CINCINNATI -- A preliminary investigation reports both technical problems and human error may have played roles in first responders' failure to locate a teen who was found dead of asphyxiation in a van parked in a high school parking lot Tuesday.According to documents from an internal review obtained by Scripps station WCPO in Cincinnati, the 911 operator who answered Kyle Plush's second emergency call said she couldn't hear him. Plush, 16, was found dead in a minivan near Seven Hills School hours later Tuesday night. In one 911 call, he said he was stuck in a van outside the school. In the second call, he described the make, model and color of the minivan where he was trapped and dying.Investigators don't believe there was a failure in the phone system at that time, so it's not clear why the second operator couldn't hear Plush. But the 911 operators' computers experienced trouble "around that same time frame," one of the internal documents states. "I am trapped inside a gold Honda Odyssey van in the parking lot of Seven Hills. ... Send officers immediately. CHULA VISTA (KGTV) — Three children are recovering Sunday morning after they were struck by a car in the Otay Ranch neighborhood of Montecito Village. The crash happened Saturday just before 5 p.m. on Santa Victoria Avenue, about a block east of Muroaka Elementary School. According to Chula Vista Police, the pedestrians were crossing the street when they were struck by a silver 2018 Toyota Corolla. The children were not in a marked crosswalk.The two younger children were transported to a nearby hospital to be treated for their injuries, said CVPD officer Justin Zualet. Witnesses at the scene described them in good condition.The 15-year old female was transported in critical but stable condition with a head injury, lacerations, and abrasions.Zualet said all are expected to survive.The driver remained at the scene and is cooperating with the investigation. Police at the scene said the driver, a male in his 50s, told officers he was using his cell phone at the time of the crash. No further details were given at this time.Drugs and alcohol do not appear factors in the collision, Zualet said. The incident remains under investigation. 1154

CHULA VISTA, Calif. (KGTV) - The Sweetwater Union High School District Monday handed out dozens of pink slips to administrators as the district deals with a million budget shortfall for the next school year. The board agreed during a meeting in Chula Vista Saturday to release up to 87 employees. Not every employee who receives a pink slip will lose their job. In a written statement, the district said the employees “may be released or reassigned from their position.” Community forums on the matter will be held at the end of the month. In September 2018, the district noticed a million dollar budget shortfall. An independent audit revealed the district was borrowing money faster than it could pay back. In December, the San Diego County Office of Education appointed a fiscal advisor with power to stay and rescind any financial decision made by the troubled district. China said Friday it will temporarily reduce tariffs on imports of American-made cars as it tries to negotiate a trade deal with the United States.Citing the meeting earlier this month between US President Donald Trump and Chinese leader Xi Jinping, the Chinese Finance Ministry said in a statement that it will remove the additional 25% tariffs on car imports from the United States for three months starting January 1.
