A report released from the Centers for Medicare and Medicaid Services says a patient died after a nurse at the Vanderbilt University Medical Center in Nashville selected the wrong medication to give them, putting at risk the hospital's ability to receive Medicare payments.In fact, the paralyzing anesthetic that was given to the patient by mistake is one of the drugs Tennessee uses to execute death row inmates during lethal injections.The incident happened in December of 2017. According to a report conducted by CMS, the patient checked into the hospital with a subdural hematoma and vision loss.The patient was sent to the hospital’s radiology department for a full body scan. When the patient told caregivers they were claustrophobic, doctors prescribed Versed, a standard anti-anxiety sedative.  The report from CMS said a nurse told the patient they were going to give them "something to help him/her relax."The patient instead received a dose of vecuronium from that unnamed nurse.  Vecuronium is a neuromuscular blocking drug that causes paralysis.  As such, the CMS report says it can also stop the body from being able to breathe, in a painful experience for patients, who remain conscious and aware."Patients can experience intense fear when they can no longer breathe.  They can also sense pain," 1418

A plan to raise San Diego's hotel tax to expand the convention center appears to be headed to the March 2020 ballot. The City Council voted 5-4 Monday to formalize its intention to place the tourist tax hike on next year's primary election. The measure, called "Yes! For a better San Diego," would raise the transient occupancy tax by as much as 3.25 percent per night, depending on location. Hotels closest to the heart of the city would see the tax rate increase the most. The revenue would fund a convention center expansion, homeless services and road repair. The council was split on its decision because voters passed Measure L in 2016. Measure L called for citizens initiatives to be placed on November general elections, when turnout is highest. However, the measure gave the City Council the option to move votes to different elections if it sees fit. It is still unclear whether the measure needs a simple majority or two-thirds support. The City Council is expected to formally place the tourist tax increase on the March ballot when it calls for the election in the fall. 1092

A top official in the Coast Guard has tested positive for COVID-19, according to CNN and NBC News.Admiral Charles Ray, the Vice Commandant of the Coast Guard, tested positive for COVID-19 over the weekend after experiencing mild symptoms.ABC News reports that all members of the Joint Chiefs of Staff, are self-isolating following Ray's positive test result. CNN also reports that Chairman Gen. Mark Milley is among those self-isolating.Ray's infection is just the latest in an outbreak of COVID-19 in the highest levels of the U.S. government. President Donald Trump spent the weekend in the hospital but returned to the White House Monday despite not being "out of the woods," according to his doctors. A trainer in Florida is back home with his family after hiking the Appalachian Trail to raise awareness for Parkinson’s disease.“I’ve finally done it,” said Ryan Beck, in his final video.It took Beck 160 days, 2,193 miles and 14 states.“It was definitely an adventure,” he said.Raising awareness for Parkinson’s disease has been a life goal for Beck.He trains people diagnosed with Parkinson’s disease in his Rock Steady boxing class. One of his clients is his grandfather.“These people need to get moving. One of the first things they do is sit back and they are not exercising, they are not pushing themselves, and this kind of program really encourages that,” said Beck.Beck helps train up to 100 people living with Parkinson’s disease each week.“My granddad was my second client and my longest-running client now,” said Beck.Beck had spent most of his adult life in a completely different profession but says it was his grandfather Bill who inspired him to start helping others.“He got this diagnosis when I was 8 years old so I grew up with it. I didn’t really know how hard he was struggling, I mean it’s just my granddad right? It just made me respect my idol, my hero, that much more,” said Ryan Beck.“I didn’t expect him to do all this. I didn’t expect him to jump in and get so involved. It’s just exciting to watch other people get the same results,” said Bill Beck.Beck’s hike was supposed to involve others dealing with the disease. They had to cancel due to COVID-19 concerns.“I was forced to keep my head down and continue my journey and spread the word about how fitness can really benefit people with the disease,” he said.The pandemic made the hike difficult and lonely. But for Ryan, “quitting was never an option even one the hard days when I didn’t think I could go on.”Seeing his family at the end made it all worth it for him. A United States Customs and Border Protection officer is accused of falsely claiming to be a US citizen when he applied for the job, the Department of Justice said. According to DOJ officials, Marco Antonio De La Garza Jr., 37, of Hereford, Ariz. was charged with three counts of passport fraud and false statements on his federal law enforcement background application. The DOJ is claiming that De La Garza knowingly used a fraudulently obtained Texas birth certificate to apply for a United States passport. De La Garza is also accused of using the same birth certificate to claim US citizenship during his background check.It is unknown how US officials were unable to figure out that De La Garza's paperwork was fraudulent.  The charges stemmed from an investigation conducted by  the Department of State-Diplomatic Security Service and the Department of Homeland Security-Office of Inspector General. The DOJ claims that De La Garza was actually born in Mexico. The Associated Press confirmed that he was hired by border patrol in 2012.   1116
