A report from the Tucson Police Department is revealing new details about the death of Carlos Adrian Ingram-Lopez while in TPD custody in April.The police department began its investigation into the incident hours after it happened on April 21. Nearly two months later, on June 19, the department finished its report and handed it off to TPD Chief Chris Magnus. After a news conference about the incident Wednesday, TPD released the full report to members of the news media.TIMELINE: What happened after Carlos Ingram-Lopez died while in TPD custodyThe report recommends termination for officers Samuel Routledge, Ryan Starbuck, and Jonathan Jackson, who had resigned the day before the investigation was completed.The discipline report focuses on how officers are supposed to treat someone in a state of “excited delirium," how it greatly increased the risk of dangerous physical distress and how the three officers failed to meet their standards and training.The report draws a number of conclusions about the officers involved in the incident. It says:The initial report of Ingram-Lopez's behavior should have prepared the officers to deal with excited delirium before they even saw him.Ingram-Lopez's behavior at the scene made excited delirium very clear.The report documents dates of when the officers had training on excited delirium at the academy and in other training sessions after that.Excited delirium and the likelihood of drug intake make overheating and rapid heartbeat something officers should anticipate.The fact that he was calling for water confirms they should have been more aware of his physical distress.The officers were trained on, and should have been alert to, signs of breathing trouble, like wheezing, and simply saying “I can’t breathe." Ingram-Lopez did both.The officers had been trained on the “recovery position” designed to reduce physical distress on a restrained suspect.One of the officers who arrived later said within 15 seconds, “Shouldn’t he be in the recovery position?” That officer is not being disciplined.The officers put a “spit sock” over Ingram-Lopez's face because of his choking and clearing his throat made them fear he would spit and spread COVID-19. The spit sock was available to officers even before the COVID outbreak.While officers did not use prohibited methods like neck holds, they noted Ingram-Lopez was a large man and one of the officers kneeled on his back for a sustained period.Officer Jonathan Jackson was Lead Police Officer -- slightly more senior than the other officers who first arrived at the scene. He was expected to take command and organize the other officers. The report says he failed to command adequately and organize the police response.Other officers either reacted appropriately or were with the grandmother, where they were not well aware of what was happening with Ingram-Lopez.Overall, the report concluded the officers ignored their training and were unaware or indifferent to Ingram-Lopez's situation and physical distress.Click here to read TPD's full report.KGUN's Craig Smith first reported this story. 3106

A Marine lance corporal who marched at a white supremacist rally in Charlottesville, Virginia last year has been kicked out of the service after he spent nearly a month jail.According to the Jacksonville Daily News in Jacksonville, North Carolina, Vasillios Pistolis, 19, was discharged from duty and kicked out of the Marine Corps on July 11. Pistolis was court-martialed on June 18 for an investigation into violations of Articles 92 and 107 of the Uniform of Military Justice.Among the violations covered in those articles are making "false statements," "failure to obey orders or regulations" and "dereliction of duty."Pistolis was sentenced to 28 days in confinement in the brig, a forfeiture of some of his paycheck and a reduction of rank.ProPublica and Frontline reported in June that Pistolis marched at the "Unite The Right" rally in Charlottesville, Virginia. At that rally, one counter-protester was killed when James Alex Fields drove his car into a crowd.ProPublica and Frontline's report also alleged that Pistolis was a member of a known white supremacist and neo-Nazi group.   1156

A toddler hanging from the roof after crawling out of his bedroom window managed to come away unscathed after falling into a passerby's arms.It's a situation that could have ended tragically if not for the quick actions of a good Samaritan.Police report Jill Harper happened to be driving by the 1300 block of Mills Street in Sandusky, Ohio and saw the toddler hanging from the edge of the roof. She ran over to help.According to the police report, the child fell into her arms, and she brought him inside."When I caught him I remember thinking, 'Oh my God, I caught him,'" said Harper."When I started pounding on the house, that's when he fell, and my hand was pounding.  So when he fell, he kind of came back and hit my arm, and then I caught him."Police said they did not notice any obvious bumps and bruises. The Sandusky Fire Department responded to the scene, looked him over and found no injuries.Kelsie Hellman, the boy's mother's cousin, signed a refusal form, not wanting him to be transported to the hospital, according to police.Sandusky police said the boy is currently in the custody of Summit County Children Services. Hellman told police she is trying to gain custody of the boy, whom she has looked after since he was 3 months old.How did he get out there? Hellman told police she was using the bathroom when the incident happened. She said the boy must have gone around the baby gate she put at the bottom of her stairs, allowing him to get up to his room where the window was left slightly open. The boy then apparently slid through the window, which opens onto the roof.Meanwhile, Sandusky Police Detective Kevin Youskievicz told Scripps station WEWS in Cleveland, he believes the child could have suffered serious injuries if Harper hadn't acted quickly."I can only describe her as a hero, she's a hero," Youskievicz said."Everyone says we need heroes, she is one,"According to the report, Hellman said her husband was fixing the window so the boy could not crawl out of it again. 2079

A San Francisco salon owner says she is “done” with the city and plans to close her shop permanently following backlash and controversy this last week after a video of Speaker of the House Nancy Pelosi inside the salon without a mask surfaced.Owner Erica Kious announced her decision Wednesday night on Tucker Carlson’s Fox News show.Kious owns ESalonSF in San Francisco. She says one of her stylists, who is an independent contractor, told her Speaker Pelosi was coming in for an appointment on August 31. Salons in San Francisco have been closed since March, and were told they could reopen for outdoor services on September 1.Kious released the video of Pelosi’s visit, showing the Speaker with wet hair and not wearing a mask at the time, the day after her appointment.Pelosi responded to the video saying her team had been told it was OK to come inside to have her hair done, and that she felt the incident was “a set-up.” A statement released by the stylist who did her hair reportedly agrees it was a set-up, and states he had gotten permission from Kious verbally the night before the appointment.ABC7 in the Bay Area reports some stylists at ESalonSF have quit, claiming Kious’ made them work during the pandemic in unsafe conditions.The video of Pelosi sparked reaction from a lot of people; including fellow stylists and salon owners, business owners in California, and President Donald Trump, among others. Kious now says she is “afraid to go back” to San Francisco.“I started to just get a ton of phone calls, text messages, emails, all my Yelp reviews… saying that they hope I go under and that I fail,” Kious told Carlson on Wednesday night. “So just a lot of negativity towards my business.”The news comes just two days after Kious thanked supporters after a GoFundMe account set up to support her and her salon raised more than 0,000.The account stated, "At the conclusion of this fundraiser, ALL donations will go directly to Erica to pay off any debts from the business that she is forced to shut down, expenses to relocate and reopen in a new location."It is not clear if Kious’ comments Wednesday night about closing permanently mean she will not re-open a salon in a different location.
