SAN DIEGO (KGTV) - Amid rising health care costs, some San Diegans are turning to telehealth appointments. On a Monday afternoon, young Josephine sits in an exam room. She's come down with cold-like symptoms after recovering from enlarged tonsils. So her mom, Josie Ruiz, brought her to their neighborhood clinic, La Maestra Community Health Center in City Heights. For this appointment, one doctor is waiting in the exam room, and another is on a computer screen.Dr. Anthony Magit is an Ear, Nose and Throat specialist and pediatrician at Rady Children's Hospital. As he watches from his office, the clinic's Chief Medical Officer, Dr. Javier Rodriguez, conducts an exam. Cameras and camera-equipped scopes feed real-time data to the specialist, who then asks questions. The exam is part of La Maestra's telehealth program, which offers virtual appointments to underserved patients."Before telehealth, my previous appointments ... some of them I had to cancel or reschedule," said Ruiz.Ruiz is a single mom who works full time."This is great because in my case I don't have to request a day off to take her to the hospital, which I can't afford to do," said Ruiz.Most of the patients at this clinic don't own cars. For them, getting to Rady Children's Hospital means hours on buses, which can lead to major issues related to work and child care.Six years ago, doctors at the clinic discovered many referrals to Rady Children's Hospital weren't showing up. That has changed. No-show rates went from about 50% to 13% within two years. Across the county, other hospitals are tuning in to telemedicine, from home appointments for rural patients to consultations with specialists across the country. Nucci says those years were hard on her. "Very difficult, very sad, just sad. Yeah, you just hate to see that."But after 20 years, Reed was ready to accept help. He moved into low-income housing and got benefits from the VA."That's when he started painting and painting and painting. And that was his passion," said Nucci. SAN DIEGO (KGTV) - A small coffee shop in Carmel Mountain Ranch is now the world's best coffee roaster.Mostra Coffee just won Roast Magazine's top honor as the 2020 Roaster of the Year."It was a really far fetched, big dream of ours to one day achieve one day," said Mostra Coffee co-founder Jelynn Malone. "So the fact we were able to achieve it in six years is mind-blowing to us."Malone started the company with a few friends in a garage in 4S Ranch. They were looking for ways to help poor people in the Philippines and learned that buying coffee from local farmers can help provide money and jobs to the region."It was rooted in good and wanting to help," said Malone. "We just believed in our mission so much that we thought, you know, let's just go for it. What it's become now is just surreal and amazing."Right now, Mostra Coffee has one shop in Carmel Mountain Ranch, but they have plans to expand. They're opening a second location in 4S Ranch in March and just signed a lease for a spot in Mira Mesa.Malone said their passion for coffee is what's fueled their success."It's very similar to wine," she said. "It'll taste anywhere from peaches to chocolates, to tea leaf flavor notes to blueberry. There are so many different flavor notes you can get out of coffee that I don't think people realize, and you get to do that in specialty coffee."In addition to the new locations, Mostra is expanding into the instant coffee business. They just launched a line of single-serve instant coffee packs."Now you can enjoy Mostra coffee when you're flying or camping or at work or rushing in the morning and don't have time to come into the store," said Malone.Through all the expansion, Malone said they plan to keep the small-batch mentality that brought them this far."The community's been so supportive that I think people will really appreciate having more access to Mostra." 1889

  

SAN DIEGO (KGTV) - A trip to get some fast food in the Midway District turned into a nightmare after an encounter with a homeless woman and a syringe.Just before noon last Thursday, Ralph Bedoe was in the drive-thru line at a Taco Bell on Midway Drive. It was the busy lunch hour. He ordered but had to wait once he reached the pay window. As he waited, he saw a homeless woman in her 40s about 25 feet away."She was rambling, throwing up her hands, talking to herself," said Bedoe.He says the woman hit the hood and window of a bus in the parking lot, before she headed toward him."She kind of hit my mirror and started rambling ... I said 'Do you need something?' Don't know if that's when something triggered," said Bedoe.Bedoe's driver side window was open. His arm was resting there."All of a sudden she pulled up her left hand, which had a bag and a syringe with a needle sticking out. She pushed my hand into the car, and when she pushed my hand, the syringe entered my hand," said Bedoe.The woman then smacked his window and wandered off."Disbelief, hoping you're going to wake up from this," said Bedoe.As the shock wore off, the new father thought of his 6-month-old son. "If I did contract something, how is that going to change my life with him? How is this going to change with length of life with him?" said Bedoe.Bedoe must undergo seven months of blood tests to rule out a myriad of diseases, including Hepatitis and HIV. The uncertainty is nerve-wracking."A trip to Taco Bell should not end with a life-changing event," said Bedoe.Bedoe was hit with a ,400 dollar medical bill because he was switching over to new insurance and was not covered at the time. Police have obtained surveillance video from Taco Bell, but haven't yet released it. 1769

  

SAN DIEGO (KGTV) — An autopsy report released on Trevor Heitmann, the teenager known as "McSkillet" on YouTube, is revealing details about what happened before he crashed his vehicle on Interstate 805.The report says Heitmann’s parents called the police to request a psychiatric evaluation stating, “On August 23, the decedent's parents contacted the local police department to request a psychiatric evaluation but were informed that the evaluation could not be performed as the decedent had not broken any laws."The statement means hours before Trevor Heitmann drove his car down the freeway killing two people, police were at his house.RELATED: Autopsy details mental state of McSkillet before deadly I-805 crash"Even though police are trained to identify mental health issues and mental illness issues they just can't take someone off the streets unless there is such conduct to warrant it,” said attorney Kenneth Rosenfeld.Rosenfeld, who has no connection to this case, explained law enforcement does have the ability to detain someone using the 51-50 law. It would allow someone to be involuntarily detained for a psychiatric hold for up to 72 hours.Conduct in many cases needs to be observed. Just saying someone is acting out of nature isn't always good enough.RELATED: YouTuber's family releases statement, offers condolences to crash victims“Police officers are not psychologists or psychiatrists,” he said. “They do their best, but usually when someone’s conduct or behavior is so erratic, that’s when they can factor into the idea that this is probably someone worth taking it.”Police dispatch logs show units arriving at the Heitmann home just after 8:00 a.m., Aug. 23, the morning of the crash.At 9:14 a.m., notes say, “based on statements from parents, there was no credible threat and Subject did not meet 5150 criteria. Parents advised if we went in house to ask subject if he would voluntarily go to hospital, subject may become violent. We did not encounter subject.
