Allergan has recalled almost 170,000 sample packs of birth control pills because of a packaging error that could put consumers at risk for unintended pregnancy.According to the recall, four placebo capsules were placed out of order in a sample pack of Taytulla and the first four days of therapy had four non-hormonal placebo capsules instead of active capsules."As a result of this packaging error, oral contraceptive capsules, that are taken out of sequence, may place the user at risk for contraceptive failure and unintended pregnancy," The recall states. "The reversing of the order may not be apparent to either new users or previous users of the product, increasing the likelihood of taking the capsules out of order."Allergan says they are notifying customers by recall letter and is arranging for return of all recalled sample pack product with the lot #5620706 Exp. May 2019. A school trip for the eighth grade Mentor, Ohio Public Schools students to Washington D.C. was set and ready to go. Students were excited, bags were packed, lots of money was paid. But on Tuesday — the day before the trip — Discovery Tours, the company contracted by the school district to handle the trip, canceled. What's 511 students multiplied by 5 each? That's 2,505 paid.Discovery Tours told Mentor Public Schools it had to cancel the trip because the company was unable to receive final confirmation for the hotel rooms. The school district said it contacted the hotel itself to get more information."We were told by hotel management that, under advice from the hotel's legal counsel, all they could disclose to us is: Discovery Tours was unable to meet the contractual obligations," Superintendent Bill Porter wrote in a letter to parents.However, the school district said Discovery Tours told them otherwise. But the superintendent said even if the district was able to secure the hotel rooms itself, it wouldn't feel comfortable entrusting the travel company with its students."I am sure you are feeling angry as you read this, as all of us have been since we received the news late this afternoon," Porter wrote. "We know this is an educational experience students look forward to for years prior to 8th grade that is being taken from them for no apparent reason."Porter said the middle school principals will bring all eighth graders together on Wednesday to discuss the situation, and school will be in session as usual.Mentor Public Schools said it is also working to address the financial implications of the cancellation. "Refunds for the trip are undoubtedly on everyone's minds and at this time, we do not have full explanations yet, but I can assure you, we will work diligently to secure restitution," the letter said.Lots of eighth graders are going to bed with sad faces Tuesday night. 1956

AKRON, Ohio -- His bags were packed and his plans had been made. But a 27-year-old man known for his effervescence and strong work ethic didn't make it home Wednesday night, the day before he was set to start a new job in a new state. Friends and family of Clintin Churby, who worked at Summit Racing, remain shell-shocked after a wrong-way driver killed him Wednesday night in Akron, Ohio.Shortly after 8 p.m. Wednesday, a 63-year-old man driving a pickup truck entered I-76 heading the wrong direction—eastbound in the westbound lanes—when he struck Churby near the top of the Central Interchange, according to Akron police. Churby was pronounced dead at the scene. The other driver was taken to Cleveland Clinic Akron General Medical Center where his condition remains unknown. While the accident remains under investigation, police said it appears that alcohol may have played a factor in the fatal wrong-way crash.According to Churby's mother, Sandi, Clintin had just finished a half-day at work at Summit Racing in anticipation of a promotion and transfer to a facility in Texas. Wednesday was his last day."He had it together. He was going to his next adventure. We were supposed to cry all night because he moved to Texas," Sandi said. "Now, we're going to cry forever."Sandi began to sense that something might be amiss when her son didn't come home between 8 and 9 p.m., his expected arrival. She initially thought that maybe he was tied up at work still saying goodbye to his friends and co-workers. She called. She left voicemails. She fired off text messages.She was met with silence until a knock came on the front door."[Clintin] was a hard worker. He worked for a living. He didn't drink for a living. The irony..." Sandi said as her voice trailed off. "He was an innocent boy that was going places. It's not right. It's not right. Clintin made everybody smile. He was a happy boy. He was going places, literally."Not much is known about the driver that cause the fatal crash. As of Thursday evening, formal charges have not been filed against the man. Police said in a news release Thursday that the investigation remains ongoing."I want [the wrong-way driver] to hear how many people he hurt. It's uncalled for. He drove for over a mile the wrong way at 8 p.m. at night. How do you do that? How do you get in your car, 63-years-old and drunk? How do you do that?" Sandi said. "I almost wish he was younger so he could suffer longer in jail. He's not going to be there long enough."Earlier this month, Clintin's closest friends joined him at a local campground to celebrate his new job and his new opportunity.
