CINCINNATI -- A preliminary investigation reports both technical problems and human error may have played roles in first responders' failure to locate a teen who was found dead of asphyxiation in a van parked in a high school parking lot Tuesday.According to documents from an internal review obtained by Scripps station WCPO in Cincinnati, the 911 operator who answered Kyle Plush's second emergency call said she couldn't hear him. Plush, 16, was found dead in a minivan near Seven Hills School hours later Tuesday night. In one 911 call, he said he was stuck in a van outside the school. In the second call, he described the make, model and color of the minivan where he was trapped and dying.Investigators don't believe there was a failure in the phone system at that time, so it's not clear why the second operator couldn't hear Plush. But the 911 operators' computers experienced trouble "around that same time frame," one of the internal documents states. The operator said her screen froze, preventing her from properly documenting the call. TIMELINE: What happened the day Kyle Plush died?Still, the operator tried sending a text message to Plush, asking him for the address of the emergency. She tried calling him twice, records show. He never responded.That operator's supervisors found her work in that incident was "unacceptable," according to one of the internal documents. Police Chief Eliot Isaac said Thursday that she was placed on administrative leave.?"Something went wrong here, and we need to find out why were weren't able to provide that help," Isaac said. RELATED: As Kyle Plush pleaded for help, why didn't officers find him?After Plush's first call, the 911 operator used cellphone GPS information to point police officers to the thrift store parking lot across from the school. Plush was within feet of those coordinates. Two Cincinnati police officers arrived about two minutes later, but said they didn't see anything. Officials haven't said exactly where the officers searched. They tried calling Plush's phone, but he didn't answer. It was then that Plush called 911 the second time. "This is not a joke," he said. "I am trapped inside a gold Honda Odyssey van in the parking lot of Seven Hills. ... Send officers immediately. I'm almost dead."Plush's words were picked up by the recording, even though the 911 operator said she couldn't hear any sounds on the line. RELATED: Sheriff: Deputy never looked in Kyle Plush's vanA few minutes later, a Hamilton County deputy called in to the emergency dispatch center to say that he had also looked for the caller, but didn't find anything. Chief Deputy Mark Schoonover said on WLW radio Friday that the deputy looked into a van, but it wasn't the right one.?"He did look into some vehicles. He looked into a van, but he never looked into the victim's vehicle," Schoonover said. "He never located that."Cincinnati police investigators who reviewed security camera footage also said the deputy looked into many vehicles, including a van, but never made it to the far part of the parking lot.Instead, a family member found Plush dead inside the van at about 9 p.m., according to police. Hamilton County Coroner Lakshmi Sammarco said he died of asphyxia caused by chest compression. Officials haven't yet said what pressed so hard into Plush's chest that he suffocated. Isaac, Sheriff Jim Neil and Prosecutor Joe Deters have all ordered investigations into what happened.? 3544

CINCINNATI -- Police officers trying to arrest a man ended up fatally shooting him and wounding one of their own after the man struggled and drew a realistic pellet gun.Police were serving a robbery warrant for 20-year-old James Clay in an apartment in Cincinnati at about 3:40 p.m. Friday, according to Police Chief Eliot Isaac.The officers knocked, but there was no response. (KGTV) - Chula Vista’s Police Chief and a city councilman hosted a public safety meeting in a community where most would think safety isn’t an issue.  Chief Roxana Kennedy and Councilman John McCann met with several residents Wednesday night at the Montevalle Community Center.“I feel the balance is like this:  Housing, people, lights, traffic, less parking and police protection and presence are here,” said Chula Vista resident Dianne Farsetta while holding one hand up and one hand down. “It is a little scary.  We want to be in a safe environment.”Community groups on Facebook and Nextdoor appear to post more instances of crimes throughout the communities of Eastlake and Otay Ranch. Farsetta said the city is growing faster than its police force.However, Councilman McCann says the crime in his district is still less than half the crime level in other Chula Vista districts.  Additionally, McCann said Chula Vista is still one of the safest cities of its size in the country.  Nevertheless, McCann and Chief Kennedy listened to the community Wednesday night.“We want to make sure that we’re working with our police and our community to continue to make it as safe as it is,” he said.Farsetta and several others want to see more police on the streets and a police substation on the city’s eastern side.  Headquarters is on the western side of the city.McCann said the city’s immediate need is more officers on patrol.  He added he wants a substation but that the estimated million price tag is too much for Chula Vista. 1598

  

CHULA VISTA, Calif. (KGTV) — Now that guidance has been issued for playgrounds to reopen in California, Chula Vista says it reopened all of its 50 playgrounds on Friday.The city said the reopening process for its playgrounds includes guidance for visitors to follow to help curb the spread of the coronavirus:Visitors over the age of two need face coveringsUsers should wash and sanitize hands before and after useMaintain six-foot distancing from those outside your householdCaregiver supervision/monitoring required at all times to ensure guidelines are followedFollow maximum occupancy rules that are posted at entrance to each playgroundChula Vista says it will implement regular cleaning protocols, but warn that the locations may get a lot of use as they reopen. China's Tiangong-1 space lab re-entered the Earth's atmosphere around 8.15 a.m. Monday (8:15 p.m. ET Sunday) in a fiery fall, China's Manned Space Agency said.Tiangong-1 plummeted into the middle of the South Pacific, the space agency said."Most parts were burned up in the re-entry process," the agency said.The out-of-control 40-foot long Tiangong-1, or "Heavenly Palace," is one of China's highest profile space projects. The unmanned space lab was launched in September 2011 as a prototype for China's ultimate space goal: a permanent space station that is expected to launch around 2022.But the Chinese government told the United Nations in May 2017 that its space lab had "ceased functioning" in March 2016, without saying exactly why.The incident was embarrassing for China's space program but it hasn't delayed its progress. In September 2016, China launched its second space lab, Tiangong-2.While it is not uncommon for debris such as satellites or spent rocket stages to fall to Earth, large vessels capable of supporting human life are rarer.NASA's first space station, Skylab, fell to Earth in an out-of-control re-entry in 1979, burning up harmlessly in the process.The last space outpost to drop was Russia's 135-ton Mir station in 2001, which made a controlled landing with most parts breaking up in the atmosphere. 1338
