A Southwest Airlines flight with 117 people aboard slid off a runway on Thursday at California's Hollywood Burbank Airport during heavy rain, the Federal Aviation Administration said.There were no injuries reported, FAA spokesman Ian Gregor said.Flight 278, a Boeing 737-700 that originated in Oakland, California, came to a stop after running through an area of crushable material designed to bring an airplane to a halt, he said.The airport is open, and all airlines are operational, the airport tweeted. One runway has been closed.There were 112 passengers and five crew members on the flight, Southwest spokeswoman Brandy King said. Southwest tweeted the airline will refund the passengers ticket costs and will make an "additional gesture of goodwill."Around the time of the incident, visibility at the airport dropped to 1 mile. The area is experiencing heavy rain -- up to a half inch in an hour at one point, and a flood advisory is in effect for Los Angeles County, where the airport is located.The FAA implemented a ground stop that kept incoming flights in the air for an extra 45 minutes.Over six hours, the area received 1.66 inches of rain. A report from the Tucson Police Department is revealing new details about the death of Carlos Adrian Ingram-Lopez while in TPD custody in April.The police department began its investigation into the incident hours after it happened on April 21. Nearly two months later, on June 19, the department finished its report and handed it off to TPD Chief Chris Magnus. After a news conference about the incident Wednesday, TPD released the full report to members of the news media.TIMELINE: What happened after Carlos Ingram-Lopez died while in TPD custodyThe report recommends termination for officers Samuel Routledge, Ryan Starbuck, and Jonathan Jackson, who had resigned the day before the investigation was completed.The discipline report focuses on how officers are supposed to treat someone in a state of “excited delirium," how it greatly increased the risk of dangerous physical distress and how the three officers failed to meet their standards and training.The report draws a number of conclusions about the officers involved in the incident. It says:The initial report of Ingram-Lopez's behavior should have prepared the officers to deal with excited delirium before they even saw him.Ingram-Lopez's behavior at the scene made excited delirium very clear.The report documents dates of when the officers had training on excited delirium at the academy and in other training sessions after that.Excited delirium and the likelihood of drug intake make overheating and rapid heartbeat something officers should anticipate.The fact that he was calling for water confirms they should have been more aware of his physical distress.The officers were trained on, and should have been alert to, signs of breathing trouble, like wheezing, and simply saying “I can’t breathe." Ingram-Lopez did both.The officers had been trained on the “recovery position” designed to reduce physical distress on a restrained suspect.One of the officers who arrived later said within 15 seconds, “Shouldn’t he be in the recovery position?” That officer is not being disciplined.The officers put a “spit sock” over Ingram-Lopez's face because of his choking and clearing his throat made them fear he would spit and spread COVID-19. The spit sock was available to officers even before the COVID outbreak.While officers did not use prohibited methods like neck holds, they noted Ingram-Lopez was a large man and one of the officers kneeled on his back for a sustained period.Officer Jonathan Jackson was Lead Police Officer -- slightly more senior than the other officers who first arrived at the scene. He was expected to take command and organize the other officers. The report says he failed to command adequately and organize the police response.Other officers either reacted appropriately or were with the grandmother, where they were not well aware of what was happening with Ingram-Lopez.Overall, the report concluded the officers ignored their training and were unaware or indifferent to Ingram-Lopez's situation and physical distress.Click here to read TPD's full report.KGUN's Craig Smith first reported this story. 3106

  

A New Jersey meat processing company has recalled nearly 43,000 pounds of ground beef, including packages sold at Walmart stores around the country, due to possible E. coli contamination.The United States Department of Agriculture's Food Safety and Inspection Service announced Saturday that Lakeside Refrigerated Services, a Swedesboro, New Jersey company, is recalling ground beef packages that were produced on June 1.The affected products were sold under the Thomas Foods International and Marketside Butcher labels and feature the establishment number EST. 46841.The recall notice said items were shipped to retail locations nationwide. Marketside Butcher products are sold at Walmart, according to package labels of the recalled products.The FSIS categorized the situation as a Class I recall, which it defines as "a health hazard situation where there is a reasonable probability that the use of the product will cause serious, adverse health consequences or death."The recall notice said the ground beef could be contaminated with E. coli O157:H7, which can cause dehydration, bloody diarrhea and abdominal cramps for 2–8 days, with an average of 3-4 days. Most people recover within a week, but some develop a type of kidney failure called hemolytic uremic syndrome, which the FSIS said is most common in children under the age of five and in older adults.The condition's symptoms include easy bruising, pallor and decreased urine output.The contamination was found during routine FSIS inspection, and the recall notice said there have been "no confirmed reports of adverse reactions due to consumption of these products."The affected products include:One-pound vacuum packages containing "MARKETSIDE BUTCHER ORGANIC GRASS-FED GROUND BEEF" and a use or freeze by date of 07/01/20 and lot code P-53298-82.One-pound vacuum packages containing four quarter-pound pieces of "MARKETSIDE BUTCHER ORGANIC GRASS-FED GROUND BEEF PATTIES" and a use or freeze by date of June 27, 2020 and lot code P-53934-28.Three-pound vacuum packages containing three one-pound pieces of "MARKETSIDE BUTCHER ORGANIC GRASS-FED GROUND BEEF 93% LEAN / 7% FAT" and a use or freeze by date of 07/01/20 and lot code P53929-70.One-pound tray packages containing four quarter-pound pieces of "THOMAS FARMS GRASS-FED GROUND BEEF PATTIES 85% LEAN / 15% FAT" and a use or freeze by date of 06/25/20 and lot code P53944-10.Four-pound tray packages containing 10 quarter-pound. pieces of "THOMAS FARMS GRASS-FED GROUND BEEF PATTIES 80% LEAN / 20% FAT" and a use or freeze by date of 06/25/20 and lot code P53937-45.One-pound vacuum packages containing four quarter-pound pieces of "THOMAS FARMS GRASS-FED GROUND BEEF PATTIES 85% LEAN / 15% FAT" and a use or freeze by date of 06/27/20 and lot code P53935-25.One-pound vacuum packages containing "VALUE PACK FRESH GROUND BEEF 76% LEAN / 24% FAT" and a use or freeze by date of 07/01/20 and lot code P53930-18.Customers should check their refrigerators or freezers and either throw away or return the packages, the recall notice said. A US Navy helicopter crashed onto the flight deck of an aircraft carrier in the Philippines Sea on Friday, injuring personnel on board.The MH-60 Seahawk hit the flight deck of the USS Ronald Reagan shortly after takeoff around 9 a.m., according to a statement released by the Navy's Seventh Fleet. All injured are in stable condition and are being evaluated by medical staff.The Navy did not say how many people were injured but that some "will be medically evacuated ashore" without specifying a location.An investigation has been opened into the cause of the crash, according to the statement.The incident occurred during routine operations in the Philippine Sea, which spreads north and east of the Philippines.The aircraft carrier "remains fully mission capable" and normal flight operations to support the mission of "support and security and stability in the Indo-Pacific Region" have resumed, the Navy said.Last week the Nimitz-class, or, nuclear-powered aircraft participated in the International Fleet Review near South Korea's Jeju Island.In May 2017, the USS Ronald Reagan began sea trials in the Pacific before replacing the USS Carl Vinson near the Korean Peninsula.The 1,092-foot Reagan carries a crew of 4,539 and is equipped with roughly 60 aircraft, according to the Navy.
