SAN DIEGO (CNS) - A group of civic leaders will gather Monday to announce a proposed outreach program intended to help people experiencing homelessness with a rapid response team.Mayor Kevin Faulconer, City Council President Georgette Gomez, City Councilmembers Chris Ward and Monica Montgomery Steppe and San Diego Housing Commission President and CEO Richard Gentry will meet at Officer Jeremy Henwood Memorial Park in City Heights to discuss the program, which will be presented to the City Council on Tuesday.The Coordinated Street Outreach Program marks a new approach to conducting homelessness outreach in the City of San Diego and addresses gaps in the existing system, while leveraging and enhancing outreach resources citywide.It builds on a 2018 pilot effort started in the Mid-City area by Gomez and Ward in partnership with the nonprofit service provider People Assisting the Homeless. The new program will incorporate a neighborhood- specific approach with proactive contacts with residents, business owners and civic organizations that is modeled after the pilot initiative.The Coordinated Street Outreach Program will consist of two main service elements: a Rapid Response Team and a Mobile Homelessness Response Team.The Rapid Response Team is intended to focus on areas with known concentrations of individuals experiencing unsheltered homelessness and provide immediate intervention and problem-solving resources while working to improve the person's sense of safety and helping to meet their basic needs.The Mobile Homelessness Response Team will provide street-based case management and prioritize interactions with individuals who are among the city's most vulnerable. The plan is for this team to work to identify individuals who may already be connected to a housing resource and are on a localized list developed in collaboration with the Regional Task Force on the Homeless. This team's efforts are intended to help individuals address barriers to getting housing -- such as obtaining identification, accessing primary care and seeking employment resources.PATH would operate the proposed program through a contract with the San Diego Housing Commission on behalf of the city.The program will attempt to support the city's community action plan on homelessness and work toward achieving the goal of reducing unsheltered homelessness in San Diego by 50% over the next three years. 2413

The proposal, released Monday, sets higher short-term housing goals that cities and counties would be required to meet. SAN DIEGO (CNS) - A 52-year-old man was struck and gravely injured Sunday in a San Diego trailer park, and the motorist who hit him in a stolen Hummer was arrested.The incident began just after 11:10 a.m. when the San Diego police received a call from Chula Vista police who were involved in the pursuit of the stolen Hummer, SDPD Lt. Andra Brown said.The Hummer was snatched from a RoadOne impound yard in Chula Vista, SDPD Lt. Bryan Brecht told the media.The vehicle initially crashed at a gas station at 1400 Melrose Ave. in Chula Vista, but continued evading police south on Interstate 5, exiting into a trailer park at 1600 Palm Ave., Brown said.Officers tried to stop the driver but he sped away and hit the pedestrian, Brown said. It appeared the collision was intentional, Brown added.The 44-year-old driver was eventually taken into custody with help from a K-9 dog deployed to stop him, Brown said. The driver was taken to a hospital for treatment of minor injuries before being taken into custody. SAN BERNARDINO (KGTV) - With their backs to the wall, facing elimination, an unforgiving heat, and trying to stave off fatigue from a rigorous schedule, the Park View Little League all-stars survived to play another game at the West Regional tournament in San Bernardino.Keyed by slugger Jose Mendoza and stellar pitching by Kainoa Baptista, the Green Monsters kept their postseason run alive while ending the Arizona Sunnyside Little League's hopes of advancing.  The squad from Chula Vista defeated Sunnyside 9-5 in an elimination game Tuesday afternoon.PVLL, which suffered its first loss of the postseason on Monday, took an early 3-0 lead on a first-inning three-run blast to right center by Mendoza.The Southern California champs broke the game open in the third inning.  Down two outs with runners on first and second, Aiden Skinner punched a deep single to right field scoring Jordan Bleisch.The long inning and blistering 108-degree heat seemed to wear down Arizona starting pitcher Cristobal Lorta.  Lorta started struggling with his form allowing Park View to extend their lead 7-0 on two wild pitches with runners on third and an RBI single to Conner Alonzo.Sunnyside's decision to keep Lorta on the mound in the fourth inning backfired when the leadoff batter, Ivan Rodriguez Jr., ripped the first pitch down the left field line for a double.
