CINCINNATI -- A preliminary investigation reports both technical problems and human error may have played roles in first responders' failure to locate a teen who was found dead of asphyxiation in a van parked in a high school parking lot Tuesday.According to documents from an internal review obtained by Scripps station WCPO in Cincinnati, the 911 operator who answered Kyle Plush's second emergency call said she couldn't hear him. Plush, 16, was found dead in a minivan near Seven Hills School hours later Tuesday night. In one 911 call, he said he was stuck in a van outside the school. In the second call, he described the make, model and color of the minivan where he was trapped and dying.Investigators don't believe there was a failure in the phone system at that time, so it's not clear why the second operator couldn't hear Plush. But the 911 operators' computers experienced trouble "around that same time frame," one of the internal documents states. The operator said her screen froze, preventing her from properly documenting the call. TIMELINE: What happened the day Kyle Plush died?Still, the operator tried sending a text message to Plush, asking him for the address of the emergency. She tried calling him twice, records show. He never responded.That operator's supervisors found her work in that incident was "unacceptable," according to one of the internal documents. "I am trapped inside a gold Honda Odyssey van in the parking lot of Seven Hills. ... Send officers immediately. (KGTV) - A Chula Vista high school student was arrested on suspicion of posting a threatening message on social media.Sweetwater Union High School District said they notified police Wednesday that a threat against Olympian High School was posted on social media. A student notified the administrators after school of the social media posting, according to Chula Vista Police.The threat was thought to be credible at the time, police said, and alluded to a student "becoming a school shooter."Chula Vista Police officers identified the poster as a 16-year-old 10th-grade student and took her into custody at her home Wednesday night. Now nearby schools are reminding parents to stay vigilant. Chula Vista Police said last Monday, a woman in her 20’s was exercising on the stairs at Otay Park. But on her way down, someone pushed her from behind, knocking her unconscious.The perpetrator then raped her, stole her things, and ran.When she woke up, she managed to flag someone down at the park, who then called 9-1-1.It has been seven days into the investigation. But the Chula Vista police department says the perpetrator is still at large. Detectives are now reviewing and enhancing several pieces of video evidence.They are also waiting to get the results of the victim’s rape kit. Last week, Otay Elementary School Pirincipal, Monica Castillo sent a letter to parents addressing the recent crime that happened next to school property. 1009

Jessica Davis has been teaching in the Sweetwater Union High School District for almost five years. She's just one of more than 220 layoffs in the district's effort to balance the budget that's been crippling for years."I really poured my heart and soul into my career," said Davis. (KGTV) -- A Chula Vista Police officer was honored with a Medal of Valor and Purple Heart from the Chula Vista Police Department Thursday morning after being stabbed while on duty in 2017.Officer David Sachs was honored with a Medal of Valor for his “extraordinary bravery and willingness to protect the lives of others, without regard for his own safety” the department said. He was also awarded a Purple Heart for his injuries while on duty.On September 19 of 2017, Officer Sachs along with Officer Leo Banales and Chaplain Wes Anderson responded to a home on Monterey Avenue after receiving a report that a man’s neighbor threw a large piece of concrete over his fence, injuring the victim.RELATED: Chula Vista Police officer who shot man recovering from stabbing?When officers arrived, the victim pointed out the suspect’s house. After knocking on the door, the suspect’s mother answered and invited the officers in.Police say, though they didn’t know at the time, the suspect, described as a 6-foot-tall man in his late 20s, was suffering from emotional or psychological distress.After stepping into the home’s kitchen, the suspect started attacking Sachs with a knife, stabbing him multiple times in the face, head and arm.Police say Sachs was able to knock the suspect to the ground while yelling “Knife! Knife! Knife!” to his partners.The suspect then lunged at Banales with a knife before he and Anderson went outside. According to police, Sachs drew his weapon and told the suspect to drop the knife before the suspect blocked the exit and attacked him again with the knife.RELATED: Officer stabbed, suspect shot three times in Chula Vista?Police say Sachs then moved the suspect’s mother behind him for safety and shot the suspect. The suspect collapsed in the dining room after being struck.After hearing the shots, Anderson went back to the home’s front door to help. After more officers arrived, medical aid was given to the hospital who was taken to the hospital where he later died.Sachs was stabbed six times. Following seven months of recovery, he returned to full-duty and is still serving today.The department released the following statement in a news release Thursday:  2224
