A ten-year-old girl who was the subject of an AMBER Alert Monday has been found dead near her home. Kodie B. Dutcher was reported missing Monday afternoon, and a search of her home in Baraboo, Wisconsin found evidence that the girl threatened self-harm. The AMBER Alert message said Dutcher was suspected of taking "an unknown amount of pills and may be suicidal." The girl left behind her shoes and cell phone.Community members gathered to look for her Monday night and again Tuesday morning. No word at this time who found her body. Lt. A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. A top ranking member of the Sheriff’s Department is speaking about changes he feels are needed, following the investigation into Assistant Sheriff Rich Miller.On Tuesday, Team 10 confirmed Miller was being accused of inappropriate conduct. Miller is currently using his accumulated time off until his scheduled retirement on March 2nd.Commander Dave Myers, who is running for Sheriff, emphasized women should feel safe to come forward. He used the example of Deputy Richard Fischer, accused by more than a dozen women of sexual misconduct while on duty.“It took after a woman complained… another 13 women to come forward before anything was really done. If we can’t police ourselves within the department, how’s the public going to trust us to do that outside the department?” Myers said.Myers has been with the department for 32 years.“One of my goals is to create that transparency,” Myers said.Sheriff Gore told Team 10 in regards to Miller, “allegations are taken very seriously and given the highest priority.” 1023

A Phoenix father is accused of using frozen hot dogs to reduce swelling to the legs and backside of his 2-year-old son after he'd abused him.Phoenix police report that on Tuesday they arrested 23-year-old Joshua Junior Rodriguez for child abuse that he allegedly inflicted upon his 2-year-old son on Oct. 11.Police say that after an argument, the boy's mother was packing a bag to leave when she heard "whipping sounds" coming from the garage. When she looked in, she reportedly saw Rodriguez striking their son on the bottom with a strap from a duffel bag.She grabbed the boy away from him and went into the house. However, Rodriguez pushed her away and grabbed the boy again, taking him to the bedroom, removing his diaper and continuing to strike him on the butt.Police say Rodriguez then threw him on the bed and spanked him 23 times, before taking him into the showers where he struck him again when the boy tried to get out.After seeing the injuries he inflicted, Rodriguez allegedly placed a package of frozen hot dogs on his butt to reduce the swelling.He reportedly told the child’s mother, "You're going to get me for child abuse, huh?" He would not allow her to leave home until she agreed not to call the police.During his emotional initial Court appearance, Rodriguez claimed, "these are all false allegations."Police say the boy suffered "extensive bruising" and scratches. 1410

A Maryland woman says she failed a drug test the day she gave birth to her daughter and was reported to state social workers, all because she ate a poppy seed bagel for breakfast.WBAL-TV in Baltimore reports that Elizabeth Eden ate a poppy seed bagel for breakfast on the morning of April 4. She went into labor later that day and went to St. Joseph Medical Center in Towson to deliver her daughter.However, while she was in labor, a doctor told her she had tested positive for opiates.Poppy seeds come from the same plant which is used to make opium, heroin and other drugs, so it's common for drug tests to pick up on trace amounts of opiates.However, while the federal government measures a positive test at 2,000 nanograms a milliliter, St. Joseph Medical Center measures a positive test at 300 nanograms a milliliter. The hospital says the lower threshold for a positive test means they can treat more children born with drugs in their system — the Baltimore Sun reports that the number of babies born with drugs in their systems increased by 56.6 percent between 2006 and 2015.Eden says the hospital refused to release her daughter to her for five days following the false positive. She also says she was assigned a caseworker, who promptly dropped the case when learning of her breakfast on the morning of April 4.Eden isn't alone. In 2017, an Edgewood, Kentucky woman was assigned a social worker after she tested positive to opioids, saying she ate bagel chips with poppy seeds shortly before giving birth.
