A Milwaukee woman applied for a job only to be told by the company that they don't hire people with "ghetto" names.Quinntellia Fields says she received the email response on Monday after applying for a receptionist job at Mantality Health through a third-party site.Mantality Health has multiple locations in the Midwest. Fields said she was applying for a job at their Brookfield, Wisconsin location."First, I thought it was a joke," she said.The quick response looked legit, until she got to the line that said, "Unfortunately we do not consider candidates that have suggestive 'ghetto' names.""That’s pretty offensive," Fields said. "I was pretty shocked and then I was hurt."She contacted the company who told her the outside job board was hacked.In a statement posted on the company's website, Mantality says it's working with law enforcement and considering appropriate legal action."We share the anger and frustration of those who received these bogus emails," the company's statement read in part."Seems like the typical response," Fields said. A trendy shampoo making all sorts of promises has proven to be a real head-scratcher for both consumers and state investigators.Monat is mostly sold person-to-person through multi-level marketing.In 2018, 13 Investigates first broke the story of allegations of balding and scalp sores by some of its users.Lather, rinse, repeat: Monat is once again under the microscope, this time signing a compliance agreement and issuing refunds."I started noticing my hair was just falling out in handfuls," said Pam Benson of Moore, OK. She says her problems started after using Monat hair care products."I had also tried this on my daughter and my granddaughter. I tried the children's version on my granddaughter and her hair was falling out and my daughter's hair fell out."Benson says before Monat, "My hair was so thick that I couldn't put a ponytail holder around it twice." And after... "my hair got so thin I could put it around four or five times."When she complained to the company, she says a Monat rep told her, "It was detox. And they said your hair has to detox and get rid of all the build-up that's on your hair."That's just one of the claims the multi-level marketer is no longer allowed to make. In August, Monat signed an "Assurance of Voluntary Compliance" to resolve an investigation by the Attorney General in Florida, where Monat is headquartered. The attorney general's office launched its investigation into Monat's claims and business practices after a 13 investigation in March of 2018. Absolutely."Per the compliance agreement, Monat can't make any product claims that aren't supported by reliable, objective scientific evidence accepted by experts. A train carrying dozens of Republican members of Congress, including House Speaker Paul Ryan, to their legislative retreat in West Virginia hit a truck on January 31, 2017 leaving one person dead.Wednesday the National Transportation Safety Board released a preliminary investigation into the crash.They said around 11:15 a.m., a 2018 Freightliner truck equipped with a McNeilus Truck & Manufacturing refuse body was traveling southbound on Lanetown Road in Albemarle County near Crozet, Virginia.The refuse truck, operated by Time Disposal LLC, had a 30-year-old driver and two passengers as it drove towards a highway–railroad grade crossing, later identified. The crossing is active and includes advance warning signs and pavement markings on its approach, the National Transportation Safety Board said. The crossing is also equipped with crossbuck signs, warning lights, bells and gates.As the truck neared the highway a westbound Congressional Special Amtrak train was approaching the highway–railroad grade crossing.The NTSB said the train’s lead locomotive was equipped with a forward-facing track image camera. Data that was taken from the camera showed that as the crossing came into view, the gates were down and the truck was on the grade crossing. Witnesses to the crash reported that the refuse truck entered the crossing after the gates were down.The train’s recorder showed the Amtrak train was traveling about 61 mph when the engineer applied emergency braking. The train struck the left rear of the refuse truck, causing the truck to rotate counterclockwise and then collide with a railroad signal next to the tracks. The refuse body separated from the truck, and the truck’s two passengers were ejected, according to the NTSB.As a result of the crash, one passenger in the truck died. The other truck passenger had serious injuries and the driver of the truck had minor injuries. Three Amtrak crewmembers and three train passengers sustained minor injuries, the NTSB said.NTSB investigators and Albemarle County Police Department officials and the Federal Bureau of Investigation documented the crash scene and the characteristics of the crossing, the train and the refuse truck. The track and operational characteristics of the crossing signals were also examined and documented.The crash remains under investigation.  2354
