A report from the Tucson Police Department is revealing new details about the death of Carlos Adrian Ingram-Lopez while in TPD custody in April.The police department began its investigation into the incident hours after it happened on April 21. Nearly two months later, on June 19, the department finished its report and handed it off to TPD Chief Chris Magnus. After a news conference about the incident Wednesday, TPD released the full report to members of the news media.TIMELINE: What happened after Carlos Ingram-Lopez died while in TPD custodyThe report recommends termination for officers Samuel Routledge, Ryan Starbuck, and Jonathan Jackson, who had resigned the day before the investigation was completed.The discipline report focuses on how officers are supposed to treat someone in a state of “excited delirium," how it greatly increased the risk of dangerous physical distress and how the three officers failed to meet their standards and training.The report draws a number of conclusions about the officers involved in the incident. It says:The initial report of Ingram-Lopez's behavior should have prepared the officers to deal with excited delirium before they even saw him.Ingram-Lopez's behavior at the scene made excited delirium very clear.The report documents dates of when the officers had training on excited delirium at the academy and in other training sessions after that.Excited delirium and the likelihood of drug intake make overheating and rapid heartbeat something officers should anticipate.The fact that he was calling for water confirms they should have been more aware of his physical distress.The officers were trained on, and should have been alert to, signs of breathing trouble, like wheezing, and simply saying “I can’t breathe." Ingram-Lopez did both.The officers had been trained on the “recovery position” designed to reduce physical distress on a restrained suspect.One of the officers who arrived later said within 15 seconds, “Shouldn’t he be in the recovery position?” That officer is not being disciplined.The officers put a “spit sock” over Ingram-Lopez's face because of his choking and clearing his throat made them fear he would spit and spread COVID-19. The spit sock was available to officers even before the COVID outbreak.While officers did not use prohibited methods like neck holds, they noted Ingram-Lopez was a large man and one of the officers kneeled on his back for a sustained period.Officer Jonathan Jackson was Lead Police Officer -- slightly more senior than the other officers who first arrived at the scene. He was expected to take command and organize the other officers. A new lawsuit says a woman who tried on lipstick at a Sephora cosmetics store in Hollywood, California got herpes from the sample.The unidentified woman claims it happened at the store in October 2015, according to a report by TMZ. She ended up with herpes on her lip and says she never had it before the visit to the store.The documents say the store failed to clearly warn the woman of the risk of getting a disease from using samples of lipstick there.The lawsuit says the woman is suing over the emotional distress for getting an "incurable lifelong affliction." Sephora did not respond to the TMZ article, but a spokesperson for the retailer did respond to Fashionista, saying the health and safety of its customers is a priority. A US Army veteran who served two tours in Afghanistan has been deported to Mexico, US Immigration and Customs Enforcement said.The deportation follows an earlier decision by US authorities to deny Miguel Perez's citizenship application because of a felony drug conviction, despite his service and the PTSD he says it caused.Perez, 39, was escorted across the US-Mexico border from Texas and handed over to Mexican authorities Friday, ICE said in a statement.Perez, his family and supporters, who include Sen. Tammy Duckworth of Illinois, had argued that his wartime service to the country had earned him the right to stay in the United States and to receive mental health treatment for the PTSD and substance abuse."This case is a tragic example of what can happen when national immigration policies are based more in hate than on logic and ICE doesn't feel accountable to anyone," Duckworth said in a statement following reports of Perez's deportation. "At the very least, Miguel should have been able to exhaust all of his legal options before being rushed out of the country under a shroud of secrecy."Perez was born in Mexico and legally came to the United States at age 8 when his father, Miguel Perez Sr., a semi-pro soccer player, moved the family to Chicago because of a job offer, Perez told CNN earlier. He has two children born in the United States. His parents and one sister are now naturalized American citizens, and another sister is an American citizen by birth.It's a complicated case. Perez has said that what he saw and experienced in Afghanistan sent his life off the rails, leading to heavy drinking, a drug addiction and ultimately to his felony conviction."After the second tour, there was more alcohol and that was also when I tried some drugs," Perez said last month. "But the addiction really started after I got back to Chicago, when I got back home, because I did not feel very sociable."In 2010, he was convicted in Cook County, Illinois, on charges related to delivering more than 2 pounds of cocaine to an undercover officer. He was sentenced to 15 years and his green card was revoked. He had served half his sentence when ICE began deportation proceedings. He had been in the agency's custody since 2016.Perez has said he was surprised to be in ICE detention and mistakenly believed that enlisting in the Army would automatically give him US citizenship, according to his lawyer, Chris Bergin. His retroactive application for citizenship was denied earlier this month. While there are provisions for expediting troops' naturalization process, a main requirement is that the applicant demonstrate "good moral character," and the drug conviction was enough to sway the decision against his application, Bergin said.Perez enlisted in the Army in 2001, just months before 9/11. He served in Afghanistan from October 2002 to April 2003 and again from May to October 2003, according to his lawyer. He left the Army in 2004 with a general discharge after he was caught smoking marijuana on base.Perez went on a hunger strike earlier this year, saying he feared deportation would mean death. A principal at Education Alternatives in Bedford, Ohio is accused of having a sexual relationship with a student at the school, who was 17 at the time."He says the relationship started when they were upstairs in the gym alone, that she kissed him on the mouth," said Bedford Police Deputy Chief Rick Suts. "Since then, there had been numerous encounters in her van."The teenager told police that his relationship with Courtney Alfred continued for months.Police said the former student came to them last week. Suts said the school knew about the alleged relationship but after their own investigation said the allegations were not credible. Bedford police disagree."He's saying that he took video of them having sex and in that encounter, you see a tattoo. We got a search warrant to see her tattoo, that tattoo matches the one on the video," added Suts.Police said there were also text messages and that money was transferred from Alfred to the teenager. Police said the teen told them it was to keep him from telling her husband.Scripps station WEWS in Cleveland tried to contact Alfred at her home, but there was no answer. WEWS also called the central office for Education Alternatives, but the call was not returned. WEWS also left two messages for her attorney, Vince Ruffa, but Ruffa also did not return messages.Alfred was arrested. She was in court last week facing a sexual battery charge. A suspect has been arrested and the gun and mask have been recovered after an incident at the Boulevard Mall in Las Vegas, according to police. The gun appears to be a fake gun, according to police. The identity of the suspect will be released Friday morning.  Police said they received calls about a person with a gun at the Boulevard Mall on Maryland Parkway. No injuries were reported and police say no shots were fired. The mall was evacuated while officers made sure the person with a gun is not inside the mall. Deputy Chief Chris Jones told the media during a press briefing that video has been obtained of a person wearing a mask and carrying a long gun. The incident began shortly after 7 p.m. Watch video from inside the Boulevard Mall belowAs the news first broke earlier tonight, thoughts of the Las Vegas shooting came to mind.
