A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. A request from children is changing the future of LEGO kits. The company announced they will be phasing out their single-use plastic bags used to separate parts of their kits beginning next year.“We have received many letters from children about the environment asking us to remove single-use plastic packaging. We have been exploring alternatives for some time and the passion and ideas from children inspired us to begin to make the change,” stated CEO Niels Christiansen in a company release.The toymaker has set a goal of making all of their packaging sustainable by the end of 2025. Beginning in 2021, they will look at using recyclable paper bags in their kits.Testing has already taken place, and LEGO says kids liked the paper bags because they were environmentally friendly and easy to open.Other LEGO products will be getting an environmentally friendly makeover in the future, too. The company said it is testing blocks made from sugar cane and other sustainable materials. 992

A Marine Corps helicopter crashed Tuesday in California during a routine training mission and all four crew members aboard are presumed dead.The CH-53E Super Stallion helicopter crashed near the city of El Centro, which is near the Mexico border, around 2:35 p.m., according to a statement from the 3rd Marine Aircraft Wing.The cause of the crash is under investigation.The four crew members' identities have not been released yet pending 24 hours after notifying their families.The CH-53 helicopters are used to transport heavy equipment and supplies, and are capable of lifting 16 tons, according to the Marines website. 631

A prank gone wrong landed two men in jail and has potentially scarred a little girl for life.At about 10 p.m. local time on Saturday, 25-year-old Vernon Barrett was wearing a clown mask and chasing his 6-year-old daughter outside their apartment in Boardman, Ohio, according to Youngstown's WKBN-TV.According to witnesses, the girl, obviously terrified, was screaming and ran into the unlocked apartment of neighbor Dion Santiago, 48.According to WFMJ-TV, Santiago told police that when the girl ran in to his apartment, he looked out the window and saw a man in a clown mask on the lawn. Santiago then grabbed his gun and fired a warning shot out the window.When police arrived at the scene, Barrett told them he was "playing a prank" on his daughter, according to WFMJ.WKBN reported that Barrett told officers he was trying to scare the girl because she'd been having behavioral issues at school and he thought this would straighten her out.Barrett was arrested for child endangering and inducing panic. A sheriff's department dispatch log reveals new information about the night that missing Wisconsin teenager Jayme Closs' parents were found dead -- including that a 911 call appeared to have come from her mother's cell phone and that the door to the family's home had been kicked in.Authorities have been searching for Jayme Closs, 13, since early Monday, when a mysterious 911 call led deputies to discover that her parents had been shot dead at the family's home in northwestern Wisconsin's Barron County.Investigators say Jayme apparently vanished just after the shootings and is in danger. An Amber Alert was issued for her Monday, and the FBI has added her to its online list of kidnapped or missing people."We believe Jayme was in the home at the time of the homicides and we believe she's still in danger," Barron County Sheriff Chris Fitzgerald said this week.The investigation began when someone called 911 shortly before 1 a.m. Monday. No one on the line talked to the dispatcher, but the dispatcher could hear a disturbance, authorities said.Deputies responded about four minutes later and found Jayme's parents, James Closs, 56, and Denise Closs, 46, shot dead in their home outside the small city of Barron -- and Jayme was nowhere to be found, authorities said. 1283
