert Rosseter, spokesman for the American Association of Colleges of Nursing."There's tremendous demand from hospitals and clinics to hire more nurses," he said. "There's tremendous demand from students who want to enter nursing programs, but schools are tapped out."There are currently about three million nurses in the United States. The country will need to produce more than one million new registered nurses by 2022 to fulfill its health care needs, according to the American Nurses Association estimates.That's a problem.In 2017, nursing schools turned away more than 56,000 qualified applicants from undergraduate nursing programs. Going back a decade, nursing schools have annually rejected around 30,000 applicants who met admissions requirements, according to the American Association of Colleges of Nursing."Some of these applicants graduated high school top of their class with a 3.5 GPA or higher," said Rosseter. "But the competition to get into a nursing school right now is so intense."Because of the lack of openings, nursing programs across the board -- in community colleges to undergraduate and graduate schools -- are rejecting students in droves.Erica Kay is making her third attempt to get into a nursing program offered in a community college near where she lives in southern California.Kay, 35, already is a certified surgical technician and a certified medical assistant."I've been working in health care since I was 21. This is my passion," she said. "I know I will be a great nurse and I'm studying very hard to get accepted into a program," she saidShe's taken the standardized admissions test for nursing schools twice and applied to three community colleges. She didn't get in."One school responded in a letter they had 343 applications and only accepted 60 students," she said. Another school had 60 slots for 262 applications."Some programs won't even consider you if you score less than 80% even if you meet all other criteria," she said. Kay is retaking the nearly four-hour-long test next month, hoping to better her score."It shocks and upsets me that there are so many hurdles to get into nursing school when we have a nursing shortage," said Kay. "But I am going to keep trying."Jane Kirschling, dean of the University of Maryland School of Nursing in Baltimore, said her school admits new students in the undergraduate program twice a year."We're averaging 200 applications each time for 55 slots," she said. "So we're turning away one student for every student we accept."She said the nursing profession has surged in popularity for a few reasons. "Nursing offers an entry-level living wage with which you can support a family," said Kirschling.There's built-in flexibility and mobility. "You can work three 12-hour shifts and get four days off," she said. And nurses aren't locked into a specific location, employer or specialty for the rest of their lives. "There's tremendous growth opportunity," said Kirschling.But Kirschling said increasing school class size to accommodate more students isn't easy or practical.For one thing, nursing schools are struggling to hire more qualified teachers. "The annual national faculty vacancy rate in nursing programs is over 7%. That's pretty high," said Rosseter. "It's about two teachers per nursing school or a shortage of 1,565 teachers."Better pay for working nurses is luring current and potential nurse educators away from teaching. The average salary of a nurse practitioner is ,000 compared to an average salary of ,575 for a nursing school assistant professor, according to the American Association of Nurse Practitioners.Mott Community College in Flint, Michigan, last year reduced its new admissions from 80 to 64 students accepted twice a year into its two-year associate degree in nursing program.The move was partly in response to a decision by the Michigan Board of Nursing to shrink the nursing student-to-faculty ratio for clinical training in hospitals and clinics. This was aimed at improving safety and avoiding crowded clinical settings."It changed from 10 students for one educator to 8 students. So we had to adjust our class size accordingly," said Rebecca Myszenski, dean of the division of Health Sciences at Mott Community College.Kirschling's school in Baltimore has made similar adjustments. "We used to send eight to 10 nursing students per instructor to hospitals for clinical rotations. Now it's six students," she said.Pediatrics, obstetrics and mental health are the areas where nursing students have the most unmet demand for clinical training," said Kirschling. "As we try to increase the number of nursing students, these three areas will be bottlenecks for nursing programs."Rosseter agrees that class size presents another challenge for nursing schools. "There's not enough available clinical space to train students," he said.Despite the constraints, nursing programs are thinking of ways to accommodate more students."We're expanding our program to new campuses, we're looking at new models of partnering with hospitals to allow [their] nursing staff to [be able] to teach," said Tara Hulsey, dean of West Virginia University's School of Nursing.For example, Anne Arundel Community College in Arnold, Maryland, offers an accelerated associate nursing program that allows qualified paramedics or veterans to be admitted straight into the second year of the two-year program.In Flint, Mott Community College has partnered with University of Michigan's accelerated 16-month undergraduate program designed for veterans with medical experience who want to transition into a nursing career."These bridge programs could really help with the [nursing] shortage," said Myszenski. "You have to address the nursing shortage by thinking out of the box."  6239

