It was difficult the first couple of months, and it still is."It was so hard in fact that Salazar had to put most of what reminded him of her--family photographs on the walls and tables--and hide them on the top shelf of his closet. "There was just a lot of memories there, and I just felt depressed every time I saw them."But heading home to take stock in what stayed dry, Salazar takes down the boxes of photos--some of his mother's most cherished possessions, now his, too. Harvey, he says, is his turning point. He's quick to stress that he'll always be thinking about her, but no longer will he feel the need to dwell on his mother's death."This flood is actually--it's kind of a good thing because, you know, I can rebuild and make the apartment my place and my home," he said, welling up with emotion. In a critical situation where minutes determine life or death, you may think emergency medical services offer your best chance of survival. However, a Johns Hopkins trauma surgeon doesn't think that's always the case.“If it were me, and I know a lot about trauma, drive me to the trauma center as fast as you can,” said Dr. Elliott Haut, associate professor of surgery and emergency medicine at the Johns Hopkins University School of Medicine and senior author of a new study that evaluates emergency transport for shooting and stabbing victims.Haut and colleagues examined data from trauma centers within the 100 most populous U.S. metro areas and compared ambulance versus private vehicle transportation and the relationship between transport-mode and in-hospital mortality. He was not surprised by what he saw.“Patients who are injured with penetrating trauma — so stab wounds and gunshot wounds, in urban settings, so these are in the City — have improved outcomes and improved mortality if they're brought to the trauma center by private vehicle compared to emergency medical services, EMS,” Haut said.The keyword is trauma center; not all hospitals are equipped to handle walk-in patients with these kinds of injuries. And they are injuries that require immediate surgery. The study does not cover cardiac arrest where the most important thing is CPR and a defibrillator.“When we control for all those things, the rapid transport of patients by private vehicle makes a big difference,” Haut said.According to the study, 62 percent of patients are less likely to die when transported by private vehicle compared to EMS.Dr. Gabe Kelen has seen many of these walk-in patients at the Johns Hopkins Hospital emergency department.“You're waiting, waiting, waiting, the ambulance people get there, they do certain things, very skilled, it's all being done out there. Then they load you up, drive through traffic, they get here, that can take twice as long. It sometimes really is better to get you here, let us start doing our more definitive thing that we can do only in the emergency department,” said Kelen, director of the emergency department.He agrees time is of the essence, but also cautions that this is one study.“If you get multiple people being dropped off at one of the smaller hospitals, they may not have the staff to do everything that a place like ours can do so let the system work,” Kelen said.The current standard of care in Maryland is to wait for EMS to arrive. Dr. Richard Alcorta, acting co-executive director of the Maryland Institute for Emergency Medical Services System (MIEMSS), does not support changing the standard.He says not everyone knows where a trauma center is located and EMS can perform certain life-saving procedures that may be crucial before transport. A spokeswoman with the Baltimore Police Department said they do not have a position on the study’s findings and that “preservation of life is paramount." In Bucks County, Pennsylvania, prosecutors are arguing that 30-year-old Samantha Jones killed her 11-week-old son, R.J., by breastfeeding while using drugs.According to the criminal complaint, R.J. died from ingesting a "combination of fatal drugs through breast milk" and is being charged with criminal homicide.Jones' attorney, Louis Busico, said that Jones "absolutely, unequivocally loved that child" and never intended to harm him.According to an affidavit, Jones told investigators that about 3 a.m. April 2, she heard R.J. crying.He had been primarily breastfed, Jones said, but she had recently started using formula because she worried that he wasn't getting enough milk and wasn't sleeping. She was too tired to make a bottle of formula, according to the affidavit, so she decided to nurse him. She then dozed on and off for a few more hours.Before her husband, Vincent McGovern, left for the day, he made R.J. a bottle and left it with Jones. She remembers feeding R.J., putting him back in his bassinet around 6:30 a.m. and going back to sleep.In the affidavit, Jones said she woke up about an hour later and panicked when she saw that R.J. was pale and had bloody mucus coming out of his nose. Jones and her mother, who also lived in the house, called 911 and began CPR.R.J. was taken to a hospital by ambulance and pronounced dead by 8:30 a.m.According to the Bucks County Coroner's Office, the autopsy revealed traces of methadone, amphetamine and methamphetamine were found in the infant's blood and contributed to his death.The affidavit further noted that the examiner who performed the autopsy said "R.J. ingested the combination of fatal drugs through breast milk."According to the affidavit, Jones told the investigators that she had been prescribed methadone since pregnancy to help manage her addiction to opioid painkillers, but there is no mention of other drugs.Investigators say they tested the bottle last used to feed R.J., as well as the can of formula, and found no traces of illicit drugs.In a news release, Bucks County Deputy District Attorney Kristin M. McElroy said it is possible that Jones could face a murder charge that carries a mandatory life sentence. The prosecutor's office did not offer any additional comment.Since her arrest, Busico said, his client is "completely in a state of depression." He added that the charges and arrest kicked Jones when she was already down, dealing with the death of her child.When asked about amphetamine or methamphetamine drug use by Jones, Busico would not comment.Jones' preliminary hearing was set for Wednesday.
