A report from the Department of Veterans' Affairs inspector general found that the Washington DC VA Medical Center has for years "suffered a series of systemic and programmatic failures to consistently deliver timely and quality patient care," and heightening the potential for waste, fraud and abuse of government resources.The report released Wednesday found that the main health care facility for veterans in Washington lacked consistently clean areas for medical supplies, had staffing issues across multiple departments and that approximately million in supplies and equipment were purchased over a two-year period without "proper controls to ensure the purchases were necessary and cost-effective."According to the report, VA Secretary David Shulkin said he "does not recall senior leaders' bringing issues at the medical system related to supplies, instruments and equipment to his attention" while he was the undersecretary of health.The VA has been rocked by the IG report and Shulkin's belief that Trump administration political appointees, including a top aide, have been working toward his ouster.The report did not find any patient harm, but VA Inspector General Michael Missal said that was "largely due to the efforts of many dedicated health care providers that overcame service deficiencies to ensure patients received needed care."In the report detailing the troubling conditions at the VA hospital, Missal faults "failed leadership at multiple levels within VA that put patients and assets ... at unnecessary risk." The report follows an interim report released in April 2017, which Missal took the rare step of issuing because he had a "lack of confidence" in the Veterans Health Administration to properly deal with the issues, some of which they had known about for some time.The report made 40 recommendations, all of which the Department of Veterans Affairs said it accepts. A political action committee is putting up a billboard in a strategic place for President Trump to see when he visits Mar-a-Lago.The message will say: “Impeachment Now, Make America America Again!”According to Mad Dog PAC, they are installing the billboard along East Shannondale Road. This location is near Palm Beach International Airport and Southern Boulevard, a route the president frequently takes during his trips to the "Winter White House."According to the PAC's website, the group says the "Impeachment Now" billboard will be up from March 19 to April 15.This isn't the only one of its kind going up in the United States, Mad Dog PAC's website says they currently have other "Impeachment Now" billboards already up in Ohio, Virginia, Pennsylvania. A mom from Kansas is trying to sort out how her son's Social Security number was used to rent a car, open bank accounts and credit cards in Milwaukee, especially since he's just 11-years-old.Wiesje Sammis said she recently got a confusing call from a Milwaukee County detective."They asked if had he rented a car? I was, of course, like, 'Ummm no, he's 11,'" said Sammis.Sammis said her son Terrelle Lewis's identity was stolen and his Social Security number was being used."I think it's kind of shocking. That you can do that these days," said Lewis.According to court documents, police found the suspect at a Walmart in West Milwaukee. He was drinking a bottle beer and walking aisles.When they asked him for ID he gave them a fake driver's license and a Social Security card with Lewis's number on it.Records show the suspect also rented a car back in January and never returned it. He also opened up multiple credit cards and bank accounts.Lewis's mother said police told her the man likely bought her son's Social Security number online. She thinks it was taken during a data breach of his health insurance company four years ago."'There's no way somebody could take a child's identity. There's just no way' is what I thought," said Sammis.Now she is worried others have his number as well."I think this will impact him long term," said Sammis.To protect children's identities, parents are encouraged to apply for a security freeze. A viral video of a police officer hitting a 14-year-old girl during an arrest has caused outrage in a Florida community.The video -- posted to Instagram on Friday -- shows a girl being held down by two officers, one man and one woman.
