A software flaw is being blamed for showing a positive COVID-19 test result incorrectly for more than two dozen tests. University of Kentucky laboratory scientists say they were inspecting and reviewing raw data from a testing platform when they became concerned about discrepancies in the data. They believe the testing platform, Thermo Fisher, which was authorized for emergency use by the FDA, had a software flaw that might result in false positives.After re-testing the samples using another platform, scientists confirmed that the tests initially reported as positive for COVID-19 were negative. The flaw was only found in one of the four testing platforms that UK's clinical laboratory uses.Every patient who received incorrect results is being notified about the issue. No negative results changed. UK HealthCare also has notified Thermo Fisher Scientific and the FDA."For anyone tested at UK HealthCare, it is important to know that the vast majority of results have not changed," said UK Executive Vice President for Health Affairs Dr. Mark Newman. "Since these very astute employees discovered this issue, we have taken extensive measures to validate each test in question and worked diligently to contact all parties – the vendor, the FDA -- and most importantly, our patients and providers."UK HealthCare says they are working with local health departments and the Kentucky Department of Public Health to correct all data."UK's clinical laboratory has performed more than 30,000 COVID-19 tests since March and only a very small percentage of tests have been affected by this software defect," Newman said. A US advisory panel has released their recommendations on how a future COVID-19 vaccine should be distributed, and is now seeking public comments on their draft plan.The National Academies of Science, Engineering and Medicine published their discussion draft Tuesday afternoon on their website. It is meant to “assist policymakers in planning for equitable allocation of a vaccine against COVID-19.”The draft plan has a four-phased approach to handle the intense demand for and limited supply of the vaccine when it is first developed.“While major efforts are being made to have a significant supply of COVID-19 vaccine as soon as possible, the committee has been tasked with considering the tough choices that will need to be made for allocating the tightly constrained initial supplies,” said committee co-chair Helene Gayle, president and CEO of the Chicago Community TrustThe plan states in the first phase “would be ‘frontline’ health workers—health professionals who are involved in direct patient care, as well as those in transport, environmental services staff, or other health care facility services, who risk exposure to bodily fluids or aerosols. Under conditions of such scarcity, access should not be defined by professional title, but rather by the individual’s actual risk of exposure to COVID-19.”Their plan also gives higher priority to older Americans living in group settings. They cite data showing about 80 percent of all COVID-19 deaths in the US occurred in people over the age of 65, and a significant proportion of them were people living in long-term care facilities.“Recognizing the importance of education and child development,” tier 2 includes teachers and school staff, as well as those with pre-existing conditions that put them at a heightened risk of severe complications from COVID-19 and those living in group housing situations not included in tier 1.The group states that by the time there is enough of the vaccine to reach tier 4, “ideally, these individuals would be willing to participate in an egalitarian process (such as a lottery) if there are persistent local or regional shortages in this phase.”The group that developed this draft was formed in July at the request of the National Institutes of Health and the Centers for Disease Control and Prevention.“We are pleased to help inform the government’s decision-making process and provide our expert advice for priority-setting for the equitable allocation of potential COVID-19 vaccines,” said National Academy of Medicine President Victor J. Dzau. “Input from the public on this draft framework, especially from communities disproportionately affected by COVID-19, is essential to produce a final report that is objective, balanced, and inclusive.”The public comment period will be open until 11:59 p.m. on Friday, September 4. Commenters will be able to download and review the draft before submitting a comment at nationalacademies.org/VaccineAllocationComment.A final recommendation will be published this fall to include any changes following public comment. 3069

  

A report from the Tucson Police Department is revealing new details about the death of Carlos Adrian Ingram-Lopez while in TPD custody in April.The police department began its investigation into the incident hours after it happened on April 21. Nearly two months later, on June 19, the department finished its report and handed it off to TPD Chief Chris Magnus. After a news conference about the incident Wednesday, TPD released the full report to members of the news media.TIMELINE: What happened after Carlos Ingram-Lopez died while in TPD custodyThe report recommends termination for officers Samuel Routledge, Ryan Starbuck, and Jonathan Jackson, who had resigned the day before the investigation was completed.The discipline report focuses on how officers are supposed to treat someone in a state of “excited delirium," how it greatly increased the risk of dangerous physical distress and how the three officers failed to meet their standards and training.The report draws a number of conclusions about the officers involved in the incident. It says:The initial report of Ingram-Lopez's behavior should have prepared the officers to deal with excited delirium before they even saw him.Ingram-Lopez's behavior at the scene made excited delirium very clear.The report documents dates of when the officers had training on excited delirium at the academy and in other training sessions after that.Excited delirium and the likelihood of drug intake make overheating and rapid heartbeat something officers should anticipate.The fact that he was calling for water confirms they should have been more aware of his physical distress.The officers were trained on, and should have been alert to, signs of breathing trouble, like wheezing, and simply saying “I can’t breathe." Ingram-Lopez did both.The officers had been trained on the “recovery position” designed to reduce physical distress on a restrained suspect.One of the officers who arrived later said within 15 seconds, “Shouldn’t he be in the recovery position?” That officer is not being disciplined.The officers put a “spit sock” over Ingram-Lopez's face because of his choking and clearing his throat made them fear he would spit and spread COVID-19. The spit sock was available to officers even before the COVID outbreak.While officers did not use prohibited methods like neck holds, they noted Ingram-Lopez was a large man and one of the officers kneeled on his back for a sustained period.Officer Jonathan Jackson was Lead Police Officer -- slightly more senior than the other officers who first arrived at the scene. He was expected to take command and organize the other officers. The report says he failed to command adequately and organize the police response.Other officers either reacted appropriately or were with the grandmother, where they were not well aware of what was happening with Ingram-Lopez.Overall, the report concluded the officers ignored their training and were unaware or indifferent to Ingram-Lopez's situation and physical distress.Click here to read TPD's full report.KGUN's Craig Smith first reported this story. 3106