It doesn't matter if you get dirty."Fifteen seconds later, the mother and daughter from Honduras were together in the U.S. And soon a U.S. Border Protection agent approached on an all-terrain vehicle to take them away in custody.U.S. Customs and Border Protection said Tuesday that the San Diego sector has experienced a "slight uptick" in families entering the U.S. illegally and turning themselves in to agents since the caravan of Central American migrants arrived in Tijuana two weeks ago.Thousands of migrants on the Mexico side of the border are living in crowded tent cities in Tijuana after a grueling weekslong journey through Mexico on foot and hitching rides with the goal of applying for asylum in the U.S. Frustrated with the long wait to apply, with the U.S. processing 100 requests at most each day, some migrants are trying to cross over clandestinely.Rachel Rivera, 19, told The Associated Press that Honduras had become unlivable. Moments before flattening herself under the fence, she said she was slipping through to the U.S. in an attempt to "give a better life" to her daughter Charlot.An AP video journalist also witnessed more than two dozen migrants scale a fence between Mexico and the U.S. on Monday evening. Once across, entire families raised their hands before border patrol agents who arrived swiftly in white trucks.It's unclear where the families were taken from there.On a typical day before the caravan arrived in Tijuana, U.S. border patrol agents in the San Diego area detained about 120 or so people trying to cross the border illegally from Mexico.President Donald Trump issued a proclamation in November suspending asylum rights for people who try to cross into the U.S. illegally. Rights groups question the legality of that proclamation.U.S. Customs and Border Protection spokesman Ralph DeSio said the U.S. was trying to deter illegal crossings by issuing the proclamation.The U.S. has an established process for asylum seekers to present themselves in an "orderly" manner at a port of entry, DeSio told AP via email. "When people choose to ignore that process, they put themselves in danger and, in the case of families, they choose to put the lives of their children at risk."Trump took to Twitter again Tuesday to drum up support for a better border wall, arguing that the expense would be less than the U.S. incurs each year due to illegal immigration.People mainly from Honduras but also from El Salvador and Guatemala formed the caravan to Tijuana, seeking safety in numbers while crossing Mexico to avoid criminals and the fees demanded by the gangs that prey on migrants. Dozens of the migrants have told AP they are fleeing poverty and searching for a better life, while many also tell of harrowing violence and death threats back home.Margarita Lopez, a migrant from Honduras, said she would definitely jump the fence to the U.S. if she got the chance. But in the meantime, Lopez stood in line Tuesday to request a humanitarian visa from Mexican officials that would allow her to live and work in Mexico for a year.Standing nearby, Luis Fernando Vazquez, a migrant from Guatemala, said he won't make a run for the border."I'm not like that," he said. There are disturbing allegations of abuse in a Livonia, Michigan nursing home, with some of it caught on a hidden camera.The family of the patient is suing; saying the abuse to their elderly father was partially motivated by race.The allegations are hard to hear and even harder to see, a caretaker at Autumnwood telling her then 87-year-old patient to “shut up”, calling him derogatory names, and throwing him in and out of his wheelchair.All of it was caught on a hidden camera over just 2 days - a sliver of the 6 months of alleged abuse.Husein Younes, the then 87-year-old Lebanese man, went to Autumnwood for help to recover from a surgery in May of 2015.The hidden camera was placed after Younes had been complaining for months about the behavior to his family.“I raised concerns with the Autumnwood many times and they assured me everything would be taken care of,” says Husein’s son Salim Younes.“This is one of the most troubling cases I have ever seen. It’s chilling and its literally a horror house and I would wish this on no one,” says the family attorney Jonathan Marko.The lawsuit against autumnwood and its owner claims the abuse was racially motivated.Autumnwood did release a statement that reads:Autumnwood of Livonia takes any allegation of abuse and inappropriate care seriously, and exhaustively investigates such claims to ensure the safety of its residents. Autumnwood’s policy is not to comment on residents in their facility to protect their privacy, and to comply with privacy laws and regulations related to health information. However, now that a family of a former resident at Autumnwood of Livonia, the Younes family, who has filed a lawsuit against Autumnwood of Livonia, made the decision to publicly litigate their lawsuit in the press and make public allegations and information related to the care and condition of Mr. Younes, my client now has the right to make a public statement.In December 2015, the Younes family’s attorney raised concerns to staff of Autumnwood of Livonia about the treatment of Mr. Younes after he had been discharged from the facility. The Younes family’s attorney informed staff that they believed that Mr. Younes had been abused at the facility. Despite requests for additional information, no details were offered. My client immediately conducted an internal investigation, reported the allegations to the state survey agency that regulates nursing homes, and reported the allegations to the local police department consistent with state and federal regulations. My client and the notified governmental agencies, were unable to substantiate the allegations with the information we had at the time.Although the Younes family had in their possession in December 2015 a video that provided information related to the allegations of abuse, the existence of the video itself was not disclosed to my client until May of 2016, when the video was sent to my client along with a demand for payment of monetary damages. The actions depicted in the video are in no way illustrative of the quality care that is provided by the caring staff at Autumnwood on a daily basis. When Autumnwood received this new information and video, another internal investigation was immediately launched, and the new information provided by the Younes’ attorney, five months after the alleged incident, was turned over by Autumnwood to the state survey agency and law enforcement.The employees identified from the video have been terminated, and additional training has been provided to all employees. Autumnwood continues to assist the involved state regulatory and law enforcement agencies with their investigations of the former staff and is committed to continuing to provide quality care to all of the residents and families they have the privilege to serve.As the family has filed a civil complaint and the matter is in litigation, Autumnwood cannot comment any further. 3917
