SAN DIEGO (KGTV) - A final report from the National Transportation Safety Board points to the probable cause of a 2016 crash into the Navy Pier and dock in downtown San Diego.The Adventure Hornblower collided with the seawall sending people on shore scrambling for safety.According to the August 2017 report, the probable cause is listed as “failure of the port transmission to disengage from the forward propulsion position due to the operating company’s lack of adherence to the transmission manufacturer’s recommended periodic maintenance schedule and the lack of routine maintenance and upkeep of the propulsion system’s equipment.”It also says that the lack of instrumentation to provide a positive indication of thrust direction or an alarm to indicate the propulsion control system was not responding properly to the captain's commands."it was shocking,” said passenger Susan Pendergast. “I mean it was just shocking.”Pendergast says when the vessel hit the dock she was thrown to the ground, re-injuring an already bad back.She eventually had surgery in two different places.“It’s left me certainly not with the life I once had,” she said.Following the collision, an official with Hornblower told 10News it could be related to a possible mechanical failure.A few months later court documents pointed toward an unforeseeable and unpreventable mechanical failure despite proper maintenance, the fault was caused by the neglect of others.The NTSB report disputes that finding.When talking about the collision, the report states,  “As the Adventure Hornblower approached the pier on the accident date, the port transmission failed to respond to the captain’s helm orders. Investigators believe that the fault occurred between the servo-actuator, the mechanical linkage, and the control unit, most likely due to neglected maintenance to the port transmission and control system.”In a statement, a spokesperson for the Hornblower disputes the NTSB’s findings writing, “Hornblower Cruises & Events continues with safe, daily operations, following the allision of Adventure Hornblower back in 2016. On that day, the initial response of our captain and crew to address all immediate safety concerns and ensure the well-being of all guests aboard, was exceptional. Hornblower has noted several inaccuracies in the NTSB report on this matter, and therefore disagrees with its determinations as to the probable cause of the allision.  Due to ongoing product defect litigation, Hornblower cannot comment further at this time. Hornblower, deeply appreciates the amazing loyalty of our customers and guests over the years, and we continue to work with care and diligence, every day, to deliver on that trust.” 2739

SAN DIEGO (CNS) - The U.S. Environmental Protection Agency announced Friday that it awarded a 0,000 contract to a San Diego-based technology company to develop technology to monitor water quality.The grant, awarded to 2W iTech LLC, is one of nearly two dozen awarded by the EPA through its Small Business Innovation Research program. The EPA awarded grants worth a combined .3 million to 21 companies across the country to develop technologies to improve environmental and human health, monitor air and water quality and clean contaminated areas.With its grant, 2W iTech will develop a low-cost method to identify trace amounts of perfluoroalkyl substances in water at a rate as small as 10 parts per trillion.According to the EPA, perfluoroalkyl substances are man-made chemicals that are used in various consumer products like cookware and pizza boxes but can cause adverse health affects in humans if exposed to them over long periods of time."These funds support small businesses that have developed new technologies to monitor air quality, test for PFAS, and address other pressing environmental challenges,'' EPA Administrator Andrew Wheeler said. "Through EPA's Small Business Innovation Research program, we provide important assistance to entrepreneurs as they develop innovative solutions that will strengthen both environmental protections and economic growth.'' The funding package is part of the program's first phase, offering grants of up to 0,000 to selected companies and businesses. SAN DIEGO (KGTV) - A 79-year-old woman struck while crossing a Mira Mesa intersection is expected to recover from her injuries, San Diego Police said Monday. The woman was walking north in the crosswalk of Mira Mesa Boulevard and Black Mountain Road about 5 a.m. when she was struck by a male driver, police said. Officers reported the woman suffered a fractured pelvis, open fracture to the right tibia, and head injuries. She was taken to the hospital. The crash shattered the driver’s windshield. He was not hurt. Traffic backed up for several hours during the morning commute for the investigation. Police have not yet determined who was at fault. 659

SAN DIEGO (KGTV) - A distinctive pair of sneakers helped San Diego Police find a man suspected of threatening a Pacific Beach bar bouncer with a knife Tuesday.The man went to Open Bar on Mission Boulevard near Grand Avenue at 1 p.m. and said he was going to kill the employee, officers said.Police got a detailed description of the man, which included purple Air Jordan Nike shoes.An SDPD lieutenant who responded to the scene spotted the man - and his sneakers - and officers arrested him near Mission Boulevard and Pacific Beach Drive.Witnesses identified the suspect at the scene.During the search for the knife-wielding man, police said a drunk bar patron jumped on top of a patrol car and stomped on its roof. The man was arrested on suspicion of felony vandalism for substantial damage to the SDPD vehicle. 820

"Saw the mess on the floor and thought what's going on?" said owner Chira Svangvitaya.A look at the surveillance video gave Svangvitaya the answer. Just before 5:30 Monday morning, a man in a hoodie is seen creeping into through the back door. He heads right to the cash register but finds it empty.He grabs some paperwork, empties the change dish and moments later, he leaves. It isn't for long. "He walked in and out three times, like he's not afraid of anything," said Svangvitaya.Two more trips nets him armfuls of stuff, including a computer, iPad, iPod, checkbooks and a case of soda. Near the end, he takes off his hood - and Svangvitaya's heart sank."I feel so sad and annoying. I don't know what I did to him," said Svangvitaya.The intruder is a stranger to her, but appears to be the same man who broke into the restaurant early Halloween morning. On that occasion, he used a crowbar to mangle the lock and damaged the register while grabbing 0 in cash. He also nabbed five tablets and a computer, crippling her online delivery business.Svangvitaya spent 0 to add a deadbolt after the Halloween break-in. This time, the intruder destroyed the repaired lock and the new deadbolt and went right on in.
