CINCINNATI -- A preliminary investigation reports both technical problems and human error may have played roles in first responders' failure to locate a teen who was found dead of asphyxiation in a van parked in a high school parking lot Tuesday.According to documents from an internal review obtained by Scripps station WCPO in Cincinnati, the 911 operator who answered Kyle Plush's second emergency call said she couldn't hear him. Plush, 16, was found dead in a minivan near Seven Hills School hours later Tuesday night. "I am trapped inside a gold Honda Odyssey van in the parking lot of Seven Hills. ... Send officers immediately. (KGTV) -- The Sweetwater Union High School District announced Thursday that it plans to reopen August 3.According to the district, only distance or virtual learning will be available on August 3. No on campus instruction will be available.On August 28, the district will transition to "Phase 1" where 10 percent of students will be allowed on campus at any given time.As public health orders permit, the campus will move into phase two, where 20 percent of students will be allowed on campus. (KGTV) - Police have arrested three people in the death of a man found slumped over the steering wheel of his still moving vehicle.Chula Vista Police arrested Britney Canal, 29, of Chula Vista, Cesar Alvarado, 39, of National City, and Michael Pedraza, 27, of San Diego, in the murder of 59-year-old Mario Serhan.Witnesses found Serhan slumped over the steering wheel of his vehicle with an apparent gunshot wound to the head just before 1 p.m. on April 11.
