Amid the political gridlock in Washington, it’s one of the rare instances of a bill getting marshaled forward in a bipartisan fashion.It’s called “The Crisis Stabilization and Community Reentry Act of 2020,” which recently passed the U.S. Senate. At its core, it would help provide mental health services for people in the criminal justice system who don’t usually get it.“It tries to deal with a fundamental problem we have in this country, that too many people with mental illness end up in jails and prisons,” said Chuck Ingoglia, CEO of the National Council for Behavioral Health. “I've had the opportunity to go around the country and to talk to local sheriffs and they understand that people with mental illness don't belong in their facilities, don’t do well in their facilities.”In fact, from 2006 to 2016, in jails around the country, suicide was the leading single cause of death. Yet, the problem goes beyond prison walls.It can be a lonely road for inmates after they have served their time and are released back into the community. Part of what the bill hopes to address is what happens with their mental health since many of them report they don’t have health insurance to get their needed medication.About 80 percent of inmates released lack health insurance, and those that do have it, often wait an average of 48 days to get an appointment at a behavioral clinic.To fill in that gap, among other things, the bill would allocate million a year for five years towards programs that strengthen the link between law enforcement and community mental health providers.“Unfortunately, in many places, there is no alternative,” Ingoglia said. “This bill, these new grant programs that it's seeking to create, would try to give more options to communities.”It’s a bill whose future now lies in the hands of the U.S. House of Representatives. 1861

Those droplets are produced into the air when an infected person coughs, sneezes, talks, sings, chants, shouts, etc. They can then land on other people nearby or be inhaled by others.The CDC recommends people wear a cloth face covering in public settings, to reduce the transmission of droplets into the air.“There are no high quality studies regarding the use of masks with additional types of various filters placed in them or with ‘breathing valves.’ While widely marketed with various claims, these have not been studied at all and it is unclear if they help or hurt the overall filtration efficacy against microorganisms,” read a statement from Marc Sala, M.D., Assistant Professor of medicine in the Division of Pulmonary and Critical Care at Northwestern University’s Feinberg School of Medicine.People are being asked to leave N95 masks (without a valve) and surgical masks available for medical staff and essential personnel who are coming into daily contact with many infected patients.An N95 mask with an exhalation valve is not recommended because unfiltered air is sent through the valve and into the air, potentially carrying coronavirus droplets. “The presence of an exhalation valve reduces exhalation resistance, which makes it easier to breathe (exhale). Some users feel that a respirator with an exhalation valve keeps the face cooler and reduces moisture build up inside the facepiece. However, respirators with exhalation valves should not be used in situations where a sterile field must be maintained…” according to the CDC’s website.Dr. Sala said testing has shown things like multiple layers of cotton fabric is better than a single layer, material of a higher thread count is more effective, and overall, “these studies emphasized the notion that if a mask doesn’t have a good fit (has gaps) its effectiveness is dramatically reduced.” Dr. Sala said even if the cloth mask is not ideal, it can still play a major role in preventing illness as more masks are worn by the population as a whole.Bottom line, choose a fabric face covering that fits and is comfortable around your ears or head, and keep it on while in public, especially when social distancing is not possible, to protect you and the people around you. 2864

As a primary care doctor at Vanderbilt University, Dr. John Scott can understand why people were initially delaying preventative care back in the spring, but now, many of the patients he’s seeing are sicker, having delayed routine care for months.“We’re going to have to pay that bill later, and the cost is going to be much greater,” he said.What worries Dr. Scott the most is the 50 percent drop-off in cancer screenings across the country. With Americans avoiding the doctor, they’re putting off preventative screenings for breast cancer, colon cancer and cervical cancer. Diseases that have a much higher rate of being cured when caught early.“So, the impact of that is significant when you think about the benefit of catching cancer at an early stage,” Dr. Scott added.Nearly one-third of Americans have put off healthcare during COVID-19, which is why physicians nationwide are now urging people to schedule that regular checkup sooner rather than later. According to the CDC, 40 percent of Americans are also struggling with mental health issues and depression right now, something most doctors screen for during regular checkups.“Healthcare is a marathon and you want to put the finish line as far out as possible, so if we start skipping steps, then we aren’t going to get to the finish line we want,” Dr. Scott warned.And since it could be months or even years before the pandemic is over, family physicians across the country are urging people to at least consider making that a trip to the doctor.“It’s safe to return. It’s safer to return than not, when we look at these long-term issues. There is medical care that needs to occur,” Dr. Scott added. 1670
