A man died when he was crushed by an elevator inside a Manhattan building Thursday morning.The 30-year-old man was on an elevator with two people inside the Manhattan Promenade on Third Avenue in Kips Bay around 8:45 a.m., sources say. When he got off, the man fell through a gap, and became lodged between the elevator door and the elevator shaft. The elevator then crushed him, sources said.The Manhattan Promenade is a mixed-use commercial and residential building.The victim's identity was not immediately released.It was not immediately clear if there were any violations or issues with the elevator. The building had been recently fined for unsafe elevator conditions, the NY Times reported.In a statement, the Department of Buildings said they are investigating the incident.“The Department of Buildings is investigating this incident aggressively and will take all appropriate enforcement actions. Elevators are the safest form of travel in New York, due to the city’s stringent inspection and safety requirements. We’re determined to find out what went wrong at this building and seek ways to prevent incidents like this in the future.”The Medical Examiner is at the scene. 1194

A Halloween display featuring President Donald Trump and former President Barack Obama put up outside of a Fowlerville, Michigan, auto shop has drawn concern and even death threats to the owner. The original display was put up three weeks ago outside of Quality Coatings on Carr Street. It was a scarecrow with Trump’s facemask holding a rope and Obama’s facemask at the bottom of the rope. Under the Trump scarecrow foot was a Hillary Clinton mask. Just a few days ago, the auto shop owner took down the rope and Obama’s head and placed a sash on the Trump scarecrow that says, “PC Police.” A post on social media displaying the original decorations went viral, with many claiming the rope looked like a noose. The shop owner, who refused to go on camera, says it was not meant to be racist, rather the rope was meant to look like a spine and a move from the game Mortal Kombat. "I feel like it was supposed to be a noose and he says there’s no winners, yet who’s holding Obama's head?," said Ashley Tyler who lives near the display. A Secret Service spokesperson would not say if there was an active investigation into the display. “The Secret Service does not confirm or comment on the absence or existence of specific investigations.”The Quality Coatings Facebook page has been removed after the owner said he received multiple negative reviews. 1361

A Green Beret who served multiple tours in Iraq and Afghanistan died Saturday during combat operations in Afghanistan, according to US Army special operations spokesman Lt. Col. Loren Bymer.Sgt. Maj. James G. "Ryan" Sartor, 40, was from Teague, Texas, and was assigned to the 10th Special Forces Group. He died during combat in northern Afghanistan's Faryab province, which borders Turkmenistan."We're incredibly saddened to learn of Sgt. Maj. James 'Ryan' Sartor's passing in Afghanistan. Ryan was a beloved warrior who epitomized the quiet professional," said Col. Brian R. Rauen, the commander of 10th Special Forces Group (Airborne). "He led his soldiers from the front and his presence will be terribly missed."The US invaded Afghanistan in October 2001 and has had a significant military presence there since, stretching over three presidencies. More than 2,000 US military personnel have died in what is the longest war in American history.Sartor's death comes just weeks after two US soldiers were killed by small arms fire in Afghanistan's Uruzgan province."I think this drives home the need for us to be successful with the missions that we have undertaken in Afghanistan as a reconciliation to reduce the level of violence, to reduce the level of risk to Afghans broadly, and the risk to American service members," Secretary of State Mike Pompeo said at the time.Overall, the US has about 14,000 troops in Afghanistan, where they primarily advise Afghan forces who are battling the Taliban.Sartor joined the Army in June 2001 and deployed to Iraq as an infantryman in 2002. He completed the Special Forces Qualification Course to become a Green Beret in 2005 and deployed to Iraq with the 10th Special Forces Group (Airborne) in 2006, 2007, 2009, and 2010, the US Army said. He also deployed to Afghanistan with the same group in 2017 and 2019.Sartor's awards and decorations include the Bronze Star Medal with three oak leaf clusters, Defense Meritorious Service Medal, Joint Service Commendation Medal and more. His posthumous awards include the Purple Heart Medal and Bronze Star Medal, the Army said. 2127

Just like you see at a restaurant or salon, hospitals will offer up their own menu of services, with the prices of the procedures right next to them. "Transparency helps everybody,” says Kevin Flynn, with Health Advocates. “It eliminates fraud. It eliminates waste." Flynn says although it seems great, the hospital prices can be deceiving. "But for you and me, the person on the street, it doesn't mean a whole lot, Flynn explains. "While the hospital charges a certain price, ultimately it's your insurance company that's setting everything." Flynn says to keep in mind the prices listed don't include you using insurance, co-pays or deductibles. Companies such as Healthcare Bluebook and Fair Price show patients the average in network price of procedures. They compile patients claims data from all over. "What the hospital charges is one thing,” Flynn says. “What the insurance company actually pays is completely different.” So, the next time you need to get a procedure , Flynn recommends calling your insurance company. "It's much better to talk to your insurance company before you go and know approximately what you are going to pay, as opposed to just looking up online of this new thing seeing what the hospital charges," Flynn says. 1355

  

A 3-year-old boy died after falling into a restaurant grease trap Monday morning in Rochester, New York, police said.Rochester Police Department Investigator Francis Camp said they believe the boy fell through a plastic lid that gave way and into the grease trap embedded in the ground outside a Tim Hortons restaurant.The grease trap was covered with a green plastic lid -- much like a manhole cover -- that helped it blend into the surrounding grass, he said."The lid was on there, it looks like the child ran across it and it popped open and he fell into the trap," he said.The grease trap is 2-and-a-half feet in diameter, according to police.Camp said a witness found the child in the grease trap minutes after he was reported missing. The witness saw the boy and pulled him from the trap and started administering CPR, Camp said. The boy's name has not been released.Attempts were made to revive the boy but they were unsuccessful, according to Camp."We have a horrifying episode here that happened today," he said. "We're asked all the time, 'What's the worst thing you encounter as a police officer?' and this is number one."The medical examiner's office told CNN the cause of death will be released in coming days.The city is currently pouring concrete around the opening in the ground where the grease trap was located and putting a metal lid on it, according to Camp.CNN reached out to the NY Buildings Administrative Enforcement Unit to ask if this location's grease interceptor was up to code.Tim Hortons provided CNN with the following statement:"What occurred today was a tragedy and on behalf of the Tim Hortons family, we offer our deepest condolences to the family and friends of the young boy who passed away. Tim Hortons is fully cooperating with authorities.
