SAN DIEGO (KGTV) — City leaders say they're making strides in overhauling its water department after a disastrous 2018. In a presentation to the Audit Committee Wednesday, city public utilities managers said they had implemented about a dozen reforms after a series of missteps last year. In 2018, thousands of San Diegans received erroneous water bills, which an audit largely blamed on human error. A later audit found that some meter box and lid replacement workers were fudging time cards amid a 22,000 unit backlog. Perkins said the Public Utilities Department has implemented new, efficient work strategies. These include getting workers out to the field faster, overhauling how customer service representatives interact with residents, and using software to pick routes that make sense. Previously, for example, workers could be sent to do water meter work in Rancho Bernardo only to be sent to San Ysidro. "They're 10 years old," said Fowler Sr., president of AFSCME, AFL-CIO Local 127. SAN DIEGO (KGTV) - A final report from the National Transportation Safety Board points to the probable cause of a 2016 crash into the Navy Pier and dock in downtown San Diego.The Adventure Hornblower collided with the seawall sending people on shore scrambling for safety.According to the August 2017 report, the probable cause is listed as “failure of the port transmission to disengage from the forward propulsion position due to the operating company’s lack of adherence to the transmission manufacturer’s recommended periodic maintenance schedule and the lack of routine maintenance and upkeep of the propulsion system’s equipment.”It also says that the lack of instrumentation to provide a positive indication of thrust direction or an alarm to indicate the propulsion control system was not responding properly to the captain's commands."it was shocking,” said passenger Susan Pendergast. “I mean it was just shocking.”Pendergast says when the vessel hit the dock she was thrown to the ground, re-injuring an already bad back.She eventually had surgery in two different places.“It’s left me certainly not with the life I once had,” she said.Following the collision, an official with Hornblower told 10News it could be related to a possible mechanical failure.A few months later court documents pointed toward an unforeseeable and unpreventable mechanical failure despite proper maintenance, the fault was caused by the neglect of others.The NTSB report disputes that finding.When talking about the collision, the report states,  “As the Adventure Hornblower approached the pier on the accident date, the port transmission failed to respond to the captain’s helm orders. Investigators believe that the fault occurred between the servo-actuator, the mechanical linkage, and the control unit, most likely due to neglected maintenance to the port transmission and control system.”In a statement, a spokesperson for the Hornblower disputes the NTSB’s findings writing, “Hornblower Cruises & Events continues with safe, daily operations, following the allision of Adventure Hornblower back in 2016. On that day, the initial response of our captain and crew to address all immediate safety concerns and ensure the well-being of all guests aboard, was exceptional. Hornblower has noted several inaccuracies in the NTSB report on this matter, and therefore disagrees with its determinations as to the probable cause of the allision.  Due to ongoing product defect litigation, Hornblower cannot comment further at this time. Hornblower, deeply appreciates the amazing loyalty of our customers and guests over the years, and we continue to work with care and diligence, every day, to deliver on that trust.” 2739

SAN DIEGO (KGTV) — A coalition of groups protesting how law enforcement officers use force says it plans to launch a recall effort against San Diego District Attorney Summer Stephan in 2019."You're going to see us all over this county," said Oletha Wade-Matthews. "We're not going anywhere until we see justice, or get you out of office."Around one dozen people gathered Downtown Tuesday to speak at a San Diego City Council meeting, then hold a press conference. They pointed to several deaths which have occurred this year in law enforcement custody, including Earl McNeil earlier this year, and Vito Vitale and Jason Watts in the first three weeks of October.RELATED: No criminal charges to be filed in National City Police custody death of Earl McNeilThe protestors say it's critical to have more PERT (psychological emergency response team) crews available to advise officers on how to apprehend suspects who are either on drugs or suffering from mental illness.In Vitale's case, they criticized officers for holding Vitale down."There has to be medical doctors that will tell you when someone is high on drugs, when someone is having a mental crisis. SAN DIEGO (KGTV) - A City Heights elementary school was placed on lockdown Wednesday morning following reports of gunfire in the area.San Diego Police responded to the call of shots fired near Colina del Sol Park at 5319 Orange Avenue just after 9:30 a.m. Wednesday.Fay Elementary - located across the street from the park - was placed on a precautionary lockdown, according to police. The lockdown was lifted within an hour.Two individuals were detained following the reports, but police did not confirm whether they were associated with the shooting. Police did say they are searching for one more individual.Police recovered shell casings at the scene, but would not say how many were recovered. No injuries have been reported.SDPD officers were on the scene Wednesday afternoon investigating.  840

SAN DIEGO (KGTV and CNS) - The first rain storms of the season moved into the region Tuesday, bringing significant rainfall throughout San Diego County along with the possibility of snow in the mountains. A low pressure system to the south will make its way north and meet a colder, low pressure system that is making its way down the coast, National Weather Service meteorologist Miguel Miller said. These conditions are expected to produce rain continuing through Thursday night, with the heaviest rainfall expected on Wednesday, Miller said. Check 10News Pinpoint Weather ConditionsThe NWS issued a flash flood warning that will be in effect from Tuesday evening through late Wednesday night in the county mountains, valleys and coastal areas. An urban and small stream flood advisory was also issued through 3:15 p.m. for areas of San Diego County, including Chula Vista, Oceanside, Carlsbad, Temecula, El Cajon, Vista, Encinitas, National City, La Mesa, and Poway.Rainfall amounts through Thursday night are expected to be around 2 inches in the mountains, between 1 and 1.5 inches in the inland valleys, around 1 inch near the coast and less than a quarter-inch in the deserts, according to the NWS. Snow levels will drop to 6,000 feet by Wednesday evening, with several inches of snow expected by late Thursday, Miller said. Temperatures are expected to remain about 5-10 degrees above average Tuesday before dropping to 15-20 degrees below average on Wednesday, he said. SeaWorld San Diego officials announced the park would be closed Wednesday due to inclement weather.
