As hospitals are becoming overwhelmed with COVID-19 patients, North Dakota is permitting coronavirus-positive health care workers to continue assisting patients.The guidance stipulates that the health care workers must remain asymptomatic and take enhanced precautions in order to stay on the job. This comes as travel nurses are stretched thin as dozens of states are reporting record hospitalizations.Given the situation in North Dakota and elsewhere, it is possible more states will have to follow suit and continue using infected staff members in order to provide care. The CDC has spelled out guidance in these situations.The CDC says hospitals must exhaust a number of other guidelines, including adding travel nurses, postponing elective medical procedures, and postpone elective time off, before going into a crisis staffing situation.“If shortages continue despite other mitigation strategies, consider implementing criteria to allow HCP (health care personnel) with suspected or confirmed COVID-19 who are well enough and willing to work but have not met all Return to Work Criteria to work,” the CDC’s guidance states. “If HCP are allowed to work before meeting all criteria, they should be restricted from contact with severely immunocompromised patients (e.g., transplant, hematology-oncology) and facilities should consider prioritizing their duties in the following order:1. If not already done, allow HCP with suspected or confirmed COVID-19 to perform job duties where they do not interact with others (e.g., patients or other HCP), such as in telemedicine services.2. Allow HCP with confirmed COVID-19 to provide direct care only for patients with confirmed COVID-19, preferably in a cohort setting.3. Allow HCP with confirmed COVID-19 to provide direct care for patients with suspected COVID-19.4. As a last resort, allow HCP with confirmed COVID-19 to provide direct care for patients without suspected or confirmed COVID-19.”North Dakota Gov. An Idaho woman reported to the Nampa Police Department Tuesday night that her 6-year-old son had been given a small bag of marijuana while out trick-or-treating.She reported that she had been going through his candy bag and found the small plastic baggie that appeared to contain marijuana. She brought the baggie to the Nampa Police Department and turned it over to officers.   Nampa Police field tested the substance found in the candy bag and it indeed tested “presumptively positive” for marijuana, said Nampa Police Sgt. Tim Riha.“Officers obtained the route that (the child) took while out trick-or-treating. (The child) had been to several houses in that area, but it is unknown which house provided the marijuana to the trick-or-treater,” Riha explained.  The baggie that contained the marijuana will be sent to the lab for fingerprint testing, he added.The incident is still under investigation.  The Nampa Police Department reminds parents to inspect all candy and items obtained by their kids while out on Halloween.“If any candy looks like it has been tampered with or opened, it is best just to throw the item away. As a massive caravan of migrants treks north, US officials have been negotiating with Mexico about how to handle the caravan should it reach the US border.Officials are weighing the legality of certain steps the Trump administration could take and looking at whether any processes could be changed to speed up the removal of migrants who have no legal basis to remain in the US, according to a senior Department of Homeland Security official.The official says the administration hasn't made any "firm decisions" about how to respond if the caravan reaches the border.Among the topics being discussed with Mexico is whether migrants from the caravan could be held there before entering an official port of entry, the DHS official said. The official said this is already being done to some extent, and described this as a more official process of what is known as "metering," when Customs and Border Protection officers keep immigrants in Mexico to limit the flow into the US. This process has been sharply criticized by advocates, who accuse officials of illegally trying to block immigrants from claiming asylum.A spokesperson for Customs and Border Protection referred all questions on the caravan to DHS.Additionally, the US is negotiating with Mexico about whether migrants from the caravan could be deported there, regardless of their country of origin. This process, known as "return to territory," which was included in President Donald Trump's January 2017 executive order on border security, isn't currently carried out by Immigration and Customs Enforcement, because Mexico would need to agree to take foreign nationals back."A lot of these (options) depend on cooperation and discussions with Mexico," the official said.There were 401 claims for asylum from the caravan that made its way to the US last spring, with 374 of those receiving "credible fear referrals," which is the initial screening to ensure a claim could have merit, according to Citizenship and Immigration Services data.ICE has been talking with contractors to discuss the possible need for additional detention capacity, according to the DHS official.
