BORDEN, Ind. — A plane crashed Friday morning in southern Indiana, killing multiple people. According to WHAS, the ABC affiliate in Louisville, the crash happened in Borden, Indiana at about 11:30 a.m., in the 2600 block of Crone Road. The plane that crashed could hold about 10 passengers, Clark County Sheriff Jamie Noel said. No survivors have been located at this time, Noel said. The sheriff's office said there were multiple fatalities, WHAS reported.He said it was very difficult to access the scene of the crash due to it being in a wooded area.  582

Brookstone filed for bankruptcy and will close its remaining 101 mall stores.The mall and airport seller, best known for massage chairs, quirky gadgets, and travel luggage, filed for Chapter 11 bankruptcy in federal court on Thursday. It was Brookstone's second bankruptcy filing in four years.The company will keep its 35 airport stores and website open and running while it attempts to find a buyer. It has secured a million loan to finance operations during the sale.Brookstone's CEO said in a statement that its airport and online businesses were successful, but an "extremely challenging retail environment at malls" forced the company to close its stores there.Unabated traffic losses at malls have plagued brick-and-mortar sellers. Mall vacancies reached a six-year high last quarter as a wave of stores closed, including Walgreens, Bon-Ton, Sears and Kmart, Best Buy, Kay and Jared, Matress Firm, and GNC."They're trapped in hundreds of these B and C malls, whose traffic has been in serial decline," said Mark Cohen, the director of retail studies at Columbia Business School. "Where they are in triple-A malls, they're faced with very high rent."In 2014, Brookstone filed for bankruptcy and was sold to a Chinese consortium for 6 million. BRUCE TOWNSHIP, Michigan — A company that supplies equipment to the University Hospitals fertility center said its storage tank did not malfunction. “The early stages of our investigation into this unfortunate incident indicate it was the result of human error,” Custom Biogenic Systems said in a detailed statement sent to E.W. Scripps Co. TV station WEWS in Cleveland, Ohio.In a letter to patients on March 26, University Hospitals said that a remote alarm system on a storage tank that was designed to alert a University Hospitals employee to changes like temperature swings was turned off. UH said that on the evening of Saturday, March 3, the temperature inside the tank, which contained thousands of eggs and embryos, rose. UH does not know when the alarm was turned off. During the period when the alarm was off, UH said it had been experiencing “difficulty with what is called the liquid nitrogen automatic fill on the storage tank” for “several weeks.” Liquid nitrogen is added to the storage tank to keep specimens frozen, and it can be added manually or automatically. According to UH, “We had been working with the tank manufacturer who had previously provided instructions on the necessary maintenance to ‘thaw’ the storage tank to correct this difficulty. To do that required transferring all specimens to an extra storage tank previously provided by the manufacturer. This process takes several weeks, and had begun when this event occurred, though no eggs or embryos had yet been moved to the extra tank.”Custom Biogenic Systems, of Bruce Township, Mich., says the extra tank was available to UH on Aug. 15, 2017, UH finalized its arrangements for delivery of the tank on Oct. 27, 2017, and the tank was delivered on Nov. 2, 2017. That’s almost four months to the day before 950 UH patients lost 4,000 eggs and embryos in an event the hospital would later call “catastrophic." With the automatic fill not working on the original tank, UH added liquid nitrogen to the tank manually. This was done by connecting the storage tank with a line to a tank of liquid nitrogen from the Embryology Lab. But, according to UH, “For several days prior to the weekend in question, a manual fill could not be done using the line in the Embryology Lab because there were no liquid nitrogen tanks available. So, containers of liquid nitrogen were obtained from the Andrology Lab. Those containers were then manually poured into the top of the tank, while amounts of liquid nitrogen and temperature were monitored.” Custom Biogenic Systems says its tank is not designed to be filled by liquid nitrogen poured into the top of the tank.The company said its product manual states:  2744

BATON ROUGE, La. — Louisiana officials have announced two deaths from Hurricane Delta.Gov. John Bel Edwards said Sunday an 86-year-old man from St. Martin Parish died in a fire that started after he refueled a power generator in a shed. The governor said it didn't appear that the man had let the generator cool down before refueling it.Louisiana officials are also attributing the death of a 70-year-old woman to the hurricane. State officials said the Iberia Parish woman died in a fire caused by a natural gas leak.Earlier Sunday, officials in Florida said a 19-year-old tourist from Illinois drowned after being caught in a rip current unleashed by the remnants of Delta in the Gulf Coast.Delta made landfall Friday evening in southwest Louisiana as a Category 2 hurricane and then began weakening quickly over land. It came weeks after Hurricane Laura battered the same region.Laura claimed 32 lives, though only seven of the deaths came the day that the hurricane struck. Body camera video released Friday shows YouTube shooter Nasim Aghdam telling Mountain View Police she had no plans for violence hours before she opened fire at the company's San Bruno headquarters.The police recording shows officers identify Aghdam’s car in a Walmart parking lot in Mountain View at about 1:30 a.m. PT on April 3. Less than 12 hours later, Aghdam would open fire in a courtyard at the YouTube campus, injuring three people before turning the gun on herself. Mountain View Police said the license plate showed Aghdam's vehicle was linked to a San Diego missing persons report filed by her family. The Mountain View Police Department (MVPD) said seven record systems were checked during this time, including the Armed and Prohibited Persons System (APPS). The APPS cross-references records of persons who lawfully purchased weapons with the records of those who are prohibited from owning them. The systems checks for Aghdam came back negative, police said.Two officers knocked on the window and woke up Aghdam, who had been sleeping in the back seat. During their conversation, officers asked Aghdam if she knew her family in San Diego had reported her missing.“We didn’t get along together so I left them,” Aghdam said. She explained that she didn’t get along with her father and drove straight to Mountain View from San Diego. Aghdam also said she went to Northern California to start fresh in a place without memories.Officers asked Aghdam if she was taking medication or needed to take medication. She shook her head, “no."During questioning, Aghdam also told officers that she didn’t want to hurt herself or anyone else, and had no plans to commit suicide.Shortly after the police body camera recording ended, one of the officers called Aghdam’s family in San Diego to report that she had been found. Her father, Ismail Aghdam, answered the phone and confirmed that he had not been getting along with his daughter, police said. He thanked officers for the call and hung up.One hour later, the father called Mountain View Police to let them know his daughter had recently become upset about changes on the YouTube platform that had impacted videos she had created on living a vegan lifestyle, police said. The father suggested that may have been one of the reasons Aghdam was in Mountain View.Scripps station KGTV in San Diego talked with Aghdam’s family at their home in Menifee the evening of April 3. They said that they tried to warn authorities before the shooting.The family said she felt she reached the most people through her YouTube page but her relationship with the company turned ugly when YouTube changed its filters and she began to lose followers and viewers."I Googled 'Mountain View' and it was close to YouTube headquarters, and she had a problem with YouTube," her brother said.
